The influence of caecostomy and colonic irrigation on pathophysiology and prognosis in acute experimental pancreatitis.

OBJECTIVE: To find out if the presence of a stoma had any influence on the pathophysiology or prognosis of necrotising pancreatitis in rats. DESIGN: Randomised controlled study. MATERIAL: 112 male Wistar rats. INTERVENTIONS: Induction of pancreatitis by intraduodenal injection of 2 ml sodium taurocholate 2% with 10,000 units of trypsin; the duodenum and common bile duct were occluded for three minutes. The control group (n = 36) had no further procedure, but the remaining rats were randomised to have either caecostomy (n = 40) or colonic irrigation (n = 36). MAIN OUTCOME MEASURES: Mortality, histological grading of the pancreatitis, white cell count, serum amylase activity, and haemoglobin and endotoxin concentrations in blood. RESULTS: Operative mortality was 14% (n = 5) in the control group, 10% (n = 4) in the caecostomy group, and 8% (n = 3) in the irrigation group, leaving 31, 36, and 33 for assessment. Later mortality was 23% (n = 7), 17% (n = 6), and 9% (n = 3), respectively. The control group had a significantly higher median endotoxin concentration (219 ng/l) than both the caecostomy group (79.2 ng/l, p < 0.05) and the irrigation group (71.7 ng/l, p < 0.05). The amount of endotoxin was mirrored by the changes in the colonic mucosa in the different groups. CONCLUSION: Our results support the hypothesis that both caecostomy and colonic irrigation have a favourable effect on the outcome of necrotising pancreatitis in rats.

HELD: Prior judicial approval is not required for a surrogate to exercise the state constitutional right of self-determination of a patient who has become incompetent and who, while competent, specifically expressed her wishes regarding medical treatment decisions orally or in writing. The surrogate, in so exercising that right, must satisfy herself that certain requirements are met.

[Transference and countertransference at a therapeutic community for young adult psychiatric patients].

Transference and countertransference especially occur in therapeutic communities. Different forms of transference are described here. Experiences and possibilities to deal with are discussed by the example of the work in a therapeutic community. Results of research are considered as well. Finally the course of treatment of an inhabitant with borderline disorders will be recorded with regard to transference and countertransference.

[Use of Vagimid (Metronidazol) for chemotherapy of bacterial genital infections (author's transl)].

Vagimid (metronidazol) was used for antibacterial therapy in 115 cases of obstetric or gynaecological infections. The infections were brought under control in all cases. No Metronidazol-sensitive organisms were recordable, following treatment. The use of metronidazol on such infectious processes, particularly in cases in which other chemotherapeutic agents have failed or also in combination with less effective medicaments, seems to be justified for its strong action on obligate anaerobes which are aetiologically involved in the majority of these diseases.

[Hyperprolactinaemic endocrinopathy in gynaecological practice (author's transl)].

Prolactin and, to some extent, the gonadotrophins were determined in the serum of 50 patients with amenorrhoea (oligomenorrhoea) and or galactorrhoea. -- Hyperprolactinaemia was safely recorded from 32 cases. That relatively high number of patients with hyperprolactinaemia was attributed to retrograde examination of sterility patients who had been resistant to therapy. -- In the presence of galactorrhoea prolactin levels were increased in nine of 16 patients, whereas the same levels were increased in only three of 18 patients with postpill amenorrhoea. -- Gonadotrophin levels usually dropped in concomitance with marked hyperprolactinaemia, but they failed to deviate from normal with moderate prolactin increase. -- The above results are likely to underscore the importance of prolactin to the diagnosis of gynaecological endocrinopathies.

[Pregnancy and delivery after renal transplantation (author's transl)].

A case report is given on a 21-years-old woman who became pregnant 18 months after renal transplantation. In spite of relatively unfavourable conditions (match 0,25; hypertension) renal function showed a good adaptation throughout the gestation. In the 38. week of pregnancy the woman was successfully delivered of a 2440 g healthy female child by vacuum extraction. There were no effects of the immunsuppressive drugs on the newborn. It is the third case of a successful delivery after renal transplantation in the GDR.

[Bacteriological aspects of trichomonal vaginitis (author's transl)].

An analysis was made of the vaginal flora of 25 gynaecological patients with acute trichomomal vaginitis, with the view to elucidating the bacteriological situation. Eighty-four isolates, an average of 3.4 per patient, were taken and included a wide range of aerobic and anaerobic bacteria. - Obligate anaerobic species, such as bacteriodes and peptostreptococci, with susceptibility to metronidazole were among the predominant pathogens. - The differentiated susceptibility of the most common bacteria to antibiotics may yield information useful to therapy in the case of aggravated infection.

[The microprobe excision as a search biopsy within the scope of cancer-trace detection on the cervix uteri].

