Bone anomalies in myotonic dystrophy.

Sixteen patients with myotonic dystrophy underwent CT examination of the skull, and measurement of bone mineral density at lumbar spine and hip by dual-photon absortiometry. The results were compared with those of 20 normal subjects of similar age and sex distribution. Hyperostosis of the calvarium, and increased bone mineral density at lumbar vertebrae were observed. One case showed basal ganglia calcification associated with hyperparathyroidism secondary to deficiency of vitamin D. In the other 15 patients, studies of calcium metabolism were normal. There results suggest the existence of generalized hyperostotic potential in patients with myotonic dystrophy.

Molecular and epidemiological study of Salmonella clinical isolates.

A survey of Salmonella infections was carried out over a 1-year period in the rural community covered by the Hospital Reina Sofía (Tudela, Spain). The 255 strains that were collected were studied by serotyping, antimicrobial resistance, and plasmid profile analysis. The predominant serotype was S. enteritidis (85.90%), followed by S. typhimurium (7.06%) and S. virchow (2.36%). Only 7.84% of the strains were resistant to antimicrobial agents. The most common resistance was to beta-lactam antibiotics. This resistance was due to the presence of one of two types of beta-lactamases, TEM-1 or TEM-2. Resistance to kanamycin was associated with the synthesis of a 3'-O-phosphotransferase. The resistance to streptomycin and chloramphenicol was either not enzymatic or was due to a 3"-O-phosphotransferase and a chloramphenicol acetyltransferase, respectively. Analysis of total plasmid DNA content revealed the presence of plasmids in 96.08% of the isolates. According to their plasmid profile, the strains could be classified into different groups. The three main groups, which accounted for 50.19, 20.78, and 4.70% of the isolates, respectively, corresponded to the antimicrobial-susceptible S. enteritidis serotype. These results suggested that plasmid profile analysis in conjunction with antimicrobial resistance determination can be useful for subtyping resistant Salmonella isolates.

[The complications of radical hysterectomy in the treatment of cervix uteri carcinoma].

Complications of radical surgery in 60 cases of cervicouterine cancer are presented with the purpose of evaluating the morbi-mortality associated with this treatment. Radical hysterectomy with pelvic lymphadenectomy was the most frequent surgery. Intraoperative complications reached to 13.3%; the most frequent was the vascular venous lesion. Minor postoperative complications reached to 15% and majors to 6.7%. There was no mortality associated with the treatment. Five year survival for this series was 91% for FIGO Stage IB and 86% for FIGO Stage IIA.

Thrombocytopenia associated with carbamazepine: a case series.

Carbamazepine is increasingly being used in the acute and maintenance treatment of patients with bipolar disorder. Thrombocytopenia represents a serious adverse effect of carbamazepine with which clinicians need to be familiar. The authors describe the course of thrombocytopenia associated with carbamazepine in four patients with bipolar disorder. In all cases, thrombocytopenia appeared 14 to 16 days after the initiation of carbamazepine; also in all cases the platelet count completely recovered within 7 days after the carbamazepine treatment was discontinued. Thrombocytopenia secondary to carbamazepine appears soon after the initiation of treatment and is rapidly followed by recovery after drug discontinuation.

[Platelets and migraine. Correlation between biochemical markers of platelet aggregation and serotonin].

The correlation between the biochemical markers of platelet activation and serotonin in migraine was investigated. The beta-thromboglobulin (beta-TG), platelet factor 4 (PF-4) and serotonin levels in 105 patients with migraine were measured. The results confirmed an increase in beta-TG and PF4 levels and a reduction in serotonin levels during the migraine attacks. However, we did not find a significant correlation between these fluctuations. These findings suggest the existence of extraplatelet mechanism in the pathophysiology of migraine.

[Insulinoma and pregnancy. Clinical case].

A 25 year old woman developed severe episodes of hypoglycemia during the first weeks of pregnancy. Insulin blood levels of 53 uU/ml and Peptide C levels of 6.5 ng/dl were shown associated to a blood glucose level of 34 mg/dl. Echotomographic examination of the abdomen was unrewarding. Surgical exploration revealed a 1.5 cm tumor located at the junction of the body and tail of the pancreas, which was removed. Hypoglycemia disappeared and pregnancy continued uneventfully ending in a normal delivery at 40 weeks. The newborn was normal and no motor or psychologic abnormalities have been noted during follow up to 2 years of age.

[Serotypes of Salmonella enterica in sewage effluent from Zaragoza. Comparison with clinical isolates. 1982-1989].

Between 1982 and 1989, 560 Salmonella enterica strains belonging to 63 serovars, were isolated from Zaragoza urban sewage. During the same period of time there were 45 different serovars isolated from faeces of patients from a hospital (in the same city). Only a reduced number of serovars comprised the majority of the human and environmental isolates. An approximately rectilinear relationship is shown between the isolated strains number and the number of different serovars, in non human Salmonella strains. In clinical isolates, we have not found this relation. We have compared the local results with those reported in other different Spanish regions.

[Transient block of the brachial plexus after catheterization of the subclavian vein].

Two cases are reported of brachial plexus anaesthesia and palsy after catheterization of the subclavian vein. This very rare complication occurred after several unsuccessful attempts to catheterize one or both subclavian veins. This was due to inexperience, with insufficient knowledge of clinical anatomy, infiltration with unnecessarily high doses of local anaesthetic, and accidental puncture of the brachial plexus by an infraclavicular approach. One of the patients also had iatrogenic pneumothorax.

