An algorithm for acute wrist trauma. A systematic approach to diagnosis.

A systematic approach to the acute traumatized wrist is of importance to achieve early diagnosis, with efficient use of advanced imaging modalities. In a 6-month period 641 patients aged 15 years or over were examined using an algorithm for acute wrist trauma in order to secure early and appropriate treatment. In 293 (46%) cases routine radiographic examination supplied sufficient information to establish a fracture diagnosis. All 33 scaphoid fractures (including five scaphoid non-unions) were demonstrated on the initial X-ray examination. In 56 patients (9%) scintigraphy proved useful for selecting cases in need of further radiographic examination. Among 31 patients with focal activity on bone scintigraphy, CT and/or tomography revealed eight distal radial fractures, one fracture of the ulnar styloid, and eight fractures of carpal bones. Fractures were revealed using tomography and/or "hot spot views" in patients with negative CT examinations suggesting that further examinations should be performed in cases with increased focal activity on bone scintigraphy and a negative CT examination.

Effects of nitrendipine on blood pressure, renin-angiotensin-system and atrial natriuretic peptide in hypertensive type I diabetic patients.

In this study, the effects of a six week treatment with the calcium channel blocking agent nitrendipine were assessed in 20 hypertensive type I diabetic patients who received a single oral dose of 20 mg daily. Plasma concentrations of atrial natriuretic peptide (ANP), plasma renin activity (PRA) and active renin, aldosterone, glycosylated hemoglobin (HbA1) and fructosamines were determined at the beginning and the end of the study. Interactions of these endocrine parameters with blood pressure behaviour were analyzed by a correlation matrix. In response to the drug treatment, the blood pressure was significantly lowered from 158.0/97.2 to 145.7/88.2 mmHg. The plasma concentrations of ANP were also significantly reduced from 106.8 to 89.7 pg/ml. There were no changes in serum aldosterone, while PRA and active renin exhibited a significant increase following the six week treatment. HbA1 and fructosamines remained unaltered. There were no significant correlations for ANP and blood pressure values, as well as for ANP and PRA or aldosterone. We did find, however, a significant correlation of the ANP values with the difference of the systolic blood pressure levels pre- and post-treatment. These data fully confirm the blood pressure lowering properties of this calcium channel blocker and its possible interference with steroidogenesis, since the effects of increased PRA on aldosterone secretion were clearly blunted. The lowering of plasma ANP levels may represent a decreased ANP secretion due to calcium channel blockade or might also be due to the natriuretic effects of nitrendipine, thus allowing ANP levels to decline as a function of lessened sodium retention.(ABSTRACT TRUNCATED AT 250 WORDS)

Long-term function of porcine islets and single cells embedded in barium-alginate matrix.

The influence of alginate-embedding on the maintenance of functioning and morphological integrity in long-term culture of isolated porcine islets and islet cells was studied. Free-floating islets and islet cells served for control. Function was tested after the 1st, 2nd and 4th week. Basal and glucose-stimulated insulin secretion of embedded islets decreased slightly, but significantly after the first week (from 4.39 +/- 0.64 to 2.87 +/- 0.47 at normal and from 11.96 +/- 1.44 to 4.76 +/- 0.78 microU pro 24 h pro islet at elevated glucose concentration, p < 0.05 and < 0.01, resp.) and remained unchanged thereafter. Glucose-stimulation resulted in significant increases in insulin secretion at all three testings (p < 0.001, < 0.01 and < 0.01). Single cells in alginate matrix had even more stable insulin secretion throughout the whole cultivation with significant increases to glucose challenge (p < 0.01, < 0.01 and < 0.05). In contrast, insulin secretion of free-floating islet cells decreased from 5.70 +/- 1.19 to 2.04 +/- 0.64 and to 1.05 +/- 0.33 at basal conditions (p < 0.01 and < 0.05) and from 11.39 +/- 1.87 to 2.76 +/- 0.76 and to 2.15 +/- 0.71 microU pro 24 h pro islet under stimulation (p < 0.01 and not sign). In addition, the secretory response to glucose challenge was significant only at the first testing (p < 0.05). Non-embedded islets could be tested only at the first week since after this time they dissociated to single cells. Embedded islets and single cells showed intact morphology after four weeks with trypan blue (TPB) positivity of less than 5%.(ABSTRACT TRUNCATED AT 250 WORDS)

Retrobulbar adipocytes and humoral immunity in Graves' ophthalmopathy.

