Comparison of maintenance and recovery characteristics of sevoflurane-nitrous oxide and enflurane-nitrous oxide anaesthesia.

We compared maintenance of anaesthesia and recovery using either sevoflurane or enflurane anaesthesia in ASA I-III patients undergoing surgery with an anticipated minimum duration of 3 h. Serum fluoride concentrations were also measured to assess the potential for renal toxicity. After induction of anaesthesia with thiopentone, patients received, for maintenance, either 1.5% end-tidal sevoflurane (0.73 MAC) with N2O 58% (0.58 MAC) (n = 40) or 1.2% end-tidal enflurane (0.7 MAC) with N2O 57% (0.57 MAC) (n = 40). Other drugs administered during anaesthesia were similar in all groups. Haemodynamic measurements, recovery times, as well as post-operative nausea and vomiting were comparable in both groups. The mean peak plasma inorganic fluoride ion concentrations were reached 4 h after operation in both groups (27.7 microM L-1 for sevoflurane and 16.75 microM L-1 for enflurane, P < 0.05) declining 50% within 24 h in both groups. A positive correlation (P < 0.05) was found between anaesthetic exposure (MAC h) and fluoride concentrations in the two groups. Sevoflurane anaesthesia resulted in similar haemodynamic stability, recovery times and post-operative side effects as enflurane anaesthesia, but produced significantly greater serum fluoride levels.

[Helicobacter pylori: comparative study of different diagnostic techniques].

OBJECTIVE: To compare the different diagnostic techniques of Helicobacter Pylori infection, confronting the bacteriological culture to other faster, less expensive and easier techniques, such as the urease test, methylen blue and Gram stained smears, the histopathologic examination of biopsies specimens, and the detection of IgG antibodies anti Helicobacter Pylori. MATERIAL AND METHODS: 43 patients with gastroduodenal symptoms who had not taken NSAIDs and who presented endoscopic signs of inlammation and/or gastroduodenal ulcer were prospectively studied. Biopsies specimens of antrum, gastric fundus and duodenal bulb were taken and processed in the following way: methylen blue and Gram stained smears and Hematoxylin and eosin stained section. Studies were performed in to all the 43 patients. Urease test was performed in the antrum specimens of 37 patients, and inmunologic test by inmunoenzimatic technique (ELISA) in 40 of them. RESULTS: Methylen blue and Gram stained smears: sensitivity (S): 100% and specificity (E): 76% Histopathologic examination (S): 86% (E): 61%, urease test (S) 100% (E) 62.5%, antibodies anti H.P. not possible to calculate. DISCUSSION: The authors have taken the microbiological culture as a reference technique because the isolation and identification of the bacteria are the most important signs of its presence and vitality. Stained smears, histopathologic examination and the urease test are fast, inexpensive techniques with high sensitivity for diagnosis. However, the inmunologic test is reveals better to the prevalence of the infection.

Mechanisms for synergistic activation of thyroid hormone receptor and retinoid X receptor on different response elements.

The thyroid hormone receptors (TR) form heterodimers with the retinoid X receptors (RXR) and activate target genes through thyroid-responsive elements (TRE). Heterodimerization elevates the DNA binding efficiency and thus can result in functional synergism between TR and RXR. Here we demonstrate that DNA sequences dictate the cooperative activation between TR and RXR despite the high affinity binding of the heterodimer to those TREs. We provide evidence that the C-terminal activation domain of RXR can modulate the triiodothyronine (T3) responsiveness of TR/RXR heterodimers on reporter genes without altering the DNA binding properties of the heterodimers. The modulation function of this relatively small region is under the control of specific TRE sequences and promoter context. These data indicate that this C-terminal region of RXR is likely involved in receptor-cellular factor(s) interactions. Finally, we propose that the synergistic activation by TR and RXR is achieved through elevated DNA binding and, dependent on the DNA sequence, the interaction of RXR with other transcription factors.

Solubilization and molecular characterization of the nitrobenzylthioinosine binding sites from pig kidney brush-border membranes.

The nitrobenzylthioinosine binding sites from luminal membranes of proximal tubule of pig kidney were solubilized by treatment of the brush-border membrane vesicles with the zwitterionic detergent CHAPS (3-[(3-cholamidopropyl)dimethylammonio]-1-propane sulfonate) in 2% solution. The high yield solubilization of a stable form of the transporter took place in the presence of adenosine in the medium of incubation with the detergent and the additional presence of glycerol as stabilizer. The solubilization of the NBTI-sensitive nucleoside transporter from pig kidney brush-border membranes did not change the nitrobenzylthioinosine (NBTI) binding characteristics; the only major change was a 3-fold decrease in the affinity. The carrier molecule was cross-linked to [3H]NBTI and by electrophoretic characterization under reducing conditions it displayed a molecular mass of 65 kDa. Treatment of the samples at low temperature prior to electrophoresis gave rise to the appearance of further bands corresponding to dimeric and tetrameric forms which interacted non-covalently. The removal of the N-linked oligosaccharides by treatment with endoglycosidase F shifted the molecular mass to 57 kDa. The chromatographic behaviour of the solubilized transporter was similar to that of human erythrocytes and differed from that found in pig erythrocytes. Since the molecular mass of the monomer before and after treatment with endoglycosidase F is the same for pig erythrocytes and pig kidney luminal membranes, the different chromatographic behaviour might result from tissue differences due to transcriptional variations or to posttranscriptional modifications of the transporter molecule.

