Cytogenetic and FISH analysis of endometrial carcinoma.

Endometrial cancer is a common gynecologic tumor, yet reports of cytogenetic studies are few. We studied chromosomes from seven primary specimens of endometrial cancer. Six had abnormal chromosomes; five had a diploid-hyperdiploid modal number and one was triploid. One specimen had a normal karyotype. Chromosome 1 was frequently involved in abnormalities (five tumors) with i(1q) in two tumors, and one tumor each had der(7)t(1;7)(q12;p11) and +add (1)(p13). One additional tumor had trisomy 1 in the single cell which could be fully analyzed. Trisomy 7 was noted in two tumors, and trisomy 10 in one. Because trisomies of these chromosomes have been reported in other cases of endometrial cancer, we used fluorescent in situ hybridization (FISH) with centromere probes to determine the prevalence of trisomies 7 and 10 in these specimens. No additional tumors were found to have trisomies 7 or 10 by FISH. Our data, in combination with published literature, suggest that additional copies of 1q or portions of 1q constitute the primary change in this tumor. Extra copies of genes in this region may play an important role in tumorigenesis in endometrial carcinoma.

Dramatic increase in levels of placental protein 14 in amniotic fluid at 10-15 weeks' pregnancy.

Concentrations of placental protein 14 (PP14) were measured by radioimmunoassay in 103 amniotic fluid samples from 10 to 20 weeks of pregnancy. Low levels of PP14 were present at 10-12 weeks (median values 82-280 micrograms/l). After 12 weeks, the levels increased by two orders of magnitude to reach a peak at 18 weeks (median 11 mg/l). This pattern may reflect the obliteration of the extraembryonic coelom and direct apposition of the decidua with the chorio-amnion.

Hyperbaric oxygen therapy in calciphylaxis-induced skin necrosis in a peritoneal dialysis patient.

A 58-year-old white woman on continuous ambulatory peritoneal dialysis for 2 years developed calciphylaxis-induced necrotic skin lesions over both lower extremities. Despite subtotal parathyroidectomy and other conventional measures, skin lesions continued to worsen. Mapping of transcutaneous oxygen pressure showed markedly low values in involved areas. Skin ulcers completely healed after 38 sessions of hyperbaric oxygen therapy. The results in our case indicate that hyperbaric oxygen therapy may be useful in the treatment of skin ulcers secondary to calciphylaxis.

Generic immunoassay of corticosteroids with minimum pre-treatment of urine samples.

A generic, rapid and sensitive enzyme linked immunosorbent assay (ELISA) test has been developed which allows large-scale simultaneous testing of synthetic corticosteroids viz., flumethasone, dexamethasone and betamethasone. This assay can be directly applied to diluted urine samples (1 + 9) without hydrolysis of glucuronide or sulfate conjugates or any other treatment of samples. The polyclonal antibody was obtained by immunizing sheep with a flumethasone derivative linked to human serum albumin. This polyclonal antibody displayed high-reactivity with several synthetic corticosteroids whilst endogenous corticosteroids such as cortisol gave very low cross-reactivity (< 0.5%). Sensitivities obtained in this assay were 2.5, 3.1 and 12.5 ng ml-1 for flumethasone, dexamethasone and betamethasone, respectively. The ability of this assay to detect several synthetic corticosteroids was demonstrated by testing urine samples from horses to which the drugs had been administered.

A comparison of ventilatory and cardiac related changes in EIT images of normal human lungs and of lungs with pulmonary emboli.

EIT images have been recorded from the upper thorax of 10 normal subjects and from two patients with pulmonary emboli. The Sheffield Mk2 system was used to obtain the EIT images during quiet tidal breathing and the images were then analysed to extract the cardiac and respiratory related components. In the 10 normal subjects the mean measured change in resistivity during tidal breathing was 9% (SD 3%) with no significant difference in four lung regions. The mean changes during the cardiac cycle were different in the four regions, ranging from -0.9% to -2.6%. The two patients showed very different cardiac related changes from those found in the normals in the posterior lung regions. The sign of the changes was positive, whereas it was negative in the normals. The changes in the anterior lung regions were within the range found in our normal group.

Simultaneous quantitation of facial movements: the maximal static response assay of facial nerve function.

An assay is described that enables the simultaneous measurement of bilateral facial movements within zones relevant to facial nerve function. The assay allows the accurate quantitation of movement of the eyebrows, radix, lower eyelids, philtrum, mentum, and oral commissures relative to these points on the resting face. Global or region-specific facial nerve dysfunction is detectable using this assay, as shown in examples of patients having single branch facial nerve palsy and bilateral facial palsy (Möbius syndrome variant). Because facial movement is tested by region, with the remainder of the face relaxed, synkinesis can be detected when present. The assay has potential to be used as an adjunct to the presently used ordinal scales of facial nerve function, by allowing actual quantitation of region-specific facial movement. This feature may prove helpful in vector planning for reanimation procedures and may allow the tracking of functional responses after such procedures, thereby providing a measurement of their efficacy.

