In patients subjected to bilio-intestinal anastomosis as treatment of several diseases of the biliary tract and the Oddi's sphincter, with some frequency have symptoms that make suspect that the anastomosis is stenosed. The experience obtained in the evaluation of 8 cases is presented. The method used was radioisotopic cholangiography, with the administration of pyridoxiphenylalanine labeled with 99m-Technetium. Serial images were obtained in Polaroid film. In three of the eight patients, an obstruction of the principal bile duct was shown. After the operation, there was normal patency of the anastomosis. In the other cases, there was normal patency of the anastomosis, and the symptoms were due to other causes. We consider the radioisotopic cholangiography as the method of election in the study of these patients, as the results may indicate a surgical re-intervention, avoid another harmful method or make unnecessary another operation.
Seven patients with severe pancreatic inflamation secondary to acute pancreatitis are reported. Symptoms, laboratory data and radiologia findings suggested the diagnosis of phlegmon of the pancreas. The ultrasonography of the pancreas was useful for discriminating pancreatic pseudocyst from this entity. Clinical evolution of the pancreatic phlegmon could be long, up to 4 months, but it has tendency to expontaneous resolution. Relevant clinical findings of the disease are mentioned.
We studied the clinical, paraclinical and laboratory findings of a patient suffering from multiple sclerosis (MS) with chronic demyelinating polyneuropathy. As well as being a new case of demyelination of the central and peripheral nervous system not normally associated with the presence of antiganglionic antibodies, our patient presented clinical signs distinct from each disease with the characteristic abnormalities of cerebrospinal fluid (CSF) (cytological albumin dissociation at the moment when an outbreak appeared at the peripheral level and an increase in intrathecal secretion when the outbreak was at the central nervous system level). Demyelinating diseases of the central and peripheral nervous systems are a field of great interest in trying to find the antigenic bull's eye for MS.