Serotypes of CNF1-producing Escherichia coli strains that cause extraintestinal infections in humans.

The O:K:H serotypes of 137 necrotoxigenic Escherichia coli (NTEC) producing the cytotoxic necrotizing factor type 1 (CNF1) isolated from human extraintestinal infection were determined. Although NTEC producing CNF1 belonged to 58 different serotypes, only 10 of them accounted for 54% of strains. The most common serotypes, in order of frequency, were: O4:K?:H5, O6:K13:H1, O83:K1:H31, O75:K95:H5, O2:K1:H6, O2:K7:H-, O75:K1:H7, O2:K?:H1, O4:K12:H1 and O22:K13:H1. CNF1 strains of serotypes O2:K7:H- and O4:K12:H1 express P-fimbriae, whereas CNF1 strains of serotypes O2:K?:H1, O2:K1:H6 and O75:K95:H5 possess the adhesin responsible for MRHA type III. Among CNF1 strains of serotype O4:K?:H5 there exist some that express P-fimbriae and others that possess MRHA type III. Lastly, the majority of CNF1 strains of serotypes O6:K13:H1, O22:K13:H1, O75:K1:H7 and O83:K1:H31 do not express P-fimbriae nor the adhesin responsible to MRHA type III. Our results show that extraintestinal infections are caused by a limited number of virulent clones, as suggested by the theory of special pathogenicity.

In vitro and in vivo evidence for direct dopaminergic inhibition of VIP-immunoreactive pituitary cells.

In order to establish whether dopamine is involved in the regulation of immunocytochemical expression of VIP in pituitary cells, in vivo and in vitro studies were carried out on male rats after treatment with haloperidol and dopamine, respectively. In the in vivo studies, following intraventricular (third ventricle) administration of haloperidol, an increase in the total number of VIP-immunoreactive cells (p < 0.05) was observed; additionally, the cellular, cytoplasmic and nuclear areas were increased (p < 0.01). The in vitro studies demonstrated that dopamine induces a significant decrease (p < 0.05) in the total number of VIP-immunoreactive cells, which were smaller than the cells from the control dishes, because dopamine induces a significant decrease in their cellular, cytoplasmic and nuclear areas (p < 0.01). These results support the hypothesis that dopamine is a physiological inhibitor of pituitary VIP expression and suggest that dopamine modulates the auto- or paracrine effects of VIP in the rat pituitary.

Serotypes, toxins and antibiotic resistance of Escherichia coli strains isolated from diarrhoeic and healthy rabbits in Spain.

One hundred and ten Escherichia coli strains isolated from diarrhoeic and healthy rabbits from 50 Spanish commercial farms were serotyped and investigated for production of enterotoxins (LT and STa), verotoxins (VT), cytotoxic necrotizing factors (CNF1 and CNF2), alpha-haemolysin (Hly) and enterohaemolysin (EntHly), for necrotic and lethal activities and for antibiotic resistance. Six serogroups (O2, O26, O49, O92, O103 and O128) accounted for 81% (67 of 83) and 26% (7 of 27) respectively of E. coli strains isolated from diarrhoeic and healthy rabbits (P < 0.001). The most common serotypes found among E. coli strains associated with diarrhoeic rabbits in order of frequency were: O103:K-:H2, O49:K?:H2, O26:K-:H-, O26:K-:H11, O128:K?:H-, O92:K-:H2 and O2:K5:H6. E. coli strains belonging to the same serotype but from different farms usually showed a distint antibiotics resistance pattern. Only one strain, of serotype O2:K5:H6 was toxigenic (CNF1+, Hly+, necrotic and lethal).

Genes coding for Shiga-like toxins in bovine verotoxin-producing Escherichia coli (VTEC) strains belonging to different O:K:H serotypes.

Forty-six verotoxin-producing Escherichia coli (VTEC) strains isolated from diarrhoeic and healthy calves in Spain were examined for DNA sequences homologous to genes for verotoxins (VT1 and VT2) and enterotoxins (LT-I, LT-II, STaH, STaP and STb). Hybridisation showed that 26 (57%) of VTEC strains carried VT1 genes, 13 (28%) possessed VT2 genes, and 7 (15%) carried both VT1 and VT2 genes. No VTEC strains hybridised with DNA probes for enterotoxins. A correlation was found between the serotype and type of VT produced. Thus, all strains of serotypes O26:K-:H11 (13 strains), O103:K-:H2 (3 strains) and O128:K?:H- (4 strains) hybridised with the VT1 probe only, whereas all strains of serotypes O4:K-:H4 (3 strains) and O113:K-:H21 (4 strains) were positive with the VT2 probe only. By contrast, O81:K?:H28 (2 strains) and O157:K-:H- (2 strains) strains hybridised with both VT1 and VT2 probes. One strain of serotype O157:K-:H7 was VT2 positive.