Co-morbidity of lifetime psychiatric disorder among male alcoholic patients.

The lifetime co-morbidity of major psychiatric disorders among male alcoholics was examined with the structured Psychiatric Diagnostic Interview (PDI), which was administered to 928 patients undergoing alcoholism treatment at six Veterans Administration Medical Centers. Thirty-eight percent were positive for alcoholism only; 62% fulfilled inclusive lifetime diagnostic criteria for at least one other additional psychiatric syndrome. Thirty percent satisfied criteria for one additional syndrome; 16% for two additional syndromes; 12% for three; and 4% for four or more disorders in addition to alcoholism. Depression and antisocial personality were the most frequently identified co-occurring syndromes (36% and 24%, respectively) followed by drug abuse and mania (17% each). The additional psychiatric syndromes in this sample were clearly not randomly distributed; instead, certain disorders tended to cluster together such as: drug abuse and antisocial personality; mania and depression; depression and anxiety disorder; and schizophrenia and affective disorder. Implications for classification and treatment are discussed.

Evaluation of topical ibuprofen cream in the treatment of acute ankle sprains.

One hundred patients who presented to the accident and emergency (A&E) department with an acute ankle sprain were entered into a study to determine the efficacy of topical ibuprofen cream by using a double-blind placebo controlled design in a single type of soft-tissue injury. The subjects were given either topical ibuprofen cream or a placebo cream in addition to the standard management of the department. Patients kept diaries recording walking ability and pain visual analogue scales for resting, standing and walking. A total of 51 patients returned diaries that were suitable for analysis. Patients using the topical ibuprofen cream had significant reduction in pain scores over the first 48 h of treatment.

Concentrations of vitamins A and E in amniotic fluid, extraembryonic coelomic fluid, and maternal serum in the first trimester of pregnancy.

Paired samples of amniotic fluid and coelomic fluid were obtained by transvaginal ultrasound guided amniocentesis from 15 women with an ultrasonographically normal pregnancy between 8 and 12 weeks' gestation. Vitamins A and E were measured in the two pregnancy fluids and in maternal serum by high performance liquid chromatography with detection by ultraviolet absorption and fluorimetry respectively. Concentrations of vitamins A and E were higher in maternal serum than in coelomic fluid and were generally undetectable in amniotic fluid. All differences in concentration were significant. The vitamin E/cholesterol ratio was similar in maternal serum and coelomic fluid. No correlation was shown between the vitamin concentrations in the three fluids. These findings suggest that the coelomic cavity plays a part in the maternofetal exchange of these vitamins.

The Functional Independence Measure for Children (WeeFIM). Conceptual basis and pilot use in children with developmental disabilities.

Few tools are available to pediatricians for tracking and monitoring disability status in children. We describe the conceptual basis and pilot use of the Functional Independence Measure for Children (WeeFIM). Our pilot use of this instrument in children with limb deficiency, Down's syndrome, spina bifida, cerebral palsy, and extreme prematurity demonstrates that the WeeFIM is a valid measure for tracking disability in preschool age and middle childhood. The WeeFIM measures the impact of developmental strengths and difficulties on independence at home, in school, and in the community. This allows the pediatrician to prioritize interventions for enhancing comprehensive functional outcomes and supporting families.

Older adults' perceptions about wellness.

This article describes a qualitative investigation regarding the usefulness of Pender's health promotion model with community-based, older adults. Specifically, the beliefs about wellness of elderly clients seen during a community outreach, health promotion project are delineated. The majority of the 33 clients interviewed stated they were not well and that their physician kept them informed regarding their health status. As part of the nurse-client termination process, clients were asked three questions related to themes that emerged during the home visits: health status, health care responsibilities, and perception of wellness. Findings did not support Pender's health promotion model assumption that individuals who engage in health promotion activities perceive their own health status positively.

Cisplatin-based chemotherapy for ovarian germ cell malignancies: the Australian experience.

