Increased interleukin-2 receptor affinity in normal HLA A1 B8 DR3 subjects.

Twenty-two subjects (11 HLA A1 B8 DR3, 11 non-A1 B8 DR3) were tested for the capacity of their lymphocytes to express Tac molecules and interleukin-2 (IL-2) receptors (quantified using radiolabelled IL-2) after mitogen stimulation. Ten of these subjects (five A1 B8 DR3 and five non-A1 B8 DR3) were also tested for the ability of their lymphocytes to proliferate under IL-2 stimulation. A1 B8 DR3 subjects express a normal number of high-affinity IL-2 receptor sites, but the affinity of these receptors sites is significantly increased. Unexpectedly, A1 B8 DR3 lymphoblasts show a lower response to IL-2 than non-A1 B8 DR3 for high doses of recombinant IL-2.

Computer-based exercises in anemia diagnosis (PlanAlyzer).

PlanAlyzer is a microcomputer-based, event-driven system for instructional research in medical education. Aimed at the second year medical student, PlanAlyzer elicits and critiques a student's approach to the diagnosis of a common medical disorder. In this paper we discuss the concept, development, implementation and formative evaluation of the PlanAlyzer system for the evaluation of anemia.

Plasma transport of 99mTc-p-butyl-IDA.

Plasma transport of 99mTc-p-butyl-IDA was measured by four in vitro methods: trichloroacetic acid precipitation, electrophoresis, HPLC, and Scatchard binding isotherm. The data are in accord with protein transport, the main carrier being albumin with two categories of sites. This work suggests that after IV injection of 99mTc-p-butyl-IDA in humans plasma protein binding is one of the limiting factors for the hepatic deposition of the radiopharmaceutical.

[Investigation of biliodigestive anastomosis with the help of radiopharmaceutical products].

Hepatobiliary investigation using 99mTc-diethyl-iminodiacetic acid (IDA) has permitted a new point of view about the morphological and functional investigation of the biliary-digestive anastomosis. Our clinical study concerning 31 patients (13 choledochoduodenostomies 10 hepaticojejunostomies, 6 choledochojejunostomies and 2 cholecystojejunostomies) helped to specify scintigraphic imaging (stasis in intrahepatic bile duct, reflux in stomach, strangulation phenomenon on the level of the mesocolon, incomplete or complete obstruction). The problems associated with current diagnostic procedures are discussed and we place the scintigraphic method amongst other radiologic methods (barium meal, endoscopic retrograde cholangiography and percutaneous transhepatic cholangiography). This non-invasive diagonostic procedure plays a leading part in the investigation of the biliary digestive anastomosis, and particularly in the hepatico-jejunostomies.

[Comparative evaluation of the function of each kidney using Tc 99m DTPA].

To evaluate the clinical validity of 99m Tc DTPA renal investigation, we have studied 60 patients with different renal diseases: 1) Renovascular hypertension (22 patients); 2) Chronic pyelonephritis (11 patients); 3) Renal hypoplasia (12 patients); 4) Bilateral parenchymatous nephropathy with hypertension (15 patients). We observed a good correlation between the creatinine clearance and the total DTPA clearance (N = 51: r = 0.68; p less than 0.001); and also a good correlation between the respective renal surface extrapolated from renal X-Rays, and the respective renal function of each kidney (expressed in percentage) measured from DTPA data. This correlation is higher in group 1 and lower in group 2. From individual kidney function percentage and glomerular filtration rate measurement or estimation, we could calculate the GFR of each kidney, which serves as a guideline for surgical decisions.

[Renal radioisotope study with 123I-hippuran in the monitoring of transplants].

Hippuran labelled with iodine 123 is now used for short- and long-term monitoring of renal transplants. Sixty-one explorations were performed in 24 patients, using this new radiopharmaceutical agent. Repeated explorations, facilitated by the lower doses of radiations as compared with 131I-hippuran, are particularly useful to diagnose thrombosis of the renal artery, early acute tubular lesions, acute and chronic rejection and impaired evacuation of the kidney. 123I-hippuran is the first-choice method of exploration of transplanted kidneys and can be followed, if required, by explorations more aggressive for the patient and the kidney.

[Use of a new rheometer for the study of the filtrability of a suspension of sickled red cells as a function of PO2].

The filtration time of a small volume (0.1 ml) of red cell suspension from normal (AA), heterozygous (AS) and homozygous (SS) subjects for sickle cell disease was investigated as a function of PO2 The curve of filtration time of AS and SS red cell suspensions was biphasic. At high values of PO2, the progressive reduction of filtrability of sickle cell suspensions with decreasing PO2 occurred without new change in morphology of most of the cells. In contrast, at lower PO2 the apparent filtrability was improved and the cells were sickled." However the red blood cells were retained by the filter and the "solvent" filtrability was improved because rigid and highly deformed sickled cells did not clogged completely the pores of the filter. This study allowed to distinguish a new concept of apparent filtrability for red blood cells in sickle cell disease.

RBC filterability, oxygen saturation, ATP intracellular stock, and cerebral microcirculation.

The filterability of erythrocytes through Nucleopore filters of 5 micrometer has been studied with a new device (Hemorheometer SPO1). A reliable and reproducible index of filterability (IF) can be obtained which depends only on the red blood cell deformability. The results showed an alteration of red blood cell deformability in some pathological circumstances like smoking and that deformability depends on the nature of haemoglobin, the ATP intracellar stock and the saturation of haemoglobin by oxygen. The cerebral microcirculatory consequences of alterations of red blood cell deformability has been studied in vitro by measuring whole blood filterability on 5 micrometer Nucleopore filters with the classical technic of Reid & Dormandy and in vivo by 99mTc cineangioscintigraphy and by determination of the regional cerebral blood flow by the 133Xenon inhalation technic. The results showed an increase of cerebral ascensional time and mean transit time of the cineangioscintigraphy and a decrease of regional cerebral blood flow in patients presenting with a decrease of whole blood filterability flow.

CT of microcystic (serous) pancreatic adenoma.

Although microcystic adenoma of the pancreas is well recognised as a benign tumour, it is often treated surgically. We present serial CT over 9 years of a patient with this tumour that demonstrated a substantial rate of growth and the development of pathognomonic features.

Hepatobiliary and pancreatic manifestations of cystic fibrosis: MR imaging appearances.

Magnetic resonance (MR) imaging and MR cholangiopancreatography are useful, noninvasive techniques for the assessment of pancreatic and hepatobiliary complications in cystic fibrosis. Abnormalities of the pancreas in cystic fibrosis are typically characterized by fat deposition, which has increased signal intensity on T1-weighted MR images, and pancreatic fibrosis, which has low signal intensity on both T1- and T2-weighted images. Pancreatic cysts are a relatively common finding; these cysts are typically quite small but are well demonstrated at MR imaging and MR cholangiopancreatography. Pancreatic duct abnormalities are also occasionally seen. Hepatic manifestations range from hepatomegaly and diffuse fatty infiltration to severe cirrhosis with fibrotic change, regenerative nodules, and portal hypertension. Splenomegaly is often characterized by siderotic nodules that manifest as multiple focal areas of abnormal low signal intensity within the spleen. Biliary manifestations include cholelithiasis, stricturization, and narrowing or dilatation of intra- and extrahepatic bile ducts. Gallbladder abnormalities including microgallbladder are also readily demonstrated. MR cholangiopancreatography can be used to help determine the presence and severity of biliary complications without resorting to more invasive procedures and, in conjunction with MR imaging, may prove useful in the assessment of patients with cystic fibrosis who present with abdominal symptoms that suggest hepatobiliary involvement.
