The role of Pseudomonas aeruginosa in nosocomial infections occurring since 1975 is reviewed. Data from the National Nosocomial Infections Study conducted by the Centers for Disease Control, from individual medical centers, and from the literature were used to compare the relative frequency of occurrence of nosocomial infection caused by P. aeruginosa with that of infection caused by other gram-negative bacilli. The relative frequency of P. aeruginosa as a nosocomial pathogen has increased, although wide variations are seen among individual medical centers. P. aeruginosa continues to be a major pathogen among patients with immunosuppression, cystic fibrosis, malignancy, and trauma. While Staphylococcus aureus has become the predominant pathogen in some large burn centers, P. aeruginosa is the most important gram-negative pathogen. Periodic review of the epidemiology of P. aeruginosa infection is warranted in view of the changing incidence of infection caused by this organism.

The authors present three cases of osteoid osteoma of the neck of the talus presenting with pain and restriction of ankle movement affecting athletic performance in young adults. If this condition is suspected in a patient, a bone scan will confirm the diagnosis. The result of treatment is positive in terms of relief of pain and return to activities in our patients.
An apparatus is described which gives a rapid and detailed picture of the distribution of pressure under the foot, this being displayed either as a continuous grey scale or a color contour map on a television monitor. Automatic analysis systems are outlined which enable the quantitation of the distribution of pressures and loads under the foot during standing and walking, utilizing computer techniques. The use of the static system is illustrated by the results of analytic procedures carried out on normal feet and in the assessment of surgical correction of a series of equino-varus feet in children suffering from spina bifida. The clinical application of the dynamic system is illustrated by the measurement of pressure/time curves encountered under the heel, the five metatarsal heads, and the great toe of a control group of feet and the feet of patients who had undergone metatarsophalangeal fusions, Keller arthroplasties, and Swanson arthroplasties.
Samples of aqueous humor from rabbits and humans were examined for growth effects on subconfluent monolayer fibroblast cultures from ocular and extraocular tissues. At a concentration of 20% aqueous by medium volume, both homologous and heterologous aqueous stimulated DNA synthesis in fibroblasts from both species. The stimulation was independent of medium serum. When fibroblasts were exposed to 0 to 100% aqueous in serum-free medium or 0 to 98% aqueous in 2% serum-supplemented medium, no concentration of aqueous was observed in which DNA synthesis was inhibited. However, higher concentrations of aqueous led to increasing cell loss resulting in an apparent net inhibition of culture growth. Net growth effects of aqueous humor were, therefore, dependent upon aqueous concentration and were apparently due to the combined effects of stimulation of proliferation and reduction in cell survival.

Anti-GBM disease has been associated with the HLA genes of the major histocompatibility complex (MHC) in previous serological studies, with an increased incidence of HLA-DR2 in patients. In this study, 36 patients with anti-GBM disease were genotyped by restriction fragment length polymorphism (RFLP) analysis using cDNA probes for DRB, DQA, and DQB. The frequency of HLA-DRw15(Dw2), a split of DR2, was significantly increased in the patients compared with the controls (63.9 per cent versus 23.3 per cent, chi 2 = 22.4, p(corr) less than 0.0001), and all but one of the patients were positive for either DRw15(Dw2) or DR4 (p less than 0.0001). The frequencies of the remaining DR antigens were not decreased randomly, with a significant decrease in DR7 in the patient group (chi 2 = 8.6, p(corr) less than 0.05). The closely linked gene HLA-DQw6 was found to be significantly increased in frequency in the patients compared with the controls (p(corr) less than 0.0001). No correlations could be made between the genetic data and clinical features of the disease.
Evidence is accruing that older individuals receive little attention for mental health problems and that any attention that is given is most often within the primary care setting. A randomized clinical trial was carried out at a primary care clinic of The Johns Hopkins University, Baltimore, testing the ability of feedback of the results of a screening instrument (the General Health Questionnaire) to increase awareness in clinicians of the emotional and psychological problems of their patients. This report contrasts those aged 65 years and older with younger patients. Detection and management of mental morbidity were lower for older individuals, but the feedback intervention increased the likelihood of attention to these problems. This was not true for younger patients. Detection was significantly higher for older patients when screening data were made available, as was management, although the latter difference was not statistically significant. There was evidence as well that the intervention moved clinicians to greater congruence with their older patients in the perception that current mental health problems existed. These findings have important implications for primary care.
We assessed what percentage of hospitalized patients treated with intravenous antibiotics would be candidates for early switch to oral therapy, and evaluated the clinical outcomes of patients after the switch. All hospitalized patients in whom an intravenous antibiotic was prescribed for treatment of an infection were prospectively screened to identify candidates for switch in therapy. Of the 655 patients treated with intravenous antibiotics, 300 (46%) were candidates for a switch, and the change was implemented in 262 (40%). Of the 171 evaluable patients, the switch was associated with clinical cure in 167 (98%) and failure in 4 (2%). In hospitalized patients with infections, the duration of intravenous antibiotic therapy can be minimized with early switch to oral therapy. This practice is associated with good patient outcome.
We conducted a prospective study into the incidence of neck symptoms in victims of high energy trauma. Thirty consecutive patients were questioned regarding neck and jaw pain and stiffness initially and at six weeks. Despite suffering extensive skeletal injuries as a result of road traffic accidents, only two patients had such symptoms. We conclude that the incidence of 'whip lash' is surprisingly low in victims of high energy trauma.
The parenting environment comprises complex interacting factors at many system levels. The Parental Environment Cluster model of child neglect identifies three clusters of factors involved in the occurrence of neglectful behavior: factors related to parenting skills and functions; factors related to the development and use of positive social supports; and factors related to resource availability and management skills. According to the model, deficits in the parent's ability to use the parenting environment are present in the occurrence of child neglect. Management of the clusters and the ability to link the clusters affect the maintenance of the child's well-being. The model offers a focal theory for research on child neglect, a structure for exploring and refining the definition of neglect, and a framework for designing new interventive strategies in cases of child neglect.
