Congenital curvature of the penis without hypospadias.

A series of 18 cases of congenital curvature of the penis without hypospadias is reviewed. Two types of curvature are differentiated. One type (12 cases) without any other anomaly of the penis is treated by a single-stage operation without involving the urethra. In the second type (6 cases), the curvature is associated with hypoplasia of the urethra on a more or less long portion, and it has to be treated as a concealed hypospadias.

[Bladder exstrophy. Transformation from an external diversion of the urine to an internal diversion].

It is possible to transform an external diversion (transcolic cutaneous ureterostomy) into an internal one (uretero-sigmoido-sigmoidostomy) in cases of bladder exstrophy which have not been reconstructed. This must not be done at too early an age (not before 7) and only under certain conditions : good renal function, normal upper urinary tract, no sigmoido-ureteral reflux good anal sphincter. Such an operation radically changes the life of the exstrophic who was previously handicapped by a collecting apparatus. One must assure a careful long term follow-up. The major details of technique and the results of 20 cases are described.

[Incomplete duplication of the urethra with intra-rectal opening of the posterior chanel (author's transl)].

We have described two cases of urethro-rectal fistulae: outstanding by their absence of anorectal abnormality. This type of malformation must be separated from the fistula observed in imperforated anus. We consider it as an incomplete duplication of the urethra; the posterior part ending in the rectum: this is a very rare abnormality. The treatment involves a separation of the posterior urethra from the rectum to bring it up to the preneum then continue as in an hypospadias usig the Duplay and Leveuf (Cecil) techniques. Seemingly normal urethra is hypoplastic in part or entirely.