Serotypes of bovine Escherichia coli producing cytotoxic necrotizing factor type 2 (CNF2).

The serotypes of 101 faecal bovine necrotoxigenic Escherichia coli (NTEC) producing the cytotoxic necrotizing factor type 2 (CNF2) were determined. Although, NTEC producing CNF2 belonged to 48 different O:K:H serotypes, only eleven of them accounted for 54% of strains. The most common serotypes in order of frequency were: O123:K-:H16, O3:K-:H21, O88:K-:H8, O15:K14:H21, O1:K-:H12, O1:K1:H46, O2:K1:H5, O55:H21, O88:K?:H25, O117:K?:H21 and O123:K-:H-. The serotypes of CNF2 NTEC were different from those found in NTEC producing CNF1 and in enterotoxigenic, verotoxigenic, enteropathogenic, enteroinvasive and enteroadherent E. coli strains that cause infections in humans and animals.

Regulation of nitrobenzylthioninosine-sensitive adenosine uptake by cultured kidney cells.

The effect of nitrobenzylthioinosine (NBTI) on [3H]adenosine uptake and the characterization of the [3H]NBTI binding in cell (primary cultures and LLC-PK1 cell line) plasma membrane and brush-border membrane (BBM) vesicles from pig renal cortices and LLC-PK1 cells was analyzed. [3H]adenosine uptake was strongly inhibited by NBTI in nonconfluent cells, whereas it was totally insensitive to the reagent in BBM. The concentration dependence of [3H]adenosine uptake in BBM was linear, suggesting simple diffusion. In both cell membranes and BBM high-affinity [3H]NBTI binding was observed. [3H]NBTI binding as well as NBTI-sensitive [3H]adenosine uptake was strongly reduced when cells grew to confluence. Both reduction effects were reproduced by treatment of nonconfluent cells with chlorophenyl adenosine 3',5'-cyclic monophosphate (cAMP), which indicates that the transporter is regulated by a cAMP-dependent protein kinase. To confirm this hypothesis, the binding of [3H]NBTI was analyzed in pig kidney BBM obtained in the presence of orthovanadate and alkaline phosphatase. With respect to control membranes, BBM obtained in the presence of orthovanadate showed a lower maximum number of binding sites (Bmax), whereas those obtained in the presence of alkaline phosphatase showed a slight increase in Bmax for [3H]NBTI binding. Taken together, these results suggest that the reduction in both [3H]NBTI-binding capacity and NBTI-sensitive [3H]adenosine uptake takes place by a mechanism that involves phosphorylation of the transporter molecule or of a protein that interacts with it.

Virulence factors and O groups of Escherichia coli strains isolated from cultures of blood specimens from urosepsis and non-urosepsis patients.

Ninety-six Escherichia coli strains isolated from cultures of blood specimens taken from both urosepsis patients (n = 48) and non-urosepsis patients (n = 48) were examined for the production of alpha-haemolysin (Hly) and cytotoxic necrotizing factor type 1 (CNF1), the expression of P-fimbriae and mannose-resistant haemagglutination (MRHA). Twenty-seven (56%) of the E. coli strains from urosepsis showed some of the virulence factors investigated, whereas only 15 (31%) of the strains associated with non-urosepsis possessed virulence factors (P < 0.05). By contrast, only 16% (P < 0.001) of the faecal isolates from healthy individuals were virulent. Of the bacteremic E. coli strains, 56 (58%) belonged to one of 8 serogroups (O1, O2, O4, O6, O8, O9, O18 and O83). Virulence factors were concentrated in strains belonging to serogroups O2, O4, O6, O18 and O83. Thus, 23 (72%) of the 32 strains of these 5 groups showed virulence factors, but only 19 (30%) of the 64 strains belong to other serogroups (P < 0.001). The majority of bacteremic O2, O4, O6 and O83 E. coli strains were Hly+CNF1+ and expressed P-fimbriae or MRHA type III, whereas the strains of serogroup O18 were Hly+CNF1- and had P-fimbriae or MRHA type III, whereas the strains of serogroup O18 were Hly+CNF1- and had P-fimbriae. We conclude that strains from urosepsis show more virulence factors than bacteremic strains isolated from non-urosepsis.

One year's experience with an acute pain service in a Spanish University Clinic hospital.

We describe the experience of the acute pain service of the University Hospital of Galicia, Spain since its inception. We have treated 1214 patients using either patient-controlled analgesia (PCA) with morphine (72%), or patient-controlled epidural analgesia with fentanyl + bupivacaine (22%). Three hundred and five patients had minor complications, mainly pruritus (35%) in patients with patient-controlled epidural analgesia. Three (0.33%) patients using PCA had respiratory depression treated with naloxone; no patient with patient-controlled epidural analgesia had respiratory depression. In our experience the creation of an acute pain service and the associated development of pain-treatment protocols and the training of hospital personnel produced excellent results.
