System, environmental, and policy changes: using the social-ecological model as a framework for evaluating nutrition education and social marketing programs with low-income audiences.

A variety of nutrition education interventions and social marketing initiatives are being used by the Food Stamp Program to improve food resource management, food safety, dietary quality, and food security for low-income households. The Social-Ecological Model is proposed as a theory-based framework to characterize the nature and results of interventions conducted through large public/private partnerships with the Food Stamp Program. In particular, this article suggests indicators and measures that lend themselves to the pooling of data across counties and states, with special emphasis on systems, environment, and public policy change within organizations at the community and state levels.

A growth factor mixture that significantly enhances angiogenesis in vivo.

Studies of therapeutic angiogenesis have generally focused on single growth factor strategies. However, multiple factors participate in angiogenesis. We evaluated the angiogenic potential of a growth factor mixture (GFm) derived from bovine bone. The major components of GFm (SDS-polyacrylamide gel electrophoresis, mass spectrometry, and Western blot) include transforming growth factor-beta1-3, bone morphogenic protein-2-7, and fibroblast growth factor-1. GFm was first shown to induce an angiogenic response in chorioallantoic membranes. Next, myocardial ischemia was induced in 21 dogs (ameroid) that were randomized 3 weeks later to received GFm 1 mg/ml (I), GFm 10 mg/ml (II), or placebo (P) (with investigators blinded to conditions) injected in and adjacent to ischemic myocardium. Dogs were assessed 6 weeks later using quantitative and semiquantitative measures. There were GFm concentration-dependent improvements in distal left anterior descending artery (LAD) opacification by angiography (P: 0.4 +/- 0.2, I: 1.1 +/- 0.14, II: 1.6 +/- 0.3, angiographic score p = 0.014). Histologically, there was also concentration-dependent vascular growth response of relatively large vessels (P: 0.21 +/- 0.15, I: 1.00 +/- 0.22, II: 1.71 +/- 0.18, vascular growth score p = 0.001). Resting myocardial blood flow (colored microspheres) was not significantly impaired in any group. However, maximum blood flow (adenosine) was reduced in ischemic territories and did not improve in GFm-treated hearts. GFm, a multiple growth factor mixture, is a potent angiogenic agent that stimulates large vessel growth. Although blood flow did not improve during maximal vasodilatory stress, large intramyocardial collateral vessels developed and angiographic visualization of the occluded distal LAD improved significantly. The use of multiple growth factors may be an effective strategy for therapeutic angiogenesis provided a more effective delivery strategy is devised that can achieve improved maximum blood flow potential.

An exploratory study of recreational drug use and nutrition-related behaviors and attitudes among adolescents.

This study examined drug use and eating behaviors among adolescents. The data were collected by phone interviews from 401 northern Nevadan students in grades seven to twelve. Students were divided for comparison into three groups according to their involvement with drugs: Abstainers, conventional users, and high-risk users. Analyses indicated that high-risk users less frequently ate lunch, meals at home, and with their families, and ate more often at convenience stores, fast food restaurants, and with friends. In addition, female high-risk users had significantly more negative perceptions regarding their food choices than the other female groups, and were more concerned with dieting than their high-risk using male peers. Male and female high-risk users believed that their drug use affected their eating habits. Implications for prevention programming and future research are discussed.

The effectiveness of adherence intervention in a colon cancer prevention field trial.

BACKGROUND: Adherence interventions were implemented in a 1-year community-based colon cancer prevention clinical trial (n = 110) using wheat bran fiber and calcium dietary supplements. The adherence promotion strategy was guided by a theoretical model. METHODS: The adherence intervention contains both a generalized portion given to all participants and an individualized portion given to marginal (50-74% intake) and low (under 50% intake) adherers. A regression model was employed to assess the effectiveness of the interventions both at the first intervention and at subsequent times. RESULTS: The Health Behavior in Cancer Prevention Model-based adherence promotion intervention was associated with retention of participants, both during the run-in period and after randomization (P = 0.05); and maximization of the percentage of the 13.5-g recommended fiber supplement consumed during the trial (92.5%). The positive effects of the adherence intervention were greater with first-time nonadherers and the control group than with the experimental group. The high-fiber group had notably more biological GI effects from the increased fiber intake, more preexisting comorbidities, and lower perceived cognitive and physical health status. CONCLUSIONS: Randomized participants had excellent adherence overall. Retention rates in the trial were better than would be expected without the adherence intervention, especially among those participants who may have been at higher risk for dropping out of the study. This suggests that a systematic, theoretically based adherence strategy should be further tested in clinical trial settings in which lower adherence is a problem.

Reasons related to adherence in community-based field studies.

This study identified participants' reasons for good, marginal or poor adherence, or withdrawing from community-based clinical studies using a dietary and/or drug intervention. Adults aged 48-75 years participated in one of three studies related to decreasing colon polyp recurrence. Qualitative data from progress notes (N = 675) and end-of-study evaluations (N = 87) were coded using constant comparative analysis with 100% content validity panel agreement. Most common reasons for non-adherence were barriers such as side-effects, interference with vacation plans, unrelated illness, forgetting and competing outside stressors. Participation motivators were benefits such as altruism, medical benefits, free service and staff rapport. Findings supported the Health Behavior in Cancer Prevention model-based approach to adherence interventions and provided directions for adherence promotion in future community-based clinical studies.

Care needs of home-based cancer patients and their caregivers. Quantitative findings.

