Vascular endothelial growth factor/vascular permeability factor is an autocrine growth factor for AIDS-Kaposi sarcoma.

Kaposi sarcoma (KS) is the most common tumor associated with HIV-1 infection and develops in nearly 30% of cases. The principal features of this tumor are abnormal vascularization and the proliferation of endothelial cells and spindle (tumor) cells. KS-derived spindle cells induce vascular lesions and display enhanced vascular permeability when inoculated subcutaneously in the nude mouse. This finding suggests that angiogenesis and capillary permeability play a central role in the development and progression of KS. In this study, we show that AIDS-KS cell lines express higher levels of vascular endothelial growth factor/vascular permeability factor (VEGF/VGF) than either human umbilical vein endothelial cells or human aortic smooth muscle cells. AIDS-KS cells and primary tumor tissues also expressed high levels of Flt-1 and KDR, the receptors for VEGF, while the normal skin of the same patients did not show any expression. We further demonstrate that VEGF antisense oligonucleotides AS-1 and AS-3 specifically block VEGF mRNA and protein production and inhibit KS cell growth in a dose-dependent manner. Furthermore, growth of KS cells in nude mice was specifically inhibited by VEGF antisense oligonucleotides. These results show that VEGF is an autocrine growth factor for AIDS-KS cells. To our knowledge, this is the first report that shows that VEGF acts as a growth stimulator in a human tumor. Inhibitors of VEGF or its cognate receptors may thus be candidates for therapeutic intervention.

IL-4R expression in AIDS-KS cells and response to rhIL-4 and IL-4 toxin (DAB389-IL-4).

Interleukin-4 (IL-4) is a pleiotropic cytokine affecting growth and differentiation of various cell types as well as regulating other cytokines. To study the effect of IL-4 on AIDS-related Kaposi's sarcoma (AIDS-KS) cells, we first examined the tumor cells for IL-4 receptor (IL-4R) expression. KS cells express a single 4 kB IL-4R-specific mRNA and 1828 +/- 408 high affinity IL-4 binding sites per cell with a dissociation constant (Kd) of 154 +/- 37 pM. Addition of recombinant human IL-4 (rIL-4) minimally inhibited AIDS-KS cell growth and expression of IL-6. We then studied the effects of a chimeric fusion toxin DAB389-IL-4 which exerts cellular toxicity only on cells expressing IL-4R. DAB389-IL-4 inhibited protein synthesis in AIDS-KS cells at low concentrations (IC50 of 5 x 10(-11) M). This effect was abrogated by neutralizing antibody to IL-4 (25D2). We conclude that KS cells express a functional IL-4R and this receptor could serve as a target for novel therapy with agents such as DAB389-IL-4.

Use of a luminescent bacterial biosensor for biomonitoring and characterization of arsenic toxicity of chromated copper arsenate (CCA).

An arsenic oxyanion-inducible Escherichia coli chromosomal operon (arsRBC) has been previously identified. Construction of a luciferase transcriptional gene fusion (arsB::luxAB) showed that ars operon expression, plus concomitant cell luminescence, was inducible in a concentration-dependent manner by arsenic salts. The present study was conducted to evaluate the potential of the arsB::luxAB transcriptional gene fusion for use as a biosensor in monitoring the toxicity of arsenic compounds. Cultures from this gene fusion strain were exposed to increasing concentrations of the wood preservative chromated copper arsenate (CCA), as well as its constituents, sodium arsenate and chromated copper solution (CC). Analysis of luciferase activity revealed that the arsB::luxAB gene fusion was expressed in response to CCA and sodium arsenate, but not to the CC solution. The detection limit of arsenic was found to be 0.01 microgram As/ml (10 parts per billion, 10 ppb) and therefore well within the range of environmental concerns. A greater induction of luminescence by arsenate was observed when cells were limited for phosphate, as phosphate can act as a competitive inhibitor of arsenate ions. Our results suggest that the E. coli arsB::luxAB fusion strain has a promising future as a specific and sensitive biosensor for monitoring bioavailable levels and toxicity of arsenic near sites where CCA-treated wood has been used.

Regression estimation using multivariate failure time data and a common baseline hazard function model.

Recent 'marginal' methods for the regression analysis of multivariate failure time data have mostly assumed Cox (1972) model hazard functions in which the members of the cluster have distinct baseline hazard functions. In some important applications, including sibling family studies in genetic epidemiology and group randomized intervention trials, a common baseline hazard assumption is more natural. Here we consider a weighted partial likelihood score equation for the estimation of regression parameters under a common baseline hazard model, and provide corresponding asymptotic distribution theory. An extensive series of simulation studies is used to examine the adequacy of the asymptotic distributional approximations, and especially the efficiency gain due to weighting, as a function of strength of dependency within cluster, and cluster size.

Heel reconstruction.

