The lateral arm flap: an anatomic study.

The anatomy of the lateral arm flap is further elucidated, and the surgical techniques involved in raising the flap are described in detail. The data are derived from studies in 25 cadavers and five clinical cases. The relative merits of this versatile free flap are discussed. Three clinical cases are cited for illustration.

[The distally based sural fascio-cutaneous flap. A new technic for the coverage of loss of substance of the lower leg].

Prognosis of compound open fractures has been transformed by the use of plastic surgery to cover wounds. However, loss of substance of lower third of leg remains a delicate problem frequently requiring the use of microsurgical techniques by experienced teams. The sural cutaneo-aponeurotic distal base flap, the subject of a detailed anatomical study by the authors, can resolve the problem without affecting muscle capital and at the price of minor esthetic sequelae. Results in two clinical cases confirm reliability of this flap.

[Postoperative pyoderma gangrenosum. Apropos of 2 cases].

The typical lesion of pyoderma gangrenosum is a rapidly extending necrotic ulceration, often associated with a system affection, inflammatory colitis, hematologic affections, rheumatic disorders. In about 30% of cases it develops on the site of a sometimes minimal injury. In the two cases reported one developed in the region of a graft after excision of a basal cell epithelioma, the other in a peritoneal drainage scar after colon resection. In patients with pyoderma gangrenosum, initial discussion must involve possible causes of the cutaneous necrosis, but in the cases reported the clinical appearance and rapid progression of the lesion, and negative results of serial bacteriologic and mycologic examinations were in favor of this diagnosis. An accurate diagnosis is essential since general corticotherapy is usually necessary.

[Late revascularization for partial amputation of the thumb].

The authors report a case of partial amputation of the thumb, still attached to the medial branch of its nerve, and which was successfully revascularized after 18 hours of ischemia with the thumb still warm. They discuss the operative technique employed, and other similar cases from the literature.

[Embolization of the brachial artery by lead-shot with ischemic syndrome].

Pre-operative arteriography in a 5-year-old girl with a gun-shot wound of the shoulder demonstrated the presence of an arterial embolus at the elbow provoking an ischemic syndrome. Following re-establishment of circulation by embolus extraction the arterial perforation at the point of entry was repaired by a venous graft, loss of shoulder substance being treated by a musculocutaneous flap from the latissimus dorsi region. Pre-operative arteriography should be a routine investigation in patients with ischemic syndromes of the limbs following injury from penetrating projectiles.
