[Venous and intragastral pressure measurements in surgical correction of abdominal wall defects].

Our own experiences with venous and intragastral pressure measurements when correcting abdominal wall defects and their consecutive symptoms are shown. The exact measurement of pressure in the V. cava inf. is a good method for the assessment of the intraabdominal pressure ratios. The intragastral pressure measurement is demonstrated as an alternative measuring technique. As a non-invasive method, it is without risk; it is easy to perform and correlates with the intra-abdominal pressure.

[Organ-preserving therapy of splenic injuries in childhood--radiologic and immunologic follow-up studies].

Care of traumatized spleen has undergone an interesting change. In view of the fact that children without spleen are exposed to an increased infection risk, attempts are now directed at preserving the organ. The author's own patients included 9 children whose spleen had been traumatized at an age between 5 4/12 and 11 7/12 years. 4 children were given conservative treatment under intensive care, whereas surgery of the traumatized spleen was performed five times. 7 children were followed up. The changed morphological structure and the immunological situation were factors of particular interest. Depending upon the degree of severity of the injury to the spleen, scars remain in the parenchyma which are of different extension and which are associated with corresponding storage defects. The humoral and cell-promoted immunity is maintained.

Age Factors↗

[The behavior of hemodynamic parameters during surgery in childhood].

Mechanical cardiographic analyses of the circulation were carried out intraoperatively and directly after the operation on 130 children of 11/12 to 15 2/12 years of age. Essential haematodynamic changes are represented by a rise in the cardiac rate, decline of stroke volume, changes in cardiac work and output, and in speed of contraction compared with initial preoperative values. Triggering factors are discussed. A healthy normal circulatory system will tolerate these reversible disturbances. Decompensation may occur in children with a primary decline in cardiac output.

[Hand burns in children. Personal experiences and follow-up study results].

The analysis of 90 thermal injuries of the hands leads to the following statement: First and second degree burns heal spontaneously by closed wound treatment. Third degree burns demand local surgical treatment, usually according to the principle of delayed early excision and consecutive skin transplantation. Contracted scars have to be corrected early by surgical plastic. Full thickness skin transplants and flap shifting techniques provide satisfactory results. Z-plastics tend to recurrent contraction. After-care for preserving the functional results is of great importance for the growing hand.

[Testis biopsy and measurement of its volume in cryptorchidism (author's transl)].

The recommendation of an optimal time for operation of cryptorchidism is based on 70 testis biopsies and 40 measurements of testis volume. Orchidopexy should be performed during the 3rd to the 4th year of age, because impairment of the testes is not to be expected until the beginning of the 3rd year of life. Orchidopexy within the first two years of life may cause additional traumatization of the tissue.

[Juvenile bone cysts (author's translation)].

37 young persons suffering from juvenile bone bone cysts underwent surgery. The operative treatment consisted of meticulous excochleation and refilling of the cavity with homologous or autologous cancellous bone chips. Relapses occurred in 14 cases twice. Allogenous material suits best for primary treatment. In case of relapse autogenous bone chips sould be used.

[Cardiorespirographical monitoring in neonatal pre- and postoperative care (author's transl)].

A further improvement of the chance of survival in neonatal surgery can be achieved by close cardiorespirographical observation. Neonates with adaptation problems should be operated upon -- if malformation permits surgery and there is no absolute urgency -- not before normalization of the cardiorespirogram. Stress of operation and anaesthesia lead to cardiac dysregulations in the very early postoperative stage. Deceleration of the heart beat rate due to respiration or a silent cardiogram with decrease of the beat rate and respiratory depression should be regarded as dangerous disturbances.

Anesthesia↗

[Therapeutic problems in the "short-bowel-syndrome" (author's transl)].

The authors report on 6 children with short-bowel-syndrome. The expectancy of life of the patients depends on the mucosal surface of the residual intestine and on the degree of adaption. After removal of larger proportions of the intestine, nutrition should be started parenterally and followed by an early but cautious oral feeding. The administration must be distributed continuously over the day. At first carbohydrates are given as monosaccharides, fats as M. C. T. and proteins as amino acid mixtures. Carrot soup has proved useful as a water-binding substance for the improvement of stool consistency. An anabolic condition of the metabolism should be maintained. Therapy with cholestyramin is indicated in chologenic diarrhoea. Substitution of vitamins, especially vitamin B12 is very important after resection of the ileum.

[Funnel chest operation and age (author's transl)].

Analysing 199 cases of funnel chest operations the following proposals are to be discussed: early operation of boys in a modified Ravitch-technique with transverse osteotomy of the anterior sternum-wall, resection of the xiphoid process and fastening of the perichondrium to the pectoral muscles. The cosmetic indication excludes a late operation. If any late operations are performed they should be considered only for girls. Stabilisers should be used only in case of later operations, recurrences and asymmetrical forms. No operation should be performed in case of genetic defects (malformationsyndrome, flat-chest and channel-chest). The therapeutic results should be made objective by means of the sagittal inner thorax-diameter after Stucki. The submammary incision should be omitted.