From 1987 through 1990 we used the compound flap formed by the lateral leg flap, the fibula, and its surrounding soft tissues, pedicled on the distal peroneal vessels, to reconstruct heel and calcaneus defect in two patients. Both were successful and functionally good.

A phylogenetic analysis of the genus Saccharomyces based on 18S rRNA gene sequences: description of Saccharomyces kunashirensis sp. nov. and Saccharomyces martiniae sp. nov.

A phylogenetic investigation of the ascomycetous yeast genus Saccharomyces was performed by using 18S rRNA gene sequence analysis. Comparative sequence analysis showed that the genus is phylogenetically very heterogeneous. Saccharomyces species were found to be phylogenetically interdispersed with members of other ascomycetous genera (e.g., the genera Kluyveromyces, Torulaspora, and Zygosaccharomyces). The four species of the Saccharomyces sensu stricto complex (viz., Saccharomyces bayanus, Saccharomyces cerevisiae, Saccharomyces paradoxus, and Saccharomyces pastorianus) were found to be phylogenetically closely related to one another, displaying exceptionally high levels of sequence similarity (> or = 99.9%). These four species formed a natural group that was quite separate from the other Saccharomyces and non-Saccharomyces species examined. Saccharomyces exiguus and its anamorph, Candida holmii, were found to be genealogically almost identical and, along with Saccharomyces barnettii, formed a stable group closely related to, but nevertheless distinct from, Kluyveromyces africanus, Kluyveromyces lodderae, Saccharomyces rosinii, Saccharomyces spencerorum, and Saccharomyces sp. strain CBS 7662T (T = type strain). Saccharomyces spencerorum and Kluyveromyces lodderae displayed a particularly close genealogical affinity with each other, as did Saccharomyces castellii and Saccharomyces dairensis. Similarly, Saccharomyces servazzii, Saccharomyces unisporus, and Saccharomyces sp. strain CBS 6904 were found to be genotypically highly related and to form a phylogenetically distinct lineage. The recently reinstated species Saccharomyces transvaalensis was found to form a distinct lineage and displayed no specific association with any other Saccharomyces or non-Saccharomyces species. Saccharomyces kluyveri formed a very loose association with a group which included Kluyveromyces thermotolerans, Kluyveromyces waltii, Zygosaccharomyces cidri, and Zygosaccharomyces fermentati. Saccharomyces spp. strain CBS 6334T, on the other hand, displayed no specific association with any of the other Saccharomyces spp. studied, although a neighbor-joining analysis did reveal that this strain exhibited a loose phylogenetic affinity with Kluyveromyces polysporus and Kluyveromyces yarrowii. On the basis of the phylogenetic findings, two new Saccharomyces species, Saccharomyces kunashirensis (with type strain CBS 7662) and Saccharomyces martiniae (with type strain CBS 6334), are described.

A method for more efficient source localization of interstitial implants with biplane radiographs.

The conventional method for source localization in an interstitial ribbon implant by means of biplane radiographs can be difficult, especially when a large number of seeds are involved. We present a new algorithm for more efficient source localization with the same conventional biplane radiographs. The method does not require a one-to-one source correspondence between two radiographs. The user needs only to digitize several points, following the shape of each ribbon from both films. The points that are digitized do not need to be the location of the seeds, and they do not have to correspond to the same points on both films. The algorithm uses the multidimensional minimization method to reconstruct the three-dimensional locus of the ribbon. The location of each seed is then determined by its pathlength relative to the corresponding starting point. We have used phantom experiments and clinical cases to test the reliability of the algorithm. The results show that the errors in the determination of seed locations are less than 2 mm, and the efficiency in source digitization for data entry can be increased by a factor up to 5.

Retinoid antagonism of NF-IL6: insight into the mechanism of antiproliferative effects of retinoids in Kaposi's sarcoma.

All-trans-retinoic acid (RA) is active in the treatment of Kaposi's sarcoma (KS), and retinoids inhibit KS cell growth in vitro. To understand the mechanism of retinoid action in KS, we studied the expression of autocrine growth factors of KS cells after RA treatment. We demonstrate that RA and its synthetic analogs inhibit the proliferation of KS cells by inhibiting the mRNA and protein levels of interleukin-6 (IL-6), an autocrine growth factor for KS cells. We further demonstrate that nuclear retinoid receptors (RA receptors [RARs] and retinoid X receptors [RXRs]) inhibit IL-6 promoter action by antagonizing the enhancer action of NF-IL6, a basic domain leucine zipper transcription factor belonging to the family of CAAT enhancer binding proteins. Furthermore, RARs and RXRs do not bind in vitro to an NF-IL6 binding site. However, the secondary folded structure of the DNA binding domain of RAR and RXR is obligatory for inhibiting NF-IL6 activity. Thus, NF-IL6 is a potential therapeutic target for the treatment of KS. Finally, using receptor-selective synthetic retinoids, we demonstrate that NF-IL6 antagonism and transactivation are separable functions of RAR alpha, thus indicating that synthetic retinoids with properties of NF-IL6 antagonism but lacking transactivation capabilities can be synthesized. Such retinoids might increase therapeutic potential in KS.

