K-region oxides and imines derived from alkylated benz[a]anthracene congeners: synthesis, stability in aqueous media and mutagenicity.

The K-region oxides and imines of benz[a]anthracene, 1-methylbenz[a]anthracene, 7-methylbenz[a]anthracene, 7-ethylbenz[a]anthracene and 7,12-dimethylbenz[a]anthracene were synthesized and characterized (melting point, 1H-NMR and electron impact mass spectra, elemental analysis, IR spectroscopy). All 10 compounds showed high mutagenic activity in Salmonella typhimurium (reversion of his- strains TA97, TA98, TA100 and TA104). The arene imines were more potent than the corresponding arene oxides. Alkyl substitutions strongly influenced the activities. Furthermore, all compounds were more active when exposure took place in the absence of inorganic ions than when KCl (125 mM) was present. The influence of the exposure medium was more pronounced with strain TA98 than with strain TA100. The half-lives of the test compounds were determined from mutagenicity experiments in which the compound was added to the exposure medium at varying times before the bacteria. In dilute sodium phosphate buffer (10 mM, pH 7.4), the half-lives of these chemicals (or their biological activity) varied from 0.5 to 110 min. Addition of KCl (150 mM) did not measurably affect the half-lives of some test compounds and appeared to slightly shorten those of others. Therefore, it is unlikely that the strong effect of KCl on mutagenicity and the dependence of this effect on the bacterial strain used can be explained by influences of KCl on the test compounds. Rather, it appears more likely than an effect of KCl on the bacteria may be an important factor. This study provides further examples of strong influences of unobtrusive media components on mutagenicity. It also demonstrates that small structural changes (alkyl substituents at diverse positions of the aromatic system) may play an important role in chemical reactivity and biological activity.

[The value of the radiologic Lachman test in anterior cruciate ligament ruptures].

A radiologic Lachman test of both knees was performed in 35 patients with surgically verified tears of the anterior cruciate ligament (ACL). We found a mean anterior drawer of 3.2 mm for uninjured joints, 4.5 mm for isolated ACL-tears and 10.8 mm for complex ACL-injuries. Sensitivity of the radiological Lachman test was 95% for complex ACL-injuries as only 23% in case of isolated ACL-ruptures. Failure of diagnosis due to clinical investigation in case of ACL-ruptures could not be proved by means of radiological Lachman-test. Technical requirements and radiation exposure are further disadvantages. Clinically doubtful cases of acute knee injuries should therefore be investigated with other diagnostic procedures.

Do patients with less than 'echo-perfect' results from mitral valve repair by intraoperative echocardiography have a different outcome?

BACKGROUND: Not all valve repairs for mitral regurgitation (MR) have a perfect result, with no MR on postpump intraoperative echocardiography. Although more than 2+ MR by postpump echocardiography has led to second pump runs for further surgery in 6% to 8% of our patients, those left with 1+ or 2+ MR, traditionally an acceptable result, have not previously been evaluated for clinical outcome. METHODS AND RESULTS: Among 530 patients undergoing mitral repair during 1987 to 1989, 76 patients with 1+ or 2+ MR by postpump intraoperative echocardiography were compared with 76 patients who had equivalent age, sex, left ventricular function, and concomitant surgery but who had no MR by postpump echo. In-hospital morbidity measured by the frequency of respiratory complications, strokes, time in intensive care unit, and duration of hospital stay was actually higher in the patients with no MR after repair. Hospital mortality was not significantly different. There were no significant differences in posthospital mortality (4-year survival estimate of 86% in both groups), thromboembolic events, hospitalizations for heart failure, or functional class, although more patients in the group with no MR after repair were using diuretics. There was a trend toward more reoperations in patients with 1+ or 2+ MR compared with those with no MR by intraoperative echocardiography after repair (4-year estimates of freedom from reoperation, 83% versus 94%; adjusted risk ratio, 3.30). CONCLUSIONS: The presence of 1+ or 2+ MR by postpump intraoperative echocardiography does not confer increased morbidity or mortality. However, the trend toward more reoperations suggests the need for close follow-up for possible recurrence of MR. These data support our low threshold for performing further surgery during second pump runs.

[Incidence of acetabular fractures before and after introduction of compulsory seatbelt fastening].

