Seizures and epilepsy in the elderly.

Recent data indicate that the incidence and prevalence of seizures and epilepsy rise substantially in the elderly. Little well-founded information is available on which to base treatment decisions. This article discusses cause, diagnostic evaluation, and antiepileptic drug pharmacology. Tentative treatment suggestions are offered, emphasizing the limits of available knowledge.

General practice--a source of specialist referral?

These then, are the roles of general practice that have been and will continue as the future unfolds. These historical as well as current and unique characteristics conclusively contradict this essay topic's assertions. In his textbook McWhinney states that "referral implies a transfer of responsibility for some or part of the patient's care". This means that specialist responsibility is shared with the general practitioner. Historically, no society in history has survived without some family structure to care for the individual. Similarly, no family will be able to survive if it has no one to care for its health. As a result of close association with the individual or family, general practice has always been central to human care and has occupied a respected and close relationship with its patients. This is separate from the role of specialists. Social change, however, will pave its own path. If the general practitioner can supply what society demands of it, then it too will survive as the family has. If all of the above roles of general practice are realistic, and we are truly patient oriented, then, inevitably, it is society who will decide what general practice will be.

Rehabilitation of elderly patients. Don't give up too quickly!

Rehabilitation means 'to restore to previous condition, rights, privileges, to attain some degree of independence (New Oxford Dictionary). The general practitioner will inevitably be involved in deciding when and where the patient will go. With increasing care, understanding and effort, it is hoped that with the above in mind, an increasing number of seemingly incapable elderly people will be helped. Just as with other age groups, an increasing number of elderly will be able to continue making a worthwhile contribution to society and to solve their own problems in ways that they choose.

Meningococcal septicaemia in a C6-deficient patient and effects of plasma transfusion on lipopolysaccharide release.

Patients whose blood is deficient in the terminal component of complement have an increased susceptibility to meningococcal infection. However, mortality from meningococcal infection is lower in these patients than in immunocompetent subjects. We studied a C6-deficient patient with meningococcal sepsis who received fresh frozen plasma (FFP). The patient's initial plasma endotoxin, C6, and terminal-complement-complex concentrations were low, but rose sharply after treatment with FFP. Samples of the patient's serum taken shortly after admission did not cause endotoxin release from Escherichia coli J5 in vitro, but endotoxin-releasing activity was restored in serum samples taken after infusion of FFP. It is possible that C6-deficient patients have reduced mortality from meningococcal infection because their serum cannot cause acute release of endotoxin from the invading organism and extensive tissue damage is thus avoided.
