Apparently isolated, spontaneous haematomas of the brain stem. Seven cases explored by CT and MRI.

The authors present seven cases of apparently isolated spontaneous haematomas of the brain stem detected by computerized tomography (CT). Magnetic resonance imaging (MRI) was performed in all patients, and in three of these on two occasions at different stages of the lesion. Changes in MRI signal according to the moment this examination was carried out (acute, subacute or chronic stage) are described. In two of three patients operated upon the haematoma was found to be due to an occult vascular malformation.

[Tumors of the pineal region: MRI aspects. Apropos of 20 cases].

MR imaging (0,5 T) was performed in twenty patients with a tumor of the pineal region (17 pathological correlations). To establish the place of this technique, MR imaging data are studied. There is a great variety of tumor types, but the signal of the tumor is usually non specific. The main indication of MR imaging appears to be the evaluation of the extension of the tumor before surgery.

Magnetic resonance imaging in familial paroxysmal ataxia.

A magnetic resonance imaging study was performed in three symptomatic members of two families with a diagnosis of acetazolamide-responsive familial paroxysmal ataxia. A selective atrophy of the cerebellar vermis, mostly of the anterior part, was demonstrated in the three cases. The first ample documentation of a neuroanatomical abnormality in this condition was achieved in this study.

[The MRI sites functioning in 1987 in France. Results of a survey].

A study of the 23 MRI sites functioning in France at the end of 1987 provides an overall view of the organizational and economic aspects of these sites as well as an initial assessment of their activity in terms of the types of equipment used and the institutions, private or public, within which they have been set up.

Prognostic factors and indications for surgical treatment of acute aortic dissections: a report based on 191 observations.

A total of 191 acute aortic dissections were examined to define prognostic factors for surgical intervention. Overall survival rate reached 40% in the 94 patients operated upon and 8% in the 97 patients not operated upon. Among the 122 patients with involved ascending aorta, survival rate was 2% in the 42 nonoperated patients versus 42% in 80 patients undergoing surgery. Since 1977, overall survival rate has reached 60% in the surgical group (21 of 35 patients) and 20% in the nonsurgical group (5 of 25 patients). Factors that showed a significant correlation with postoperative death include: persistent shock; persistent anuria; persistent neurologic deficit; diffuse intravascular coagulation; and involvement of either the celiac trunk, superior mesenteric artery, or both renal arteries at angiography. With the exception of those patients exhibiting any of these high-risk factors, emergency surgical treatment is recommended for all dissections that involve the ascending aorta.

[Urographic signs of thrombosis of the inferior vena cava and renal veins].

The case history of a 39 year old male with abdominal pain is described. Intravenous urography showed, during the late vascular period, collateral venous drainage around kidneys. Bilateral venous impressions on the ureters were observed at the excretory time. Inferior vena cavography and venous phase of selective renal arteriography confirmed the expected diagnosis of inferior vena cava thrombosis with bilateral renal vein involvement. Urographic signs of inferior vena cava thrombosis are discussed.

[Mediastinal bronchogenic cysts in adults. Cardiovascular symptomatology (author's transl)].

The authors describe seven patients operated upon for mediastinal bronchogenic cysts and emphasize the problems of differential diagnosis from cardiac and vascular affections: enlarged left auricle and aneurysm of the thoracic aorta. They review the published literature on atypical bronchogenic cysts with cardiac or vascular symptoms, and the diagnostic problems they raise in adults.
