Cytomegalovirus (CMV) infection is a major cause of morbidity and death in heart transplant recipients. Cardiac graft involvement in CMV infection is a matter of controversy, considering its frequency and its relationship with acute or chronic rejection. Four heart transplant patients were selected because of a severe CMV infection (systemic, gastrointestinal, ophthalmic, and neurologic involvement). Immunoglobulin M and increased immunoglobulin G CMV antibodies developed. Twenty-two routine endomyocardial biopsies (EMB; mean: 5.5 EMB per patient; range, 3 to 9) from these patients were selected covering the period of CMV infection. Grading of rejection showed 12 biopsies with "no evidence of rejection (grade 0)," nine biopsies with "mild acute rejection (grade 1B)," and one biopsy with "moderate acute rejection (grade 3A)." One EMB exhibited a single CMV inclusion in an endothelial cell detectable by light microscopy. The EMB were assessed for CMV infection using in situ hybridization (ISH) for the detection of CMV genome with a biotinylated CMV probe and immunohistochemistry (IHC) for the detection of CMV immediate-early antigen with the monoclonal antibody E13. ISH and IHC detected a single CMV-infected cell, respectively, in one and two EMB from two patients. The patient with a CMV inclusion determined by light microscopy was also positive with both techniques. Positive ISH and IHC were always in enlarged inclusion-bearing cells, which were easily observable with routine staining. One EMB had mild acute rejection, and the other one had no rejection.(ABSTRACT TRUNCATED AT 250 WORDS)

A 57 year-old-man with acute renal failure was diagnosed as having retroperitoneal fibrosis. Malignant lymphoma is suspected because of peripheral lymphadenopathy, but biopsies are unrevealant. Nephrostomy restores kidney function. Many other histologic samplings performed during three laparotomies are necessary to diagnose Hodgkin's disease. Nephrostomy is complicated with iterative acute pyelonephritis and septicaemias which make chemotherapy unadvisable. Ureterolysis is unsuccessful. Urinary tract infection disappears after uretero-ileoplasty. Subsequently chemotherapy induces a complete remission during 5 years with normal renal function. We review two previously reported cases of this unusual cause of malignant retroperitoneal fibrosis. Diagnostic procedures and therapeutic management are discussed.
The light chain deposition disease was recently identified as a systemic clinicopathological entity characterized by amorphous extracellular deposits which differ from the amyloid substance. Various organs may be involved, notably the kidney, the liver, the myocardium and the skin. The histopathological aspects were investigated in 3 cases. By immunofluorescence using frozen sections the deposits were shown to contain monoclonal light kappa or lambda chains. By electron microscopy they appeared to be granular and usually located close to epithelial and/or vascular basal lamina. There was in every case a monoclonal lymphoplasmacytoid proliferation. In one case amyloid deposits were associated in small vessels. In another one, follow-up study after chemotherapy showed improvement of hepatomegaly, stabilization of renal function, and regression of light chain deposits in skin biopsy specimens.
1. The PGE1, infused into the renal artery of the dog, increased RPF, Sodium and Potassium excretion rate and UV; it diminished the EPAH and FF but did not alter GFR and PRA. 2. The indomethacin, injected IV, in an other group of animals, decreased RPF, GFR, Sodium and Potassium excretion rate and PRA, and it increased the EPAH and FF. The administration of the PGE1, inversed the results obtained by indomethacin except those of PRA. 3. The infusion of the PGF1a, in other groups of dogs, did not alter either before or after the administration of indomethacin the parameters measured. 4. It was suggested that 1) neither the PGE1 nor the PGF1a stimulated renin synthesis, in the conditions that our experiments took place. The decrease of renin synthesis by indomethacin must be provoked by the inhibition of an other substance than the PGE and PGF and 2) the role of PGE in renal haemodynamic and exocrine renal function seems to be important.
