Sexual arousal in response to aggression: ideological, aggressive, and sexual correlates.

In two experiments we assessed the impact of aggression on nonrapists' sexual arousal. In the first, both male subjects (n = 37) and female subjects (n = 43) reported more sexual arousal in response to nonaggressive than to aggressive depictions when the portrayals were sexually explicit, but the opposite occurred when the portrayals were nonsexual. Only male subjects (N = 359) participated in the second experiment. On the basis of their self-reported sexual arousal to the use of force, they were classified into the no arousal, moderate arousal or high arousal from force groups. To evaluate the veridicality of this classification, we assessed some subjects' (n = 118) penile tumescence in response to various depictions. The findings generally replicated those of the first experiment and confirmed the accuracy of the arousal-from-force classification. The no arousal and the moderate arousal from force subjects were less sexually aroused by aggressive than by nonaggressive portrayals, but the opposite was found for the high arousal from force group. Using the entire sample (N = 359), we also assessed differences on various factors among these three groups. We found strong differences on ideological factors, including acceptance of violence against and dominance over women. We also found differences in acceptance of nonsexual aggression and in subjects' beliefs that they might actually use force against women. In contrast, differences were not found on sexuality factors. The data's implications for theories on the causes of rape are discussed.

Is the HLA-linked haemochromatosis allele implicated in idiopathic refractory sideroblastic anaemia?

In order to assess the previously reported association of HLA-linked idiopathic haemochromatosis with idiopathic refractory sideroblastic anaemia (IRSA), the prevalence of HLA-A3 antigen in a group of 22 patients with IRSA was compared to that observed in healthy controls and in patients with homozygous idiopathic haemochromatosis and to that calculated for a population heterozygous for idiopathic haemochromatosis. The prevalence of A3 in patients with IRSA (0.23) was quite similar to that observed in controls (0.29) and significantly different from that observed in homozygous (0.73; P less than 10(-5] and heterozygous (0.57; P less than 10(-3] haemochromatosis. Serum iron, transferrin saturation, serum ferritin and liver iron concentration showed no difference in IRSA patients with or without A3. It is concluded that there is neither systematic association between the haemochromatosis allele and IRSA nor systematic implication of such an allele in the development of iron overload observed in IRSA.

Acute renal failure caused by extreme hyperphosphatemia after chemotherapy of an acute lymphoblastic leukemia.

The authors report a documented case of hyperphosphatemia-induced nephrocalcinosis after chemotherapy of an acute lymphoblastic leukemia (ALL). Microscopic examination of the kidney showed numerous calcium deposits with concentric structure in the calyces and in the tubules. Electronic microsound analysis proved the deposits to be composed of calcium phosphate. The nephropathy mechanism is studied, and a review of the 34 similar cases of this acute tumor lysis syndrome is made. Some suggestions are put forward to present the consequences of this therapy-induced syndrome.

Philadelphia chromosome-positive chronic myelocytic leukemia in children. Survival and prognostic factors.

The survival and the prognostic significance of the diagnostic characteristics of 39 children with Philadelphia chromosome-positive chronic myelocytic leukemia (Ph1-positive CML), seen between 1963-1976 at the Hôpital Saint-Louis of Paris, have been analyzed. The disease predominated in children older than age 4 years (95%), with girls being more affected than boys (24 versus 15). The clinical and hematological picture at presentation was similar to that observed in adults with Ph1-positive CML. Most children of this series were treated with busulfan which, as in adults, led to reduction of leucocytosis and organomegaly but did not prevent the occurrence of blastic crisis. Well-documented blastic crisis was observed in 78% of cases. Of 39 children, 12 were still alive, all in the chronic phase. Twenty-seven have died, 21 of them after blastic crisis, 4-156 months after diagnosis (median survival, 53 months). The effect of each diagnostic characteristic on survival was evaluated using the log-rank test. Of the 14 characteristics studied, only the degree of blood and marrow blastosis was associated with a shorter survival. Age, sex, bleeding, lymphadenopathy, hepatomegaly, degree of splenomegaly, hemoglobin level, total leucocyte, immature granulocyte (promyelocytes + myelocytes + metamyelocytes), eosinophil, basophil, and platelet counts in the peripheral blood were of no prognostic significance. The failure to attain a level of statistical significance for some characteristics found to be of prognostic value for adults, could be due to the small sample size and/or to the disease homogeneity. The results of this study, however, stress the importance of the initial blastic infiltration in determining the duration of survival, which is ultimately determined by the occurrence of terminal acute leukemia. In conclusion, this study shows that the Ph1-positive CML of childhood exhibits the same course, incidence of blastic crisis, and survival as the disease of adults. It also indicates that treatment with moderate chemotherapy, such as busulfan, has no effect on the duration of survival. Therefore, new therapeutic approaches are urgently needed for the treatment of this disorder in children.