PURPOSE: This study was performed to evaluate the Australian experience with cisplatin-based treatment of ovarian germ cell tumors (OGCT) with respect to survival and toxicity of treatment. PATIENTS AND METHODS: A retrospective review was undertaken based on a standardized questionnaire, which was sent to all major gynecologic oncology centers in Australia. RESULTS: Data on 58 patients were obtained. Overall survival at 5 years for all patients was 87%. There was one death from disease among 14 patients with dysgerminoma, and four deaths from disease among 44 patients with nondysgerminomas. Cisplatin-based chemotherapy was associated with a low incidence of serious complications, with only one treatment-related death (from bleomycin-induced respiratory failure). CONCLUSION: Our large series demonstrates that cisplatin-based chemotherapy is highly effective for patients with OGCT. Although direct comparisons cannot be made, the survival of our patients with advanced tumors was comparable to that seen in male germ cell tumors, rather than inferior as is commonly believed. Future studies should aim to refine treatment to minimize toxicity, while further increasing curability.

A new model of diffuse brain injury in rats. Part I: Pathophysiology and biomechanics.

This report describes the development of an experimental head injury model capable of producing diffuse brain injury in the rodent. A total of 161 anesthetized adult rats were injured utilizing a simple weight-drop device consisting of a segmented brass weight free-falling through a Plexiglas guide tube. Skull fracture was prevented by cementing a small stainless-steel disc on the calvaria. Two groups of rats were tested: Group 1, consisting of 54 rats, to establish fracture threshold; and Group 2, consisting of 107 animals, to determine the primary cause of death at severe injury levels. Data from Group 1 animals showed that a 450-gm weight falling from a 2-m height (0.9 kg-m) resulted in a mortality rate of 44% with a low incidence (12.5%) of skull fracture. Impact was followed by apnea, convulsions, and moderate hypertension. The surviving rats developed decortication flexion deformity of the forelimbs, with behavioral depression and loss of muscle tone. Data from Group 2 animals suggested that the cause of death was due to central respiratory depression; the mortality rate decreased markedly in animals mechanically ventilated during the impact. Analysis of mathematical models showed that this mass-height combination resulted in a brain acceleration of 900 G and a brain compression gradient of 0.28 mm. It is concluded that this simple model is capable of producing a graded brain injury in the rodent without a massive hypertensive surge or excessive brain-stem damage.

Community elderly. A nursing center's use of change theory as a model.

1. Despite functional limitations, independent elders usually voice satisfaction with the quality of their lives. However, life satisfaction can have a negative impact on self-care. As a result, older adults may become complacent about personal health practices. 2. Improvement in health behaviors and self-care practices increases the probability that older persons will be able to maintain independent functioning for a longer period. 3. Change theory can be used to determine client readiness to change health behaviors. The first stage, unfreezing, involves a desire for change. During the second stage, moving, new behaviors are identified that are perceived as problem-solving approaches. During the last stage, refreezing, new behaviors are integrated and internalized, while advice, reinforcement, and support are offered by the change agent. 4. Health screening alone is insufficient to create the discomfort necessary for change; unfreezing does not occur and health attitudes and behaviors remain unchanged. As valuable as health screening sessions are in raising participants' awareness, the study reported on suggests that it is the combination of health screening and nursing visits over time that creates the atmosphere in which change can occur.

The reliability and factor structure of the index of spouse abuse with African-American women.

This study investigated the reliability and validity of the Index of Spouse Abuse (ISA) when used with a sample of 504 African-American women. The initial factor validity analysis for the ISA (Hudson & McIntosh, 1981) resulted in two subscales: a physical abuse scale (ISA-P) and a nonphysical abuse scale (ISA-NP). Factor analysis with this sample of African-American women revealed three factors instead of two. Coefficient alpha estimates of reliability were over .90 for the original ISA subscales and with the African-American sample. The discussion compares and contrasts the factor structure observed with the sample of African-American with the factor structure reported in the original validation studies. The study demonstrates the need to critically evaluate the extent to which instruments developed to measure spouse or partner abuse are valid and reliable when used with diverse groups that may not have been represented in the samples used for initial instrument development and validation work.

A comparative study of signal versus aggregate methods of outcome measurement based on the WOMAC Osteoarthritis Index. Western Ontario and McMaster Universities Osteoarthritis Index.

OBJECTIVE: To compare signal versus aggregate measurement strategies using the VA3.0S version of the Western Ontario and McMaster Universities (WOMAC) Osteoarthritis (OA) Index. METHODS: Seventy patients with OA of the knee were asked to identify a signal item for each of the 3 dimensions of the WOMAC OA Index at baseline and termination of a 12-week, double blind, randomized, controlled trial. RESULTS: The signal method detected statistically significant alterations in health status at relatively small sample sizes and with a relative efficiency close to or at unity. In addition to a low prevalence of deterioration in nonsignal items, we observed some inconsistency in signal selection. CONCLUSION: Signal methods of measurement may provide an alternative approach to outcome measurement provided issues of nonsignal deterioration and the consistency of signal selection can be addressed.
