The chronological relationship between the thickening of smooth muscle, epithelial cell proliferation and myenteric neural denervation in the rat jejunum.

The jejunum of rats was treated by serosal application of a 0.2% solution of benzalkonium chloride (BAC) for 30 min. Control animals were treated with saline (0.9% NaC1). The animals were allocated to eight groups of 10 rats each and sacrificed 15, 30, 45, 60 days after BAC treatment. Segments were removed from the jejunum for neuronal counting, measurement of the smooth muscle area and morphokinetic study of the epithelium. There was a significant reduction in neuron number in the myenteric plexus 30 days after BAC treatment, thickening of smooth muscle 15-60 days after BAC treatment, but no change in epithelial cell proliferation in the jejunum at either time.

Usher syndrome: results of a screening program in Colombia.

Otological, ophthalmological and genetic studies were performed in 46 patients with Usher syndrome, identified through a screening program in Colombia. Of them, 69.6% had Usher syndrome type I, 26.1% type II, and 4.3% type III. Thirty-three patients showed profound deafness (71.7%), while 13 (28.3%) had moderate to severe hearing loss. The ophthalmologic manifestations showed marked variability. Although the majority of the patients had serious ocular impairment before age 20, 32.6% had good central visual acuity. The prevalence of Usher syndrome in Colombia, estimated at 3.2/100,000, warrants the implementation of screening programs in schools for the deaf and for the blind. Our study confirms that Usher syndrome shows no geographic or racial variation and that the disorder has a wide variability of expression and genetic heterogeneity. The large size of the families we have detected may provide important opportunities for further genetic studies, particularly in terms of the assignment of the locus and gene mapping.

Immunopathological aspects of experimental Trypanosoma cruzi infection: correlation of immune complexes and other serological features with muscle lesions during the infection.

The pathogenesis of the tissue lesions observed in American trypanosomiasis (Chagas' disease) seems to depend on a variety of mechanisms. The present study was dedicated to the investigation of the evolution of muscle lesions and its relationship with immunological and parasitological parameters. Mice were infected i.p. with 10(5) blood forms of Trypanosoma cruzi (Columbian strain). A progressive increase in parasitaemia was observed which correlated with an increase in serum levels of IgG and IgM Trypanosoma-specific antibodies. Immune complex like material was detected in the serum (125I-C1q binding assay) on day 8 and reached maximum level between day 21 and 28. A concomitant fall in C3 levels was also observed. Inflammatory lesions and parasites were first observed in the striated muscle 2 weeks after infection. The cellular infiltrates involved macrophages and lymphoid cells; later on (week 3 onwards), intense necrosis appears and at the same time immunoglobulin and complement deposition were observed. These observations may indicate that, in addition to the cellular immune response, humoral mechanisms could contribute to the worsening of tissue lesions.

[Continuous Holter electrocardiogram in children. Analysis of 100 traces].

Authors analyse the value of the Holter system for diagnosis and follow-up of cardiac dysrhythmias. They report their experience from 100 records performed to 87 patients, who were between one day and 14 years old. Among them, 58 were male and 42 female. The recorder and analyser were automatics holter II/Quickscan, and the program was 24 hours in all cases. For the study of results, patients were divided in five groups (34 dysrhythmias; 18 neurologic; 10 were normal control; 20 newborns between 1 and 7 days; and 17 in a miscellaneous group). They emphasized on the possibility of the system to be performed in pediatric patients, even in the youngest; on the worth of records from neurologic with normal clinical examinations; on diagnostic aspects of undetected dysrhythmias by ECG (30%), and to know the normal values from the control group. Tables and figures show rhythm types and quantitative values; finally they comment on artifacts and indications.

[Immunological diagnosis of collagen diseases and rheumatoid arthritis (author's transl)].

Most helpful immunological tests for diagnosis of connectivities are revised. Among them, antinuclear antibodies (AAN) are the most important. They are usually detected by immunofluorescence and show different patterns, being the peripheral one the most specific of SLE and it is related with antibodies anti-DNA which can make immunocomplexes DNA-anti-DNA and be the cause of lupic nephropathy. The "nucleolar" and "speckled" patterns are more frequent in scleroderma. The LE cells have less value than AAN. The rheumatoid factor is a IgM antibody anti-IgG, but also can be of IgG or IgA class. It is uncommon in the juvenile rheumatoid arthritis of systemic onset and more frequent in oligoarticular adult type. May became positive during disease evolution. Other factors as immunoglobulin of complement usually are increased, except when the activation of complement take place. Acute phase reactants are helpful just for evaluating the importance of inflammation, they have not diagnosis value and commonly do not exceed the value of erythrocyte sedimentation rate.

Drosophila mediopunctata P elements: a new example of horizontal transfer.

Sequences homologous to the P element of Drosophila melanogaster were previously identified in Drosophila mediopunctata, a member of the tripunctata group, subgenus Drosophila. We report here that the P element is present in about three to five copies in the D. mediopunctata genome. While one of the insertion sites appears to be fixed, others may be polymorphic, indicating relatively recent P element activity. Phylogenetic analysis revealed that the D. mediopunctata element belongs to the canonical subfamily of P elements and that divergence of the D. mediopunctata element from other members of this subfamily ranges from 2% to 5% at the nucleotide level. This is the first report of a canonical P element outside the subgenus Sophophora. Based primarily on the striking incongruence between P element and host species phylogenies, the presence of a canonical P element in D. mediopunctata is most likely explained by horizontal transfer between species.

Intestinal paracoccidioidomycosis simulating colon cancer.

We report a case of intestinal involvement of Paracoccidioidomycosis, in a patient considered to have colonic cancer. The diagnosis of this mycosis should be considered when an abdominal mass associated with intra-lesional calcifications on X-ray is observed. CT scans increase the findings.

[Nursing practice and public health policy in Campinas, São Paulo, Brazil].

Redemocratization in the 1970s brought about changes in the public health system in Brazil. The Unified National Health System (SUS) was implemented in 1988, based on local and regional administrative systems. This was an important step that resulted in greater local control and the introduction of new technologies. This study focuses on the local health system in Campinas, a relatively affluent and technologically advanced region some 100km from the city of São Paulo in Southeast Brazil. Qualitative and quantitative methods were used to evaluate the work of 233 nurses in the local health system. The first nurse was hired in 1977, and now there are 53 nurses working in specific areas: 10 in the central health administration, 36 in district administrations, and the rest in local services. Nursing services are organized in six main areas: administration, coordination of human resources, education, information technology, health surveillance, and health administration. Nursing intervention may be found in various areas, but it generally focuses on medical consultations, and there is little emphasis on collective activities and health promotion. Local health objectives can only be achieved by creating new practices that promote collective health.