The most common causes of acetabular fractures of the hip joint are high speed car accidents, most often as a result of a direct trauma against the knee. With these causal relations in mind this study wanted to show the preventive character of the law for obligatory safety-belt-wearing in Germany introduced in August 1984. Therefore we analyzed the incidence and the severity of acetabular fractures of the hip joint five years before and five years after 1984 within the group of patients treated by the traumatology department of the university hospital of Mainz. We found 54 patients with acetabular fractures of the hip joint, within a period of five years after 1984 in contrast to 84 injuries within an equal five year period before 1984. Car accidents where the cause of injury in 63 cases within the period of five years before 1984, and in 23 cases in the same period after 1984. Also the severity and complexity of these injuries decreased significantly. Therefore we consider the introduction of obligatory safety-belt-wearing in Western Germany as a good preventive measurement to reduce the incidence of acetabular fractures of the hip joint as a result of car accidents.

[Simultaneous rupture of the patellar tendon and the contralateral quadriceps tendon in chronic kidney insufficiency].

Simultaneous ruptures of the patellar tendon and the contralateral quadriceps tendon, happening either spontaneously or with a preceding minor trauma, is a rare event especially in young patients. Major degenerative changes in the tendons' structure are most often a prerequisite for a trauma such as this. The reasons for these excessive pathological findings in tendon tissue structure can be chronic renal failure and resulting secondary hyperparathyroidism. The causal relationship will be discussed in front of a patients case history followed by a review of the relevant literature.

Effects of thermocycling on the margins of transitional acrylic resin crowns.

Transitional resin crowns were subjected to thermocycling and examined for marginal integrity since disturbance of the marginal seal is detrimental. Two groups of 10 crowns, one with a chamfer and the second with a shoulder finish line, were subjected to two series of 2500 thermocycles. In the first series, the crowns received cold thermocycling and in the second, hot thermocycling. Impressions were made of the facial margins before thermocycling, after cold thermocycling, and after hot thermocycling. The impressions were analyzed for loss of marginal integrity and changes in axial contour. Cold thermocycling resulted in only limited deterioration, but the changes after hot thermocycling were more pronounced. Tooth preparation designs did not influence the effects of thermocycling.

Strong influence of the exposure medium on mutagenicity in the Ames test: 7-methylbenz[a]anthracene-5,6-oxide.

We have previously shown that the activity of the ionized mutagen, 1-hydroxmethylpyrene sulphate, is strongly enhanced in Salmonella typhimurium TA98, when KCl is present in the exposure medium (50-fold at a concentration of 125 mM KCl) and that the halogen ion is responsible for this effect. Here we show that KCl has the opposite effect on the activity of the lipophilic mutagen, 7-methylbenz[a]anthracene-5,6-oxide, (10-fold decrease at a concentration of 125 mM) and that K+ accounts for this influence. Many other solutes also decreased the mutagenicity of 7-methylbenz[a]anthracene-5,6-oxide, but to a smaller extent than the K+ salts. The stability of 7-methylbenz[a]anthracene-5,6-oxide did not appear to be altered in the presence of KCl (t1/2 approximately 12 min), as determined from mutagenicity experiments in which the test compound was added to the exposure medium at varying times before the bacteria. Furthermore, the influence of the exposure medium was significantly stronger in strain TA98 than in strain TA100. Taken together these findings argue for an influence of the medium on the bacteria rather than on the test compound. Parallel studies with other mutagens indicate that exposure in distilled water enhances the mutagenicity of many compounds. Exposure in distilled water, in combination with some other modifications, led to a 400-fold increase of the assay sensitivity towards 7-methylbenz[a]anthracene-5,6-oxide, as compared to the usual plate incorporation assay.

Alpha-adrenergic agonist drugs, left ventricular function, and emergency from cardiopulmonary bypass.