The purpose of this study was to identify care needs of home-based patients receiving treatment for cancer and their caregivers. The sample consisted of 30 patients receiving radiation therapy and/or immunologically based treatments for a variety of cancers and 29 caregivers. Patient and caregiver needs scales were used to elicit the needs of patients and caregivers. Patients and caregivers were able to cite their unique needs. Patients had more needs for themselves in the areas of personal care, activity management, and interpersonal interaction than did the caregivers. The necessity for individualization of care was noted by both patients and caregivers, an area that nursing is in a unique position to address.

A community-based feasibility study using wheat bran fiber supplementation to lower colon cancer risk.

METHODS: In this feasibility study, free-living older adults (n = 180; means = 67.5 years old) were randomly assigned to one of three levels of a 3-month standardized compliance enhancement program. RESULTS: Regarding subject compliance with the 18 g/day wheat bran fiber supplement, the high compliance enhancement group had a superior regimen compliance rate (88%) versus the medium and low groups, (66 and 29%, respectively) (P = 0.01), with similar attrition rates. CONCLUSION: No significant gastrointestinal side effects and changes in body weight were reported. For similar efficacy, the comprehensive compliance enhancement group had the greatest cost effectiveness.

An exploratory investigation of elders' views on diet, nutrition and cancer prevention.

The goal of this exploratory investigation was to describe views related to diet and cancer prevention among the residents of a southwestern retirement community. Ethnographic interviews were conducted with twelve residents, providing over 500 data bits for this investigation. Several categories representative of the elders' views related to diet and cancer prevention were identified using content analysis. These included Determinants of food choices, Vigilance and diet and Expectations related to diet and cancer prevention. Two cultural themes illustrated the semantic relationships among the categories: "It's not easy to change your diet, especially when you are all alone," and "Eating right helps you resist disease but it is just too late for us." As diet becomes a primary means of reducing one's risk for certain types of cancer, the informed expertise of nutrition educators and health practitioners is needed to assist the public in adopting dietary guidelines. Qualitative data, as presented here, will be useful in developing effective education materials to meet this need.

Effects of diltiazem on anoxic injury in the isolated rat heart.

The effect of diltiazem, a calcium channel blocking agent, on anoxic injury was studied in isolated perfused rat hearts. Anoxia for 60 minutes caused a considerable release of creatine kinase and significant cell injury. Reoxygenation of these anoxic hearts for 20 minutes accelerated the creatine kinase release and caused severe cell injury. Reoxygenation of anoxic hearts with diltiazem at a rate of either 2 or 4.5 mg/liter did not reduce creatine kinase release significantly (probability [p] greater than 0.05). However, the higher dosage of diltiazem (4.5 mg/liter during both anoxic and reoxygenation phases resulted in significant (p less than or equal to 0.05) preservation of healthy tissue. The data suggest that diltiazem in the higher concentration prevents cell injury and reduces mitochondrial damage in anoxic injury.

Epidemiology study of the use of asbestos-cement pipe for the distribution of drinking water in Escambia County, Florida.

Cancer mortality for the population census tracts of Escambia County, FL, which use asbestos-cement (AC) pipe for public potable water distribution, was compared with cancer mortality data collected from census tracts in the same county where other types of piping materials are used. An analysis of covariance was run to test for differences in standard mortality ratios for seven cancer sites among three potential asbestos exposure groups based on AC pipe usage. Twelve variables representing nonexposure-related influences on disease rates were combined in four independent factors and used as covariates in these analyses. No evidence for an association between the use of AC pipe for carrying drinking water and deaths due to gastrointestinal and related cancers was found. The limitations on the sensitivity of the analysis are discussed.

Ultrastructural alterations of the mitochondrial ATPase in the calcium paradox as revealed by negative staining.

We have used a simple negative staining technique to study the structural alterations of mitochondria from biopsies of hearts subjected to the calcium paradox and treatment with diltiazem, a calcium channel blocker. A significant (P less than 0.05) decrease in the number of spheres on the mitochondrial membranes occurs during the calcium paradox (58.0 +/0 4.1/micrometer vs. control 80.5 +/- 6.5). Treatment with diltiazem prevented the loss of spheres from mitochondrial membranes during the calcium paradox (75.5 +20 5.0 micrometer). We found that this negative staining technique can be used for quick assessment of the condition of mitochondria in biopsies from normal and pathological organs.

Surface ultrastructural features of isolated perfused rat hearts during calcium paradox.

Cellular damage caused by calcium (Ca++) paradox is of significant importance under both experimental and clinical conditions. Electron microscopy was used to study the surface changes of the isolated rat hearts perfused with normal Krebs-Henseleit (KH) medium, Ca++-free KH medium, Ca++-free KH medium followed by normal KH medium. The hearts perfused with Ca++-free medium and normal KH medium showed elongated parallel myofibrils with transverse ridges spaced at regular intervals. The major change in Ca++- free hearts consisted of wide cellular separation. For the Ca++- paradox hearts, wht myofibrils were twisted and narrowed at several locations. The myocardial fiber surfaces were bulged out, frequently with sarcolemmal ruptures and holes. Supercontraction of cells caused breaks in the sarcolemma and aggregation of mitochondria at the cell periphery. These results suggest that Ca++-depletion primarily affects the myocardial cell junctions which, following reintroduction of Ca++, accelerate the entry of Ca++ into the cells, supercontraction of cells and stretching of sarcolemma.