As CT and MR-imaging revealed an enlargement of retrobulbar fat tissue in patients with Graves' ophthalmopathy, the role of the retrobulbar adipocytes in the pathogenesis of this disorder remains to be elucidated. To evaluate the in vitro influence of humoral immunity on retrobulbar adipose tissue, the effects of IgG and sera from 56 euthyroid ophthalmopathy patients and 53 controls on both porcine and human (patients' and controls') retrobulbar adipocytes were measured by means of several assays: An enzyme-linked immunosorbent assay was employed to reveal specific binding of antibodies to the adipocytes. Metabolic activity was determined by means of a colorimetric dimethyl thiazolium-diphenyl-tetrazolium bromide test which quantifies the activity of mitochondrial dehydrogenases; cell proliferation was measured by incorporation of [3H]-thymidine in 24 h, and activities of adipocyte specific enzymes, such as membrane-bound lipoprotein lipase and 1-glycerol-3-phosphate-dehydrogenase were determined. By means of these specific enzyme tests no distinctions could be made between patients and controls. Furthermore, a significant difference between patients' (untreated and treated) and controls' IgG to bind to, to activate or to stimulate the proliferation of porcine or human (patients' and controls') retrobulbar adipocytes could not be detected under the employed experimental conditions. The effects of patients' heat-inactivated and non-inactivated sera were indistinguishable from those of the controls. Incubation with autologous sera, however, led to an activation of the retrobulbar adipocytes which was higher than the median caused by the patients' group and that engendered by incubation with autologous IgG.(ABSTRACT TRUNCATED AT 250 WORDS)

Glycosaminoglycan antibodies in endocrine ophthalmopathy.

An increased accumulation of glycosaminoglycans (GAG) in the orbita has been reported in endocrine ophthalmopathy (EO). In this study we investigated whether antibodies directed against GAG are present in the sera of 52 EO patients and 47 healthy controls. Three out of 52 patients exhibited low titers of antinuclear antibodies and all patients were negative for antibodies against extractable nuclear antigens. Isotype IgG antibodies were detected by means of an ELISA using hyaluronic acid and dermatan sulfate as antigens. Values were expressed as optical density at 405 nm. In comparison to the control group (0.445, 0.364, 0.588; median, 25th, 75th percentile) significantly (p < 0.001) higher hyaluronic acid antibody levels were found in EO patients (0.626, 0.473, 0.801). Untreated patients had significantly higher hyaluronic acid antibody values compared to controls (0.770 vs 0.437, median, p < 0.005). Thyroid status or antithyroid treatment did not influence binding of hyaluronic acid antibodies. When screening for dermatan sulfate antibodies no such difference could be observed between patients and controls. Thus, the detection of GAG antibodies in EO patients emphasizes the important role of glycosaminoglycans in the pathogenesis of thyroid eye disease.

[Selective blood sampling from the sinus petrosus inferior: a comparison of CRF and TRH stimulation].

In 10 patients with hypophyseal Cushing microadenomas, selective bilateral sampling from the inferior petrosal sinuses was performed and the effect of stimulation by iv TRH and CRF was compared. On the side of the microadenoma. ACTH concentration rose from 650 +/- 242 pg/ml to 2712 +/- 843 pg/ml following injection of CRF and 2025 +/- 242 pg/ml after TRH. Contralateral values were 165 +/- 79 pg/ml, 490 +/- 200 pg/ml and 165 +/- 72 pg/ml respectively. Prolactin concentration on the side of the adenoma was 98 +/- 49 ng/ml before stimulation, 236 +/- 62 ng/ml after CRF and 747 +/- 168 ng/ml after TRH. Contralateral concentration was 22 +/- 10 ng/ml, 64 +/- 19 ng/ml respectively. Sampling localised all adenomas correctly, whereas contrast-enhanced MRT diagnosed only four adenomas.

Automated processing of human bone marrow grafts for transplantation.

Prior to purging or cryopreservation, we concentrated 21 bone marrow (BM) harvests using a modification of the 'grancollect-protocol' of the Fresenius AS 104 cell separator with the P1-Y set. Within 40-70 min, the initial marrow volume of 1,265 ml (+/- 537 ml) was processed two to three times. A mean of 47% (+/- 21%) of the initial mononuclear cells was recovered in a mean volume of 128 ml (+36 ml). The recovery of clonogenic cells, measured by CFU-GM assays, was 68% (+/- 47%). Red blood cells in the BM concentrates were reduced to 7% (+/- 4%) of the initial number. The procedure was efficient and yielded a BM cell fraction suitable for purging, cryopreservation and transplantation. At this time, 10 of the 21 patients whose BM was processed using this technique have been transplanted. Seven of these 10 patients have been grafted using the BM alone. Three of the 10 patients showed reduced cell viability and colony growth in the thawed BM samples, and therefore obtained BM and peripheral blood-derived stem cells. All transplanted patients showed an evaluable engraftment, achieving 1,000 granulocytes per microliter of peripheral blood in a mean of 18 days.

