Interferon alpha-2b injections used as an adjuvant therapy to carbon dioxide laser vaporization of recalcitrant ano-genital condylomata acuminata.

We retrospectively analyzed our treatment of men with anogenital condylomata acuminata (CA) at Hennepin County Medical Center and the University of Minnesota. Between October 1985 and August 1989, 48 patients with CA were treated with laser vaporization, and 27 were treated with laser vaporization followed by a series of intralesional interferon (IFN) alpha-2b injections. The patients in the IFN group were more likely to have perianal CA (44% vs 24%), had larger numbers of CA, and were more likely to have had previous treatment, including previous laser vaporization. The CA in the laser group were more likely to be on the penis alone (67% vs 45%) and had a longer mean duration. The laser-treated group experienced a 38.2% rate of recurrence or reinfection with a mean follow-up period of 13 weeks. The laser and IFN group had an 18.5% rate of recurrence or reinfection with the same follow-up. We conclude that intralesional injections of alpha-2b interferon following carbon dioxide laser vaporization of recalcitrant ano-genital condylomata substantially reduce the risk of recurrence or reinfection.

Salicylic acid in karaya gum patch as a treatment for verruca vulgaris.

A clinical study was conducted to evaluate the efficacy of a new delivery system for administering salicylic acid for the treatment of verruca vulgaris. The study compared wart resolution among volunteers who used karaya gum patches. The cure rate was 69% for warts treated with patches containing salicylic acid, which was significantly higher (p less than 0.01) than for warts treated with control patches (35%).

Teaching school health to undergraduate medical students.

This paper describes a teaching programme for final- (sixth-) year undergraduate medical students during their 10-week term in child health. Students attend schools on two occasions and are actively involved in the physical, educational and emotional assessment of children. The advantages of the programme include active participation by the students, contact with children in a less threatening environment, learning new skills in the assessment of children, and knowledge of common screening procedures. Student evaluation of the programme has been positive.

Pyogenic granuloma following laser treatment of a port-wine stain.

A pyogenic granuloma occurred in a port-wine stain treated with first an argon, and subsequently a carbon dioxide, laser. Although reports of this phenomenon are infrequent in the literature, our review suggests that this is not a rare complication. The pathogenetic implications are discussed.

Graduate evaluation of undergraduate teaching programmes in paediatrics.

In a new undergraduate teaching course in child health, medical students were encouraged to prepare and present topics to their colleagues. These presentations covered major subjects in paediatric medicine and were supervised by an experienced teacher. Students were then asked to evaluate the programme at the end of their course in child health and after graduation. Evaluation scores for these presentations were high at the end of the course. Spontaneous comments suggested some disadvantages such as poor or too-detailed student presentations. However, some noted the value of personal research. After graduation, more positive comments were made and the majority felt that the course had advantages in the area of personal research, that it was better than or as good as other teaching methods, and that it should be continued in its present format.

Spontaneous resolution of extrahepatic biliary obstruction in a neonate.

A patient with spontaneous resolution of complete extrahepatic biliary obstruction shortly after the neonatal period is described. Liver biopsy prior to resolution revealed widened portal tracts and extensive fibrosis; these changes normalized following resolution. The aetiology was not established but possibilities included a choledochal cyst, localized inflammation; for example, pancreatitis, a calculus of the common bile duct or a congenital membrane. Management of this patient would have been advanced if there were a paediatric size side-viewing duodenoscope which would have diagnostic and therapeutic potentials.

A corneal abnormality associated with trisomy 8 mosaicism syndrome.

Eye abnormalities are a significant feature of trisomy 8 mosaicism syndrome. This paper gives the first account of the specific histopathology of a corneal opacity which is characteristic of this syndrome. The importance of early recognition is stressed because of potential therapeutic visual improvement. The necessity of including mosaic trisomy 8 in the differential diagnosis of such corneal opacities is illustrated by this case.

Intralesional recombinant alpha-2 interferon for the treatment of patients with condyloma acuminatum or verruca plantaris.

We conducted a multicenter double-blind study comparing human recombinant intralesional alpha-2 interferon (IFN) and placebo in 237 patients with the clinical diagnosis of condyloma acuminatum or verruca plantaris. A single wart on each patient was injected with 0.1 mL containing 10(6) IU of IFN, 10(5) IU of IFN, or placebo three times weekly for three weeks, and the response to treatment was followed up for 12 weeks. Among 91 of the 114 patients with condyloma acuminatum who completed the study, complete clearing of the treated wart occurred in 16 (53%) of 30 patients receiving 10(6) IU of IFN compared with six (19%) of 32 receiving 10(5) IU of IFN and four (14%) of 29 receiving placebo. In the group of 100 patients with plantar warts, there was no apparent benefit associated with interferon administration. Seven patients (3%) had treatment discontinued due to adverse reactions. Intralesional alpha-2 IFN is of benefit in the treatment of a single condyloma; its role in the treatment of multiple lesions remains to be clarified. Its role in the treatment of verruca plantaris, where no response was seen, also remains to be clarified.

Gnathostomiasis. Infestation in an Asian immigrant.

With the recent influx of southeast Asians into the United States, an increased incidence of human gnathostomiasis can be expected. A Laotian woman had had two weeks of pruritus associated with fleeting erythematous patches on her abdomen. A peripheral eosinophilia was present. She withdrew a Gnathostoma spinigerum from the skin of her abdomen. Gnathostomiasis is endemic in much of Asia and causes not only migratory cutaneous swellings with erythema but also serious visceral sequelae such as eosinophilic encephalomyelitis. Excision of the parasite is the treatment of choice when possible.

The effect of chronic nephrotic syndrome on the affected child.

The effect of the nephrotic syndrome on various aspects of behavior and development was examined in a group of 44 patients. The results of parent interviews, teacher ratings, and psychological tests (using the Self Observation Scales and the Children's Service Questionnaire) were compared with a closely matched group of healthy children. Very few statistically significant differences were noted. Comparisons were made with results of other studies of chronic disorders and their effect on the child's behavior and development.

Demodex folliculorum and Demodex brevis in cutaneous biopsies.

The hair follicle mites Demodex folliculorum and Demodex brevis are ubiquitous obligatory ectoparasites of man. We studied these mites in a consecutive series of skin biopsies submitted to a dermatopathology laboratory; 10% of all biopsies and 12% of all follicles contained demodectic mites. The prevalence of both species increased with age, but D. brevis had a lower prevalence. The face was most heavily infested by both species, but D. brevis had a wider distribution on the body. Males were more heavily infested than females with both species, the difference being strongest for D. brevis. Other ecologic characteristics noted include the type of follicles infested, follicular dilation, perifollicular inflammation, and the presence of a dense homogeneous eosinophilic material surrounding the mites. These data confirm ecologic and epidemiologic differences between the two species and emphasize that they should be distinguished in future studies of the roles these mites may play in a variety of disease states.