Resistance to apramycin in two enterobacterial clinical isolates: detection of a 3-N-acetyltransferase IV.

Considering the possible role of farm animals in the contamination of human consumers by plasmid-mediated apramycin-resistant enterobacteria strains, this type of resistance should be tested more systematically in human isolates. Very recently we isolated in Zaragoza one apramycin-resistant Escheria coli strain obtained from the blood of a hospitalized patient; this clinical isolate produced a plasmid-mediated 3-N-aminoglycoside acetyltransferase IV. We describe also the isolation in Madrid of one multiresistant Klebsiella pneumoniae clinical strain. This isolate harbored a single plasmid and carried determinants for apramycin, gentamicin, tobramycin, hygromycin B, streptomycin, and ampicillin, which could be transferred en bloc to E. coli K-12 J62. Extracts from donor and transconjugant strains carrying pUZ6776 plasmid produce acetyltransferase activity AAC(3)-IV and double phosphotransferase activity (HPH and APH(3'')).

[Hormonal profile and changes in platelet aggregation in menstrual migraine].

We evaluated the hormonal profile and the platelet function in a group of 20 patients with menstrual migraine and a control group of 10 healthy women. Plasma levels of estradiol, progesterone, FSH and LH, as well as betathromboglobulin, platelet factor 4 and platelet serotonin were measured in different phases of the menstrual cycle. The hormonal profile did not show significant differences between both groups. The patients with menstrual migraine had higher BTG, FP4 and serotonin levels in the premenstrual phase than the control group. The results of this study show that in premenstrual migraine a platelet hyperagregation develops during the premenstrual phase. This abnormality is not directly correlated with the hormonal fluctuations observed during the menstrual cycle.

Gonadotropins and prolactin serum levels during the perimenopausal period: correlation with diverse factors.

We studied 490 women aged 35 to 55 years randomly selected from the urban population of León, México. The mean age for the onset of menopause, calculated with a probit regression was 48.5 years. The median of luteinizing hormone (LH) and follicle-stimulating hormone (FSH) increased with the rate of menopause women, but the 10th percentile for both hormones did not increase above 15 IU/L in older groups. Meanwhile, prolactin did not change. In menopause women FSH correlated positively with cigarette smoking, and negatively with body mass index. Luteinizing hormone had negative correlation with the number of pregnancies. Most symptoms studied were not associated with menopause or gonadotropins levels, except hot flushes, and the empty nest syndrome. It was concluded that during menopause, FSH increases with the smoking habit, and decreases with overweight.

[Platelet function in tension headache].

The possibility of abnormal platelet function in patients with tension headache was evaluated with serial plasmatic measurements of serotonin, beta-thromboglobulin and platelet factor 4. In 40 patients with tension headache we found increased serotonin, thromboglobulin and platelet factor 4 levels during the painful phases as compared with the asymptomatic phases. These data might indicate that an increased platelet aggregation plays some role in the pathogenesis of tension headache.

[Double-outlet right ventricle with mitral atresia. Study of 9 cases].

Double-outlet right ventricle with mitral atresia is an uncommon anomaly with a few cases reported in the literature. We present 9 cases of this malformation that have been diagnosed by two-dimensional echocardiography (7 cases), cardiac catheterization (9 cases) and anatomical study (2 cases). We classify them into two groups according to whether or not they have associated pulmonary stenosis. The dominant symptoms were cyanosis and hypoxemia in the first group and cardiac insufficiency signs in the other. The left ventricle was hypoplastic in eight and normal in the one with tricuspid overriding. Six of the cases had ventricular septal defect. The great arteries were in normal relationship in 4 cases, with D-malposition in 3 cases and side-by-side in 2 cases. Rashkind atrial septotomy was performed in 5 patients, but was effective only in two. Palliative surgical treatment was performed on six of them. The actual survival rate is 44%.

[Meta-analysis of the prevalence of peptic ulcer in relation to tobacco consumption].

The reported studies of the association between smoking and the incidence of peptic ulcer have had nonconclusive results. Therefore, we have carried out a quantitative review including and evaluating all the factors related to that association. To this end, the techniques of meta-analysis suggested by Rosenthal have been used, and it has been found that the type of study design acted as a moderating variable. A correlation of 0.22 was found for studies with a case-control design, and 0.07 for studies with a sampling survey design. The small power of this association can be attributed to the great variety of factors which, in addition to smoking, may play a role in the etiology and pathogenesis of peptic ulcer.

The susceptibility to amoxycillin/clavulanate of Enterobacteriaceae with plasmid-mediated ampicillin resistance: a twelve-year study of strains in one Spanish hospital.

We studied the antibacterial activity of amoxycillin, clavulanic acid and the combination, against 1210 clinical isolates with plasmid-mediated ampicillin resistance, isolated at the Zaragoza Clinical University Hospital during 12 years. MICs were determined by an agar dilution technique with an inoculum of 10(4) cfu per spot. MIC50 values of amoxycillin/clavulanate ranged from 8 to 16 mg/l. The antibacterial activity of amoxycillin/clavulanate did not show any statistically significant variation during the 12-year period.