Blood donations indeterminate in HIV-1 western blot analysed by IgM immunoblot and polymerase chain reaction.

The presence of IgM antibodies to human immunodeficiency virus 1 (HIV-1) was investigated in blood donor sera which were indeterminate in anti-HIV-1 IgG Western blot testing. In 7 of 173 instances out of approximately 1,000,000 blood donation sera with an isolated anti-p24 IgG produced an anti-gp41-45 IgM immunoblot reaction. Applying polymerase chain reaction (PCR) to 29 indeterminate samples out of approximately 125,000 blood donations it was found that 2 of them were IgM-positive and also contained HIV-1-specific DNA sequences. Eleven months later 1 of these 2 donors was retested and found IgM and PCR negative.

Aerosol amphotericin B for prevention of invasive pulmonary aspergillosis.

The pharmacokinetics and applicability of aerosol amphotericin B administrations were studied in 40 neutropenic patients and 4 healthy volunteers. Particle size was measured and pulmonary deposition was demonstrated by radioisotope studies. Inhalations were easy to administer and were well tolerated, with minimal systemic absorption of the drug.

Serum concentrations of G-CSF during high-dose chemotherapy with autologous stem cell rescue.

In 20 patients treated for refractory germ cell tumors, endogenous G-CSF levels were monitored before and after high-dose chemotherapy with autologous stem cell rescue (ASCR). An inverse relationship was found between endogenous G-CSF levels and peripheral blood leucocyte counts. G-CSF levels were not detectable prior to ASCR, but increased to a maximum of 3400 pg/ml (range 120-15000 pg/ml) thereafter. There was no relationship between peak G-CSF levels and time to engraftment. A decrease in G-CSF levels preceded engraftment and was paralleled by a rise of nadir leucocyte counts.

[Walkaway systems in general practice: Behring ELISA Processor III].

The evaluation of a new Walkaway instrument (Behring Elisa Processor III) for testing HBsAG, anti-HCV and anti-HIV 1/2 in three blood transfusion services with different sample volumes (from 60,000 to 450,000/year) gave the following results: the instrument fulfills the promises of being a real Walkaway system under routine conditions, saving labor and gaining security. The instrument must work in agreement with GLP rules, and all test steps are documented. The organization of the laboratory becomes more transparent and less flexible.

[Cushing syndrome in ACTH-producing neuroendocrine tumor of the hepatic duct].

We present a patient with an ACTH producing neuroendocrine tumor of the hepatic bile duct which was detected by chance during abdominal surgery for Cushing's syndrome. Diagnostic strategy and surgical therapy in patients with neuroendocrine tumors are discussed. The diagnostic problems caused by combination with an endocrine function disorder like Cushing's syndrome are pointed out.

[Prophylactic inhalation of amphotericin B in invasive aspergillosis: a distribution study].

Invasive pulmonary aspergillosis (IPA) during periods of immunosuppression or prolonged neutropenia is a serious condition with high mortality. Prophylaxis is unsatisfactory because of the low effectiveness of systemic administration of amphotericin B (AMB) and because of its side effects. Prophylactic inhalation of AMB by means of a nebulizing system may be able to reduce the incidence of IPA. Therefore, we studied the distribution of nebulized 99mTc-labeled AMB and estimated its particle size. AMB is homogeneously distributed in the lung and partly deposited in the terminal respiratory unit; it remains there with a half-life of at least 14 h. Therefore, prophylaxis of IPA with AMB nebulized as described can be recommended.

[Experimental reproduction of the respiratory form of infection with the agent of epidemic late abortion of swine].

Porcine reproductive and respiratory syndrome has been observed in a large swine-farm. Diagnostic investigations were undertaken. Piglets infected with homogenate from the lung of a sick gilt developed biphasic fever up to 42 degrees C, inappetence and apathy. A few piglets showed red-blue discolored parts of the ears and of the skin in the abdominal region. A cytopathogenic agent has been isolated on cultivated alveolar-macrophages. Infection of another group of piglets with macrophages-propagated agent resulted in similar clinical signs too. Organs of euthanatized piglets have been examined anatomical, histological and with the fluorescent antibody technique. Specific fluorescence could be seen in tonsils, bronchial lymph nodes, spleen and in pathological-changed parts of the lungs. Antibodies against PRRS-virus were detected in blood samples of experimentally infected pigs.