The prevalence and severity of white matter lesions, their relationship with age, ethnicity, gender, and cardiovascular disease risk factors: the ARIC Study.

White matter lesions (WMLs) detected by cerebral magnetic resonance imaging (MRI) are putatively a consequence of cerebral hypoperfusion or ischemia. We investigated the prevalence, severity and correlates of WMLs in a population-based sample of 1,920 African-American and European-American men and women aged 55-72 years, during the second follow-up examination of the Atherosclerosis Risk in Communities Study. The spin density images from 1.5-tesla MRI scans were used to define WMLs using a 0-9 scale with 0 for normal and 9 for most severe WMLs. Age was positively associated with the prevalence (percent) and severity of WMLs. African-Americans had lower overall prevalence of WMLs, but a higher prevalence of relatively more severe WMLs, than European-Americans. After adjusting for age, sex, and ethnicity, WMLs were significantly associated with smoking, lower education, hypertension, systolic blood pressure, and pulse pressure, and weakly associated with diastolic blood pressure. The associations of smoking, alcohol intake, systolic and diastolic blood pressure, pulse, pressure, and hypertension were stronger in African-Americans than in European-Americans (p < 0.15 for interactions by ethnicity). This population-based MRI study documents significant relationships between several cardiovascular disease risk factors and WMLs. The findings suggest that such factors play a role in the pathogenesis of WMLs, an elements linked to hypoperfusion and/or fluid accumulation, which presumably lead to WMLs. African-Americans exhibited both a higher proportion of normal white matter and a higher proportion of relatively more severe WMLs than European-Americans.

[Study on microsatellite instability in gastric cancer].

OBJECTIVE: To evaluate the microsatellite instability (MIN) in gastric cancers (GC). METHODS: MIN was examined in 106 cases of GC, using the radioactive polymerase chain reaction-based method. RESULTS: The frequency of MIN was significantly higher in expanding type of GC as compared with the infiltrative type (P < 0.005, chi 2 test). No significant association with histologic type, pTNM stage, age and sex was found (P > 0.05, chi 2 test). CONCLUSIONS: These findings reveal that MIN might be an important contributor to the development and progress of a significant number of GC, especially the expanding type which, as acknowledged traditionally, has a favorable prognosis.

[Effects of overdose fluoride on metabolism of extracellular matrix collagen and proteoglycan in condylar cartilage].

The study was conducted to evaluate the effects of chronic fluorosis on the metabolism of extracellular matrix in the development of rabbit condylar cartilage using the technique of pathology, biochemistry and radiographs. The results demonstrated that collagen content of condylar cartilage and the bone under condylar cartilage decreased (P < 0.01), and the proteoglycar contents increased significantly during the process of fluorosis (P < 0.01). The chondroprogenitor was obviously thin, and the cells of maturation zone increased in number. All the results indicated that overdose of fluoride retarded the growth of the condyle and ramus of the mandible.

[Therapy for 29 sarcoma of the breast].

We discuss the best therapeutic method for sarcoma of the breast. The data of all cases treated from March 1964 to May 1994 were analyzed retrospectively. Of all cases, 7 died, 9 recurred, and 15 survived more than 15 years and 5 over 20 years. The follow-up time was 4 months to 25 years. Most sarcomas were pathologically proved. The best therapeutic method was operation. The sarcomas of all cases must be extensively resected including surrounding tissues. The recurrent sarcoma should be resected again. The axillary lymphnodes should not be eliminated when lymphnodes is not discovered.

[Changes of cardiac function during +Gz exposure in rabbits].

Changes of cardiac function was studied during +Gz exposure in rabbits. Seven New Zealand rabbits were anesthetized and exposed to +2, +4, and +6 Gz (each for 30s, onset rate 1G/s, with 15 min interval in between). The left ventricular systolic pressure (LVSP), peak rate of left ventricular pressure rising (+dp/dtmax) and ECG were recorded. The results showed that during +2, +4 and +6 Gz exposures, LVSP decreased by 62.96% (P<0.01), 63.34% (P<0.01) and 82.01% (P<0.01) respectively, and +dp/dtmax decreased by 58.46% (P<0.01), 53.59% (P<0.01) and 63.06% (P<0.01) respectively, and the left ventricular end diastolic pressure decreased by 67.78 % (P>0.05), 332.74 % (P<0.01) and 500.54% (P<0.01) respectively, as compared with those of control. It is suggested that cardiac function is depressed with +Gz exposure.

Seven outbreaks of trichinosis in China (1992-1996).