Sex-typing and cross-sex-typing in "androgynous" subjects.

Bem Sex-Role Inventory protocols from 848 subjects were categorized as androgynous or nonandrogynous according to each of three current scoring systems. Androgynous protocols by each scoring system were then examined for sex differences in trait item endorsements. The t-score method generated androgynous subjects who were cross-sex-typed, while androgynous subjects defined by the median split method tended to be same-sex-typed. Androgynous subjects by the intersect method presented the fewest number of sex differences. These method-specific versions of androgyny appear linked to the use of summated Masculinity and Femininity subscales of the BSRI.

[Non-secretory myeloma of heavy mu-chain type].

We report on a patient with a bone marrow plasmacell infiltration, since this case displays some outstanding clinical and laboratory features: the presence of massive splenomegaly and the absence of bone lesions; the cytoplasmic heavy chain type mu, without any light chain detectable by immunofluorescence (IF); the non secretory feature of this plasma cell proliferation. The position of such an entity within the spectrum of B-lymphoproliferative disorders, from mu heavy chain disease to non secretory IgM myeloma, is discussed.

[5 new cases of solitary fibrous mesothelioma of the visceral pleura].

Five new cases of solitary fibrous mesothelioma of the visceral pleura support this review of the literature. Well encapsulated, mostly pedunculated, the implantation on the visceral pleura remains asymptomatic for a very long time and is more likely to be an incidental finding on a chest radiograph. Two extrathoracic signs are associated with certain huge tumors : hypoglycemia and osteoarthropathy. Histologic examination discloses whorls of collagen with scattered spindle-shaped cells. Electron microscope reveals characteristic features of both fibroblasts and mesothelial cells. Complete surgical removal is the best treatment for localized fibrous mesotheliomas of the visceral pleura. Recurrences after resection are rare and late recurrence, as long as 16 years after initial excision, usually behaves in a benign fashion.

Essential thrombocythemia: response during first year of therapy with melphalan and radioactive phosphorus: a polycythemia Vera Study Group report.

Thirty-one patients with essential thrombocythemia were randomized to receive either melphalan or radioactive phosphorus as myelosuppressive therapy. Twenty-seven patients were evaluable for response. Of 13 patients treated with melphalan, 11 had a complete response (platelet count less than 450,000/mm3) at 3 and 6 months. This response rate was significantly better than the response to radioactive phosphorus. The response rates were similar at 12 months. No significant toxicity was observed with either regimen.

[Massive poisoning by paraquat with early death (19 hours). Apropos of a case with ultrastructure study].

The authors report a case of a forty-years-old woman who died nineteen hours after ingestion of a massive dose of Paraquat (about 125 ml of Gramoxone, 52 g of Paraquat) with vomitings, anuria and cardiac arrest. Early autopsy (less than two hours after death) showed necrotizing esophagitis, ulcerative gastritis, tubular nephritis, congestion of lung, interstitial myocarditis which could explain the heart failure. The ultrastructural study of lung showed interstitial edema associated with minimal endothelial changes but important epithelial cell necrosis of both pneumocytes. These findings are very similar to experimental studies of massive Paraquat poisoning emphasizing the early involvement of pneumocyte II.
