Periodontal status and total tooth extraction in a medium-sized city in the Netherlands.

A combined sociodental research project was started in the city of Groningen in 1982 aimed at gaining insight into the reasons for total tooth extraction. Patients showing up for total tooth extraction during 1982 participated in this study by filling out a questionnaire concerning the underlying reasons for their decision to have all their teeth extracted. The dentists in this city were asked to collect all the extracted teeth and to fill out a short questionnaire for each patient about the reason (diagnosis) for the total tooth extraction. The periodontal status of the extracted teeth was established afterwards by measuring the percentage of the loss of attachment (L.A.). It appeared that most surfaces in the molar region and the front teeth in the mandible were affected by periodontal disease. Advanced generalized periodontal disease was established in 17% of the patients, accounting for 64% of all teeth with an L.A. measurement of greater than 50%. Periodontal disease could not be identified as the main clinical condition requiring tooth extraction: even in the group of 55 yr of age and older advanced periodontal disease was found in only one third of the extracted teeth. Finally, the contribution of regular dental attendance to periodontal health is discussed.

Radical surgery after preoperative intracavitary radiotherapy for Stage IB and IIA carcinoma of the uterine cervix.

During the years 1970-1978 one hundred nineteen patients with Stage IB and fifty-eight patients with Stage IIA carcinoma of the cervix were treated by combined preoperative radium and Wertheim hysterectomy with lymphadenectomy at the State University Hospital in Groningen. The overall 5-year survival was 87% for Stage IB and 70% for Stage IIA. The incidence of pelvic lymph node metastases was 14.8 and 35.4% in Stage IB and IIA, respectively. The presence of lymph node metastases was significantly related to the presence of residual tumour in the cervix after preoperative radium treatment (P less than 0.01) and was the most significant prognostic factor. The 5-year survival was 37% for those patients with node involvement as compared to 94% for those without lymphatic extension. Complications particularly concerned the urinary tract. The incidence of these complications was greatly reduced as experience grew.

Effective high-dose chemotherapy with autologous bone marrow infusion in resistant ovarian cancer.

Two patients with persistent minimal ovarian cancer after conventional polychemotherapy were treated with high doses of cyclophosphamide and VP 16-213 followed by autologous bone marrow infusion. Ten months afterward no clinical signs of tumor were apparent. In one patient the complete response was surgically documented. Toxicity included cardiac and pulmonary arrest during marrow infusion in one patient, but was otherwise manageable. This method of late intensification of chemotherapy in patients with persisting ovarian cancer merits further investigation.

Treatment of malignant germ cell tumors of the ovary with cisplatin, vinblastine, and bleomycin (PVB).

Seven patients with germ cell tumors of the ovary were treated with four cycles of cisplatin, vinblastine, and bleomycin. Six patients were without evidence of disease 17+ to 37+ months after completion of the remission-induction chemotherapy, and one patient relapsed after 9 months. Substantial toxicity of the cisplatin, vinblastine, and bleomycin regimen was noted. Despite the high response rate suggested in this small series, the optimal chemotherapeutic treatment for this disease has yet to be determined.

Palliative embolization in gynaecological patients.

Palliative embolization in patients with gynaecological malignancies is indicated, if other methods of treatment like irradiation, surgery and chemotherapy are impossible or contra-indicated. The experience in 9 patients is reported, with embolization of one renal artery because of metastasis in the kidney and in eight patients the hypogastric arteries uni- or bilaterally by means of Gelfoam and/or Gianturco coils because of inoperable bleeding malignancies.
