Although primitive plasma cells leukaemia is uncommon, it still stands in a particular place among malignant lymphoplasmocytary syndroms. It borrows from multiple myeloma and acute leukaemia some of its clinical and biological manifestations. Its course is quickly adverse, its treatment ineffective and the average life expectation is three months.
The chronic myelomonocytic leukaemias are difficult to be ascertained. Their clinical, haematological and biochemical features are not accurate enough to discriminate them in any cases from the following illnesses: smoldering leukaemias, acute myelomonocytic leukaemias, monoblastic leukaemias. A retrospective study of twelve cases demonstrates the frequency and the diagnosis value of spontaneous hyperleucocytic status which in some cases seem to be provoked or increased by prednisone treatments, and may induced a spontaneous regression.
A prospective study of tamoxifen therapy in doses of 10 mg twice daily was carried out in 79 patients with advanced renal cell cancer. The objective response rate (complete response plus partial response) was 6.3% (five of 79 patients), while 34.1% (27 of 79 patients) had stable disease. Survival of this group of patients (complete response plus partial response plus stable disease) was significantly longer than that of patients with progressive disease (P less than 0.04). Also, among patients with a good performance status at the beginning of the study, survival was longer than among those with a poor performance status (P less than 0.001). No statistical significance was found in the survival of these patients according to sex (P = 0.55) or whether or not they had received previous systemic therapy (P = 0.97). Toxicity was low and acceptable. We concluded that stratification according to performance status may be used in future randomized systemic therapy protocols for patients with metastatic renal cell cancer. Also, in spite of the low response rate to hormonal therapy including tamoxifen, the use of these agents with combination chemotherapy may enhance the overall response rate without increasing side effects.
This workshop intended to perform a "state-of-the art" of current research on adhesion molecules in various pathophysiologies, and to determine pharmacological targets. Indeed, recent important progress concerning the cellular and molecular physiology of adhesion molecules led to the development of various integrin antagonists in several domains, like cardiovascular disease, inflammation and cancer. Integrins play a major role in numerous process like embryonic development, tumor growth and metastasis, apoptosis, hemostasis, leucocyte recruitment and activation, and bone resorption. The development of integrin antagonists is well advanced in the cardiovascular domain, since the first marketed drug (abciximax, Reopro) is an antibody directed against the GPIIb/IIIa complex (integrin alpha IIb/beta 3) involved in the final pathway of platelet aggregation. Another active domain of research in pharmacology is 'cardioprotection', i.e. the prevention of cardiac damages induced by the reperfusion of the coronary bed after an ischemia secondary to thrombolysis, angioplasty, of coronary bypass. The pharmacological targets of these antagonists are integrins involved in various process like leucocyte and platelet adhesion and endothelial function. Other potential indications in the cardiovascular field are restenosis after angioplasty, and atherosclerosis.
