Evaluation of a lysis-centrifugation system for recovery of yeasts and filamentous fungi from blood.

A lysis-centrifugation system (Isolator; E. I. du Pont de Nemours & Co., Inc., Wilmington, Del.) was compared with a biphasic brain heart infusion (BHI) medium in a prospective study of 5,125 fungal blood cultures. The Isolator recovered 90.3% of the positive cultures, compared with 63.4% recovered by the biphasic BHI medium. Overall, the detection of fungemia was increased 36.6% by the Isolator. Mean recovery times for yeasts were 2.12 and 4.90 days for the Isolator and BHI bottles, respectively. Cultures of Histoplasma capsulatum required 8.0 and 24.14 days for recovery by the Isolator and BHI bottles, respectively. The Isolator provided a more rapid and sensitive means of detecting organisms associated with fungemia.

Protection against chronic pyelonephritis in rats by suppression of acute suppuration: effect of colchicine and neutropenia.

Previous experiments in rats have suggested that renal scarring after acute, obstructive pyelonephritis due to Escherichia coli results from parenchymal damage due to acute inflammation and suppuration. To assess the role of acute infiltration by polymorphonuclear leukocytes (PMNLs) in the pathogenesis of chronic pyelonephritis (CPN), rats were either treated with colchicine to depress leukocyte motility or rendered neutropenic with a single dose of cyclophosphamide. Colchicine given during acute pyelonephritis reduced kidney inflammation and protected against CPN two months later. Similarly, neutropenia reduced acute inflammation and protected against chronic parenchymal destruction and scarring. Protection against renal scarring in both colchicine-treated and neutropenic rats occurred despite higher renal bacterial counts during acute pyelonephritis. These experiments provide further evidence that CPN (renal scarring) results from kidney damage that occurs during early acute obstructive pyelonephritis. This damage appears to result from infiltration of the kidney by PMNLs rather than direct damage from bacterial infection.

Polysaccharide (amylopectin-like) storage myopathy histochemical ultrastructural and biochemical studies.

A case of an adult polysaccharide myopathy is reported in a patient with progressive muscular atrophy and weakness of limb girdles. Histochemistry and electron microscopy showed in some muscle fibers, a storage material composed of amylopectin-like filaments. Biochemical results were normal and no enzyme deficiency was found. This case is compared with three other published cases. Pathological conditions with amylopectin or amylopectin-like storage material are reviewed.

Prevention of acute and chronic ascending pyelonephritis in rats by aminoglycoside antibiotics accumulated and persistent in kidneys.

Gentamicin, because it is stored in renal tissues, can prevent acute retrograde pyelonephritis. Since different aminoglycosides accumulate and persist to various degrees in the kidney parenchyma, the prophylactic activities of gentamicin, tobramycin, amikacin, and netilmicin were compared. The four antibiotics were given intramuscularly to rats 3 days before initiating ascending unilateral pyelonephritis with Escherichia coli. Despite different degrees of renal accumulation at the time of infection (tobramycin and amikacin accumulated significantly less), all four aminoglycosides displayed similar protection against ascending pyelonephritis. This protection was conferred in the absence of active antibiotic detectable in the urine and was therefore attributed to antibiotic stored in the renal parenchyma. Those animals that developed pyelonephritis despite aminoglycoside prophylaxis had less severe acute kidney infection and inflammation. This resulted 3 months later in an almost complete protection against renal scarring (chronic pyelonephritis). These results in rats suggest that renal accumulation and persistence of aminoglycosides may be used to advantage in the prophylaxis or in the treatment of kidney infections.

Dental caries in a group of 20-year-olds after previous participation in public child dental health services in Copenhagen, Denmark.

The present study was undertaken to investigate the prevalence of dental caries in a group of 20-year-olds who had previously participated in Public Child Dental Health Services. Dental caries was related to social status and preventive vists to the private dentist after termination of school. The study population comprised 389 persons aged 20, out of which 313 (80.5%) were examined clinically and radiographically by one dentist according to standardized criteria. Mean DMFT was 10.4 and mean DMFS 16.7. Differences in relation to sex were not significant. The distribution of subjects according to DMFS was uneven. The 20% with the highest DMFS accounted for about 45% of the total amounts of DS. A consistent pattern with higher mean DMFS in the low social groups was found. Attendance to Regular Youth Dental Service was higher the longer the subjects had been students and higher mean DMFS was found in nonregular attenders. The presence of a high caries risk group is discussed in relation to the dental care system received by the study population.

Evaluation of a calibration trial to increase interexaminer reliability of radiographic diagnosis of approximal carious lesions.

Eighteen dentists working in the same School Dental Service participated in a calibration trial with the purpose of reducing interexaminer disagreement on radiographic diagnosis of approximal caries. The calibration program was in two stages: firstly, discussion in small groups and, secondly, the use of a reference standard. The effect of the training programs on interexaminer disagreement was limited, but a statistically significant decrease in the number of D-surfaces recorded per child was observed. The finding that the major part of the error variance was due to random error, rather than systematic error, may explain why the calibration program was unsuccessful at reducing error.

Development and distribution of proximal caries in 303 9-20-year-old individuals in a Copenhagen suburb.

The purpose of the present study was to establish a theoretical basis for the practice of screening for identification of caries risk groups. Longitudinal data concerning the development of proximal caries in 303 persons from the age of 9 to the age of 20 were examined with regard to statistical distribution. Data from each year and from the entire period showed a close fit to the negative binomial distribution. This distribution can be the result of independent random occurrences, but varying susceptibility. Thus the consistent existence of a caries risk group is illustrated by this analysis, but no prediction is made. It is suggested that future evaluations of preventive measure directed toward caries risk groups should express the degree to which the similarity between the distribution of proximal caries and the negative binomial distribution can be eliminated.