The relationship between preoperative left ventricular (LV) dysfunction and the use of alpha-adrenoceptor agonists during weaning from cardiopulmonary bypass was studied in 102 patients undergoing coronary artery surgery. LV function was evaluated subjectively by examination of the 30 degree right anterior oblique left ventriculogram. Group 1 consisted of 75 patients with normal or mild impairment in LV function, whereas group 2 consisted of 27 patients with moderate-severe LV dysfunction. Cardiovascular parameters were measured at baseline, during hypothermic bypass with the aorta clamped, during normothermic bypass after unclamping, and 1 and 15 minutes after separation from bypass. Thirty-seven patients in group 1 (49%) and 11 patients in group 2 (39%) required some form of inotropic and/or vasopressor support during weaning from bypass. Group 2 required significantly higher cumulative doses of phenylephrine after bypass compared with group 1 (1.69 +/- 0.70 mg, n = 9, v 0.24 +/- 0.04 mg, n = 27, P less than 0.05). Similarly, the dose of norepinephrine infusion in group 2 was 3.3 times that in group 1 (10 +/- 6 micrograms/min, n = 2, v 3 +/- 0.6 micrograms/min, n = 2, P = NS). The higher doses of alpha-adrenoceptor agonists required in patients with moderate-severe LV dysfunction may be due to differences in alpha-adrenoceptor agonist affinity and/or receptor density.

Structure-activity relationships in mutagenicity and in nucleophilic ring opening of N-(arylmethyl)phenanthrene 9,10-imines.

Ten derivatives of N-benzylphenanthrene 9,10-imine with different substituents on the phenyl ring were synthesized and subjected to mutagenicity tests in Salmonella typhimurium TA100. While electron donating groups were found to enhance the biological activity, electron attracting and bulky substituents lowered the mutagenic potency. A similar dependence on the electronic structure was observed in triethylamine/acetonitrile-promoted interaction of the title imines and 4-nitrothiophenol. This similarity suggests that both biochemical and chemical processes involve mechanisms in which protonation of the aziridine nitrogen is rate controlling, and the attack of the cellular or model nucleophile is a fast step. In contrast to these processes, the reaction of the imines with 4-nitrothiophenol in the presence of 1,5-diazabicyclo[4.3.0] non-5-ene proved to proceed by an SN2 mechanism and to be enhanced by electron attracting substituents.

Immunological reactions of pigs during long-term beta-adrenergic treatment.

Experiments were performed to determine the effects of the beta-adrenergic agonist Ro 16-8714 on the immune system of finishing pigs fed an adequate or a low-protein diet. Growth rate, nitrogen and fat retention, body composition as well as spleen weight were influenced by both beta-agonist and/or protein level, as expected. However, lymphocyte stimulation with Concanavalin A and phytohemagglutinine in vitro or in vivo antibody production against horse red blood cells were not changed by the beta-agonist in pigs fed sufficient or insufficient amounts of protein.

Basic FGF treatment of endothelial cells down-regulates the 85-KDa TGF beta receptor subtype and decreases the growth inhibitory response to TGF-beta 1.

Transforming growth factor-beta 1 (TGF beta 1) and basic fibroblast growth factor (bFGF) are known to be potent inhibitors and stimulators, respectively, of endothelial cell growth in vitro. In the present study we examined the effect of bFGF on endothelial cell growth inhibitory activity of TGF beta 1 and on the binding of (125I)-TGF beta 1 to these cells. The concentration of TGF beta 1 required for half-maximal inhibition of endothelial cell growth was increased in a dose-dependent manner by bFGF (a 20-100 fold increase at 1 ng/ml of bFGF). A 24 h-pretreatment of cells with bFGF resulted in abolition of the TGF beta 1 inhibitory effect on DNA synthesis. Moreover, the binding of (125I)-TGF beta 1 to the endothelial cell surface was decreased in a concentration-dependent and time-dependent manner after a preincubation of these cells with bFGF. Analysis of the binding parameters showed that bFGF decreased by two-fold the number of TGF beta receptors (to approximately 6000 receptors per cell). Cross-linking experiments with disuccinimidyl suberate demonstrated the presence of two TGF beta receptor subtypes, a predominant 85 kDa form and a minor 65 kDa form. Basic FGF decreased selectively the labeling of the 85 kDa TGF beta receptor subtype. These findings suggest that the growth stimulator bFGF can attenuate the cell's response to the growth inhibitor TGF beta 1.

[Chronic hemolysis and mitral insufficiency after surgical mitral valvuloplasty. Treatment by valve replacement, a case report].

