Effect of timolol versus propranolol on hypertension and hemodynamics.

The effect of timolol versus propranolol on hypertension, hemodynamics, and plasms renin activity was evaluated in 20 men. After two weeks of placebo, 11 men received timolol 30 to 60 mg daily, and nine men received propranolol, 240 to 480 mg daily, for five weeks in a double-blind randomized study. The 20 men then received placebo again for two weeks. Right heart catheterization was performed in all 20 patients after two weeks of the first placebo and after five weeks of timolol or propranolol. Equipotent doses of timolol and propranolol were equally effective in significantly lowering supine and upright systolic and diastolic blood pressure and heart rate recorded on an outpatient basis. Equipotent doses of timolol and propranolol caused similar hemodynamic effects including similar significant depression of cardiac index. Equipotent doses of timolol and propranolol caused similar marked depression of plasma renin activity. The hypotensive action of timolol and of propranolol was unrelated to their effect on plasma renin activity.

Effect of ethanol on angina pectoris.

We evaluated the effect of ethanol on exercise performance until angina in 12 patients in a double-blind, randomized study. The mean resting heart rate times systolic blood pressure was not changed after Fresca but was increased after 2 ounces of ethanol (P less than 0.001) and after 5 ounces of ethanol (P less than 0.01). Compared to the control periods, the mean exercise time until angina was not different after Fresca but was decreased after 2 ounces of ethanol (P less than 0.001) and after 5 ounces of ethanol (P less than 0.001). Compared to the control periods, the mean maximal ischemic ST-segment depression after angina was not changed after Fresca but was increased after 2 ounces of ethanol (P less than 0.01) and after 5 ounces of ethanol (P less than 0.001). Drinking 5 ounces or 2 ounces of ethanol decreases exercise duration until angina and increases ischemic ST-segment depression after angina.

Effect of smoking marihuana and of a high-nicotine cigarette on angina pectoris.

The purpose of this study was to determine the effect of smoking marihuana and of high-nicotine cigarettes on exercise-induced angina pectoris. Smoking 1 marihuana cigarette increased the resting product of systolic blood pressure times heart rate 54%, increased the venous carboxyhemoglobin level, and decreased the exercise time until angina 50% in 10 patients with angina pectoris. Smoking 1 high-nicotine cigarette increased the resting product of systolic blood pressure times heart rate 36%, increased the venous carboxyhemoglobin level, and decreased the exercise time until angina 23%. Smoking either marihuana or high-nicotine cigarettes decreases exercise performance until angina by increasing myocardial oxygen demand and by decreasing myocardial oxygen delivery. Smoking 1 marihuana cigarette decreased the exercise time until angina more than smoking 1 high-nicotine cigarette (p less than 0.001).

Five-year follow-up of resting and postexercise phonocardiogram in asymptomatic persons.

Phonocardiograms were recorded in 200 clinically normal adults, mean age 51 +/- 6 years, at rest and after a double Master's test. Five-year follow-up clinical data was obtained in 199 of these 200 subjects. The resting fourth heart sound correlated with the subsequent development of coronary heart disease within 5 years (p = 0.036). Neither the postexercise fourth heart sound nor the resing or postexercise third heart sound showed any correlation with the subsequent development of coronary heart disease within 5 years.

Five year follow-up of double Master's test, maximal treadmill stress test, and resting and postexercise apexcardiogram in asymptomatic persons.

The double Master's test, the maximal treadmill stress test, the resting apexcardiogram, and the postexercise apexcardiogram significantly correlated with the development of subsequent coronary heart disease within five years in 100 asymptomatic persons. The maximal treadmill stress test correlated better than the double Master's test in predicting subsequent coronary heart disease. The presence of both an abnormal maximal treadmill stress and an abnormal a-wave ratio in the postexercise apexcardiogram had the best value in predicting subsequent coronary heart disease.

Effect of sublingual nitroglycerin on ejection fraction and left ventricular asynergy in coronary disease.

After sublingual nitroglycerin, the ejection fraction increased at least 8 percent in 10 of 19 patients (53 percent), changed less than 8 percent in 5 of 19 patients (26 percent), and decreased at least 8 percent in 4 of 19 patients (21 percent). After sublingual nitroglycerin, 16 of 21 areas (76 percent) of hypokinesis improved, 2 (10 percent) did not change, and 3 (14 percent) became worse. After sublingual nitroglycerin, 6 of 6 akinetic areas did not change. After sublingual nitroglycerin, 2 of 3 dyskinetic areas did not change and one improved. Left ventriculography performed before and after sublingual nitroglycerin may be useful in predicting viable areas of ischemic myocardium.

The effect of isometric exercise on the systolic murmur of patients with idiopathic hypertrophic subaortic stenosis.

The effect of isometric handgrip exercise on the systolic murmur and bifid pulse in patients with idiopathic hypertrophic subaortic stenosis (IHSS) was evaluated. During handgrip exercise, the systolic murmur became inaudible or decreased in intensity by auscultation and disappeared or decreased in amplitude in phonocardiographic recordings in nine of nine patients with IHSS. During handgrip exercise, the bifid pulse observed in seven patients with IHSS disappeared or markedly decreased. Handgrip exercise may be helpful in the bedside diagnosis of IHSS.

Effect of carbon monoxide on maximal treadmill exercise. A study in normal persons.

The effect of breathing 100 ppm of carbon monoxide versus compressed, purified air for 1 h on maximal treadmill exericse was studied (double-blind crossover design) in 10 middle-aged, healthy nonsmokers. The mean venous carboxyhemoglobin level significantly increased from 1.67% to 3.95% after breathing carbon monoxide (P less than 0.001) and significantly decreased from 1.63% to 1.30% after breathing compressed, purified air (P less than 0.001). The mean exercise time until exhaustion significantly decreased from 697.7 to 662.7 s after breathing carbon monoxide (P less than 0.001) and insignificantly increased from 694.9 s to 703.4 s after breathing compressed, purified air. Ischemic S-T segment depression larger than or equal to 1.0 mm after exercise occurred in 1 of 10 subjects after exercise following carbon monoxide inhalation. Increased carboxyhemoglobin levels of the magnitude encountered after smoking or heavy atmospheric pollution impair exercise performance in normal persons.

Pasteurella haemolytica serotypes isolated in Northern Ireland during 1989-1991.

Pasteurella haemolytica isolates from clinical veterinary samples were serotyped over a three year period to determine the distribution of strains in Northern Ireland, to compare the results with the recorded distribution in the remainder of Great Britain and to extend information on the geographical prevalence of the serotypes. Four hundred and nine typable and 91 untypable strains were isolated from 490 animal cases. From 127 typable cattle isolates, the commonest serotype was A1, most frequently associated with calf pneumonia. Serotypes A2, T4, T10 and T15 were the commonest sheep isolates, A2 being most frequently associated with lamb pneumonia and the T-biovars more prevalent in adult pneumonia and lamb septicaemia. The isolates from approximately half the cases were evaluated in terms of the results obtained from gross and histopathological post mortem examination. Approximately two-thirds of these isolates were associated with significant pathology, including a small number of serotypes which are not included in the sheep vaccines currently used in Great Britain.
