[Side effects of dermatologic local therapy].

After application of dermatics systemic as well as topical side effects are to be expected, the latter also by usage of remedies and cosmetics. Even though life-threatening or lethal complications caused by potent substances like mercury compounds, phenols, salicylic acid, boric acid and others seldom have been observed, it is necessary to prevent them completely. Most frequent are allergic contact eczemas, but occasionally also toxic irritations of the skin can be registered. The side effects discussed, especially those caused by steroids and antibiotics, demand strong criteria for indication, scientific mode of administration and intensification of the topical efforts for standardization of dermatological treatment.

[Psoriasis therapy with the aromatic retinoid Ro 10-9359 (Tigason)].

At three University Departments of Dermatology (Berlin, Leipzig, Erfurt) of the GRD, 63 psoriatics (46 men, 17 women) were treated with aromatic retinoid Ro 10-9359 (Tigason) up to a maximum of 84 weeks. In 34 of these cases, photochemotherapy was additionally applied. This combination resulted in a more rapid healing than Tigason alone. The average initial dose amounted to 1 mg per kg body weight. Only in cases of psoriatic erythroderma, the treatment started with a daily rate of 25 mg Tigason. The maintenance dose came to 50 to 25 mg daily. On account of deterioration of the cutaneous condition and/or heavy itching, the treatment had to be discontinued in 4 cases. Side effects as cheilitis, rhagades, loss of hair, and onycholysis disappeared after reduction of the dose. Our study confirmed the good therapeutical effect of Tigason with the various forms of psoriasis.

Mild head injury.

The pathology of mild head injury, its common behavioral sequelae, the nature of the recovery process, and the psychosocial consequences are reviewed. Acceleration-deceleration head trauma, causing brief unconsciousness, and leaving no frank neurological deficit, is associated in primates with axonal and terminal degenerative changes in brain stem nuclei. Although the presence of clinical demonstrable neurological abnormality increases the likelihood of subsequent deficits, their absence does not guarantee full recovery. Among mildly injured children, the relationships between persistent behavioral deficits and their age of onset correspond closely with those observed in children with overt damage, making it unlikely that psychological-behavioral deficits are attributable to non-neurological aspects of the injury. Deficits observed include distractability, irritability, headaches, inability to maintain a prior level of cognitive competence, and impulsivity. Although longer periods of unconsciousness are associated with permanent memory deficits, minimal unconsciousness is also followed by consistent deficits in complex memory tasks. Detailed neuropsychological testing of mildly injured children has revealed persisting deficits even after 5 years. Furthermore, deficits arising from repeated injuries are cumulative. The importance of adequate neuropsychological assessment and accurate information to the patient are stressed as important in helping satisfactory adjustment.

Synthesis and biochemical properties of chemically stable product analogues of the reaction catalyzed by S-adenosyl-L-methionine decarboxylase.

Structural analogues of decarboxylated S-adenosyl-L-methionine (dc-SAM), product of the reaction catalyzed by S-adenosyl-L-methionine decarboxylase (SAM-DC), with modifications in the side-chain portion of the molecule have been synthesized, and their ability to inhibit SAM-DC has been investigated. Mainly, compounds with a nitrogen atom in place of the sulfur were investigated. The data from these inhibition studies have resulted in a delineation of the structural features required for binding on SAM-DC. It was concluded that a terminal primary amino group, a terminal carboxyl group, and the sulfonium functionality are not required for binding on SAM-DC. It was also found that analogues of dc-SAM in which replacement of the sulfur by nitrogen was the only modification were still able to form an azomethine with the enzyme. As found for SAM and dc-SAM, these compounds also caused a time-dependent inactivation of SAM-DC.

[Rhabdomyosarcoma of the pulmonary artery (author's transl)].

A case of a 55-year-old man with the histological diagnosis rhabdomyosarcoma of the left pulmonary artery has been seen. Lung scanning and pulmonary arteriography are the clues for the diagnostical procedure. 55 cases from the literature are reviewed and clinical findings of the early and late stages of the diseases are discussed. Surgical treatment is the therapy of choice if ever possible; aggressive chemotherapy might be an acceptable alternative.