The authors report a case featuring a chronic increase in lactic dehydrogenase (LHD) levels, 8 years after surgical mitral valvuloplasty with residual mitral regurgitation. This increase in LDH levels was the sign of chronic haemolysis and disappeared after valve replacement with a mechanical prosthesis. This rare complication was attributed to contact between the sutures and the teflon used for mitral valvuloplasty and turbulence created by the mitral regurgitation.

Antibiotic single-dose prophylaxis of shunt infections.

Shunt infections after implantation or revision of a shunt for CSF drainage in hydrocephalic patients are serious complications. In view of their frequency, this study investigated the efficacy of prophylactic administration of a single dose of the antibiotic cefazedone in reducing in the post-operative infection rate. Fifty children of up to 14 years of age suffering from hydrocephalus of various etiologies were treated prophylactically and compared with a similar untreated group. The diagnosis of infection was based mainly on microbiological and clinical observations and investigations. Classical infection parameters were also recorded and evaluated, but played only a secondary role in establishing the diagnosis as they can change post-operatively without an infection being present. The infection rates observed were 6% in the group of patients who received antibiotic treatment prophylactically and 14% in the untreated control group.

Synthesis and hypolipidemic and antidiabetogenic activities of beta,beta,beta',beta'-tetrasubstituted, long-chain dioic acids.

beta,beta,beta',beta'-Tetrasubstituted, long-chain dioic acids of the general formula HOOC-C(XY)-C(R2)-Q-C-(R2)-C(XY)-COOH have been synthesized and evaluated as hypotriglyceridemic-hypocholesterolemic agents in rats and as antidiabetogenic agents in ob/ob diabetic mice. The free carboxyl function of analogues of the series was mandatory for their hypolipidemic-antidiabetogenic effect while nonhydrolyzable diesters were inactive. Other structure-activity relationships were determined as a function of the overall chain length (C12-C22), alpha,alpha'-substitutions (X, Y = H, F, Cl, Br, OH, CN), beta, beta'-substitutions (R = CH3, C6H5), and core substitutions [Q = (CH2)10, (CH2)4CH = CH(CH2)4, 1,4-C6H10[(CH2)3]2, 1,4-C6H4[(CH2)3]2, 1,4-C6H4(CH = CHCH2)2, CH2(OCH2CH2)3OCH2)]. The most effective hypolipidemic-antidiabetogenic members of the series were alpha,alpha'-nonsubstituted, beta,beta'-methyl-substituted analogues of 14-18-carbon chains having either a saturated aliphatic core or a 1,4-bis(propenyl)benzene core in the cis/trans configuration. The hypotriglyceridemic rather than the hypocholesterolemic capacity of members of the series was found to correlate with their respective capacities as liver peroxisomal proliferators in rats.

Insulin-like growth factor I during growth in bulls.

In 10 bulls, changes in blood plasma concentrations of insulin-like growth factor I (IGF1) were studied during rearing and during the ensuing growth period. IGF1 continuously increased from 32 micrograms/l at the age of 15 d to 194 micrograms/l at the age of 307 d together with body weight. However, IGF1 was not related to daily rate of gain, which remained fairly constant during the growth period. An age-dependent increase was also observed for blood levels of insulin, thyroxine and triiodothyronine. The data suggest that insulin and thyroid hormones may be causally related to the age-dependent increase in IGF1 levels.

Insulin-like growth factor I responses to recombinant bovine growth hormone during feed restriction in heifers.

Insulin-like growth factor I, other hormones and blood metabolites were measured in growing heifers before, during and after a 3-day period of normal feed intake and a corresponding period of reduced feed intake. In addition, 0.1 or 0.5 mg recombinant bovine GH/kg was injected daily for 5 days during normal or during and following reduced feed intake. During reduced feed intake blood concentrations of insulin-like growth factor I, insulin, T4, T3, glucose and urea-nitrogen decreased, whereas those of non-esterified fatty acids, albumin and protein increased (P less than 0.05). GH, insulin-like growth factor I and insulin increased, whereas urea-nitrogen decreased in response to exogenous GH when heifers were adequately fed (P less than 0.05). In contrast, insulin-like growth factor I did not change during GH injections while heifers received reduced amounts of feed. Therefore, during insufficient energy and (or) protein intake, characterized by low glucose, insulin and thyroid hormone levels and increased non-esterified fatty acid concentrations, insulin-like growth factor I concentrations and responses to GH administration were markedly reduced.