Seven outbreaks of trichinosis occurred in Zhengzhou (central China) from 1992 to 1996, 250 patients were diagnosed. Trichinella larvae were found in 3 of 4 portions of the pork examined and in 5 of 7 cases by muscle biopsy. There was a higher occurrence of outbreaks in winter. Young and middle-aged workers, cadres and merchants were the majority of patients, (higher in males than in females). The most important clinical manifestations were fever, eyelid edema, tiredness and eosinophilia without any gastrointestinal symptoms, rash and with less myalgia. Anti-Trichinella antibodies were detected by IF in 62.6% individuals with the history of infection. The specific antibodies were only observed in 78% patients one week after onset of disease, while the antibody positive rate was increased to 92.5%, 97.6% and 100% two, three, four and five weeks after the onset and evidently increased from 89.6% before therapy to 100% one week after therapy, and decreased to 74.6% and 23.7% one and four months after therapy respectively, which suggested that IF was a useful tool for immunodiagnosis and checking up therapeutic effect of trichinosis.

Maintaining the integrity of a Web-based medical vocabulary glossary.

Medical vocabulary databases are vital components of electronic medical record (EMR) systems. While their performance and efficiency has been extensively explored by many authors, few have dealt with the maintenance of their semantic integrity. Clinicians' preference for an optimistic system has introduced a need for monitoring and filtering proposed additions to the vocabulary tables. We propose the use of batch processing and memo-posting as means of implementing a World-Wide-Web-based Controlled Vocabulary Glossary as a monitored optimistic system.

Transcriptional regulation of the gltA and TLC genes in Rickettsia prowazekii growing in a respiration-deficient host cell.

The regulation of the citrate synthase (gltA) and ATP/ADP translocase (tlc) genes of the obligate intracellular bacterium, Rickettsia prowazekii, was analyzed in rickettsia-infected respiration-deficient G14 cells. The level of the gltA mRNAII and the tlc mRNA was much lower in the total RNA isolated from the infected G14 cells grown in 1 g/l glucose (low glucose, GL) medium than in that from infected G14 cells grown in 4.5 g/l glucose (high glucose, GH) medium. However, the level of the gltA mRNAI relative to 16 S rRNA was the same in GL and GH media. An increase in the level of the gltA mRNAII and the tlc mRNA could be observed as early as 2 hrs after shifting from GL to GH medium. We conclude that, under these experimental conditions, the tlc promoter and the gltA promoter P2, but not gltA promoter P1, were transcriptionally regulated.

[Surgical treatment of extrahepatic bile duct carcinoma].

OBJECTIVE: To evaluate the experience in the diagnosis and surgical treatment of the extrahepatic bile duct carcinomas. METHODS: 242 patients with extrahepatic bile duct carcinoma over the past 20 years was retrospectively studied. RESULTS: The origin points were carcinomas of the upper bile duct in 168, of the middle bile duct in 18, and of the lower bile duct in 56 patients. The preoperative diagnostic rates for the location and the nature of the lesion were respectively raised to 97.2% and 94.5% by combination of ultrasonography and CT. The curative resection rates for the tumors in the upper, middle, and lower bile duct over the recent five years reached to 50.0%, 50.0% and 71.4%. respectively. Follow-up of patients with curative resection showed a one year recurrent rate of 73.9% and a three year recurrent rate of 100.0% with a mean recurrent time of 9.6 months in patients with local metastasis, in contrast to 13.3%, 71.4% and 17.5 months in those without metastasis. Metastasis was mainly responsible for the recurrence. Liver or multiple organ failure, intra-abdominal infection and gastrointestinal hemorrhage were the common and serious complications. CONCLUSION: The case number of the bile duct carcinoma presented a remarkable increment tendency. Ultrasonography and CT were satisfactory enough for diagnosis. To reduce the recurrent rate, resection of the tumor together with the lymph, nervous, fatty and connective tissues in the hepatic hilus, even the right celiac ganglia, should be considered the necessary procedure. Monitoring and protecting the main organs to prevent the multiple organ failure, controlling the gastrointestinal hemorrhage and the intra-abdominal infection are important to decreasing the mortality.

[Microsurgical restoration of foot tissue defects].

From 1984 to 1994, 236 different types of traumatic defects of foot were repaired by microsurgical tissue grafting. They included simple cutaneous flap in 187 and composite flap in 49. Among the 236 different tissue flaps, vascularized flap was used in 97 and pedicled flap in 139. The 4 fore-foot and 6 heel defects were repaired by composite skeleted cutaneous grafts with scapula and vascularized febula respectively. After the follow-up from 1 to 10 years, all the grafted tissues were survived and healed well. The functions were satisfactory, and 186 patients had resumed their original works. The key to good function following repair was to maintaion the integrity of foot structures and the sensation of the sole and heel.
