Rural health care requires a response system that is unique and substantially different from other, more traditional systems of health care delivery. Any reform of the present rural health care system must offer services that include the public health sector as well as other aspects of the social "safety net." The authors present a vivid picture of the multiple realities operating in the rural environment, and they explore the opportunity for the creation of Accountable Health Plans suitable for the rural health care delivery mechanism.

Thirty eight patients with neurofibromatosis 1 (NF1) had neurological examinations, intellectual assessments and MRI scans. Increased intensity lesions on T2 weighted images were found in 13 patients. These abnormalities were more common in patients aged under 18 years. The lesions occurred predominantly in the basal ganglia, brainstem and cerebellum, and were multiple in 11 patients. They did not produce symptoms or neurological deficit in any patient and did not enhance with gadolinium-meglumine-triamine-pentaacetic acid contrast medium (Gd-DTPA). In 2 patients, however, the abnormalities exerted mass effect distorting the brain and in 3 patients they occurred in conjunction with known gliomas. The lesions remained unchanged over a three year follow up period. The nature of the lesions is uncertain but the fact that they may produce mass effect and occur in association with gliomas suggests that they have malignant potential. There was no correlation between the presence of these abnormalities and intellectual impairment.

General practitioners and accident and emergency departments are often involved in the management of hand or finger sepsis. Such cases are usually easily diagnosed and treated. We report a more serious disorder which may mimic the condition and cause diagnostic confusion.
The use of zidovudine in drug misusers, especially current drug misusers, has not been extensively studied. Since periods of abstinence may be interspersed with drug misuse, it is necessary to establish the safety of zidovudine in injection drug misuse-related human immunodeficiency virus (HIV) infection under a variety of conditions. HIV serology became available in October 1985 and we have now examined medically 289 HIV seropositive patients, 85% of whom acquired their infection via injection drug misuse. Since March 1987 we have treated 40 individuals with zidovudine, 25 of whom were former or current injection drug misusers and one who was a heterosexual contact of a drug misuser. Eighteen patients were taking various types of opiates. Six of this latter group injected either occasionally or regularly whilst taking zidovudine. There were no adverse clinical events associated with zidovudine treatment and continued opiate drug misuse whether by mouth or by injection. Although defaults from clinic visits were a problem, these defaults were not associated with any particular form of drug misuse. Compliance with zidovudine therapy as judged by change in the mean corpuscular volume was no different for the various risk groups. In our experience it is possible to treat safely current and former drug misusers with zidovudine.

A number of computer programs exist for the determination of ejection fraction from gated blood pool images. Three commercially available algorithms were compared on the same radionuclide data from 50 catheterized patients. The methods used were: (1) simple thresholding, (2) second derivative edge determination, and (3) a combination of these two. There was good correlation with catheterization ejection fraction for all three methods using smoothed data [r = 0.781, 0.77, 0.84 respectively (P less than 0.01)]. However, interobserver variance was significantly decreased (P less than 0.01) by use of the second derivative method when compared with the other two methods and catheterization. Thus, if accuracy and low interobserver variance are required, then a combination of a second derivative and thresholding method is preferable.
Lorazepam was compared to placebo to assess its control of nausea and vomiting in patients receiving cytotoxic chemotherapy and prochlorperazine. The study design was a randomized, double-blind crossover in which three consecutive chemotherapy courses were compared so that each patient acted as his or her own control. Of 107 patients entered, 80 were evaluable for analysis. Lorazepam significantly reduced the severity and duration of nausea, the severity of vomiting, and the number of vomiting episodes when compared to placebo. Anxiety was reduced during lorazepam courses but not significantly when compared to placebo. There was significantly more sedation with lorazepam courses. Overall, patients preferred lorazepam courses although this preference was significant only in the patient subset receiving doxorubicin and cyclophosphamide. Lorazepam is a useful adjunct to prochlorperazine in patients receiving cytotoxic chemotherapy.
Testosterone undecanoate (Restandol, Organon Laboratories Ltd) dissolved in oleic acid, was administered orally to seventy-six hypogonadal males for three consecutive 3-week periods and the subjective clinical response assessed by a standard interview. Plasma testosterone and testosterone undecanoate levels were determined by radioimmunoassay before the study and after 3, 6 and 9 weeks treatment. The treatment was effective in sixty of the sixty-six patients who completed the trial. Ten patients did not complete the trial; two for reasons unrelated to the drug and eight because of side effects, mainly gastro-intestinal. There was a significant rise in plasma testosterone levels during treatment and a positive correlation between plasma testosterone and testosterone undecanoate levels. Testosterone undecanoate is a potentially valuable drug for the oral treatment of male hypogonadism.