Besides colposcopy and cytology microbiopsy as a detection method for early detection of malign cervical lesions was applied to 142 women patients of the UFK Halle. A precondition for the successfull application of this method is a carefull preliminary colposcopic examination and close co-operation with the histologist. The interim result of the microbiopsy corresponded to a high degree with the definitive result. In this way, it was possible to indicate 20% of the preclinical carcinoma for histological evidence despite of negative cytology. An individual combination of all 3 ways of research guarantee a maximum of certainty for the early finding of cervical carcinoma and their pre-stages.

[Clinical significance of common anaerobes in pyogenic infections of the female genitalia].

From 1972-1977 one hundred and fouteen pyogenic infections of gynaecologic and obstetric patients at UFK Halle had retrospectively been examined. 168 isolations were analysed. By improving the methods of taking away, transporting and bacteriologic cultivation the specimens anaerobic organisms were recovered in 67.3% of all positive results. The pathogenic importance of them is being emphasized by 30,8% of anaerobic monoinfections. The difficult culture of these microogranisms requires a close cooperation between hospital and the place of research. In the treatment surgical operative measures often are preferred to use of chemotherapeutica because of the latter is characterized by a distinctive resistance against numerous antibiotica. This is especially characteristics of Genus Bacteroides besides E. coli the most isolated organism of all.

[Behavior of serum immunoglobulin E in female genital cancer].

More seldom allergic patients suffer from cancer than healthy ones. The level of serum-IgE of these patients is higher whereas IgE of cancerous patients is often lower. 16 patients with gynaecologic cancer and without allergic anamnesis were examined and the results were compared with references in literature. A possible importance of IgE for the physical prevention against tumor is being discussed.

[Abortion induction in early pregnancy using prostin F2 alpha and 15(S)-15-methyl PGF 2 alpha].

In early pregnancy up the 8th week of pregnancy Prostin F2 alpha was infused and 15(S)-15-Methyl PGF 2 alpha was used i.m. to induce menstruation in 20 or 19 cases, respectively. In the tested form application 15(S)-15-Methyl PFG 2 alpha is effective in 89 per cent of the cases and in 74 per cent complete abortion was achieved, Prostin F 2 alpha produced bleeding in 80 per cent only and complete abortion in 55 per cent. The differences in these two groups were not statistically significant.

Investigations of hormones during early abortion induced by prostaglandin F2alpha and 15(S)-methyl-PGF2alpha.

In early pregnancy up the 7th week of pregnancy PGF2alpha was infused and 15(S)-methyl-PGF2alpha was applied i. m. to induce menstruation in 20 or 19 cases, respectively. In the tested form of application 15(S)-methyl-PGF2alpha is effective in 89 per cent of the cases and in 74 per cent complete abortion was achieved. PGF2alpha produced bleeding in 80 per cent only and complete abortion in 55 per cent. The differences in these two groups were not statistically significant. The steroid hormones estradiol and progesterone decrease in a successful application of PGs for induction of abortion and reach a value of 75 per cent at the onset of bleeding. The LH concentration in plasma becomes smaller too. In some cases there is a temporary increase in hormones shortly after starting treatment. The results could indicate that the considerable decrease in hormones before the onset of bleeding might be caused by an alteration of the corpus luteum, which is effective during early pregnancy.

Gallbladder bile tumor marker quantification for detection of pancreato-biliary malignancies.

BACKGROUND: The pre-operative differentiation of tumors of the pancreas, Papilla of Vater and the biliary tract is still unsatisfactory. Tumor marker analysis of the pancreatic juice did not improve the pre-operative diagnosis by a great deal. METHODS: Bile from resected gallbladders of patients suffering from carcinomas of the pancreato-biliary system was analysed for CA 19-9, CEA, CA 72-4, CA 125 and AFP concentrations. The results were compared to patients suffering from acute cholecystitis, cholecystolithiasis as well as those suffering from benign tumors of the pancreato-biliary region. RESULTS: Extreme high CA 19-9 concentrations were found in bile. Evaluations of the tumor-antigens CA 19-9, CA 72-4 and CEA in gallbladder bile were superior to any serum and pancreatic juice examination with respect to sensitivity and specificity. Observed sensitivities amounted to 100% for patients suffering from bile duct carcinoma (CA 19-9) and papillary carcinoma (CEA) at a specificity of 100%. CA 19-9 showed a sensitivity of 76.5% for pancreatic carcinomas at a specificity of 96.4%. CA 19-9 showed significant differences for the local tumor burden and for the degree of lymph node metastasis. Examination of tumor antigens in the gallbladder results in a high degree of discrimination for malignant and benign lesions of the subhepatic pancreato-biliary system. CONCLUSIONS: CA 19-9 must follow a entero-hepatic circulation, since it showed raised bile concentrations (factor: 10(4)) compared to serum analysis. Analysis of CA 19-9, CEA and CA 72-4 gives an opportunity for improvement in the detection of cancers of the pancreato-biliary system. Since the clinical important differentiation of tumors of the head of the pancreas (carcinoma vs. pancreatitis) remains unclear, an improvement by bile analyses must be assumed.
