[Is the ultrasonic dissector an advance? Apropos of 70 hepatectomies].

The aim of this retrospective study was to determinate whether the ultrasonic dissector (U.S.D.) is a major advance over existing methods in hepatic surgery. Between 1983 and 1989, we performed 70 hepatectomies "réglées". Twenty seven patients were operated because of benign lesions and 43 patients because of malignant tumours. Transparenchymal approach using "Kellyclasy" or digitoclasy with control of the hilar vessels was carried out 39 times. U.S.D. was used 31 times. No mortality was observed during operating time or post-operative period. The postoperative morbidity was not attributable to the use of the U.S.D. On the whole, U.S.D. has modified neither the amounts of blood loss nor the duration of hilar clamping. However the use of U.S.D. presents 3 advantages: it allows sometimes to perform hepatectomy without pedicular clamping, it makes easy the transparenchymal approach of the large vascular and biliary structures, and it is useful in hydatic cyst surgery. Although the U.S.D. is not indispensable to carry out hepatectomy, it improves intraparenchymal control of vessels and biliary ducts, making therefore hepatic resection easier.

[Diffuse urothelial tumor. Treatment by bilateral nephro-ureterectomy, cystoprostatectomy, renal autotransplantation with cutaneous trans-ileal pyelostomy. Results after 6 years].

A diffuse urothelial tumour in a solitary kidney does not necessarily require nephroureterocystectomy in order to satisfy oncological imperatives. The authors report the case of a 69-year-old man who, in an identical situation, was treated conservatively. Ex vivo surgery of the kidney offers remarkable exposure, allowing total pyelectomy followed by intra-hilar plasty of the excretory tract suitable for intestinal loop diversion. After a current follow-up of 6 years, the patient does not present any recurrence or metastasis.

[Renal transplantation in thrombosis of the inferior vena cava. Implantation of the graft vein in the portal vein].

An 8-year old girl with chronic renal failure underwent allogeneic renal transplantation with implantation of the renal vein of the graft on the portal vein, as the inferior vena cava was obstructed by thrombosis. The possible technical obstacles to renal transplantation are reviewed on that occasion, and solutions are suggested. In the presence of thrombosis of the inferior vena cava it seems necessary to determine its level and to look for a patent venous segment all the way up to the diaphragm. If no such segment is found, then the renal vein can be implanted on the portal vein.

[Radiological exploration of the inferior vena cava in nephroepithelioma].

The development of non-invasive procedures, namely ultrasonography and computed tomography, has modified the strategy for evaluating the spread of a nephroepithelioma to the inferior vena cava. When a complete study can be performed, ultrasonography is the most informative method. Angiographic studies are only performed as the second choice.

[Closed traumatic ruptures of the normal upper urinary tract].

In the context of urinary tract trauma, closed rupture of the upper urinary tract is rare, although not exceptional. It usually occurs in the region of the pyelo-ureteric junction and more often on the right side. It is more common in children, especially in boys. The diagnosis is often delayed and only made after the appearance of a urinary pseudocyst. The diagnosis depends on intravenous pyelography, ultrasound and retrograde uretero-pyelography, which should only be performed immediately prior to the operation. In the great majority of cases, the urinary tract can be simply repaired by a uretero-ureteral or ureteropyelic anastomosis, or more rarely, a ureterocalyceal anastomosis. The indications for autotransplantation are exceptional and are essentially based on the associated lesions, especially of the renal vascular pedicle. The results are very encouraging; a review of the literature reveals 64 cases of ruptured upper urinary tract whose outcome was able to be followed: 10 nephrectomies (only 3 followed failure of attempted reparative surgery) and 54 salvaged kidneys. Great progress has been made since Bienayme's report in 1962 of 40 cases in which he found only 4 successful repairs.
