Jejunal bleeding from a solitary large submucosal artery. Report of two cases.

Two young patients with massive intestinal bleeding resulting from a solitary submucosal arterial abnormality of the jejunum are reported. In one case, the bleeding lesion was localized only by emergency abdominal angiography, while a previous exploratory laparotomy proved negative. The vascular lesion encountered in both patients consisted of a wide caliber artery lying in the jejunal submucosa; there was no evidence of true aneurysm on serial sections. The clinicopathological picture closely resembles that of lesions previously described in the stomach under the name of "gastric submucosal aneurysms" in the English literature, or of "gastric Dieulafoy's erosion" in the French literature. It is suggested that the vascular lesions herein reported should be called "solitary large submucosal artery" or "Dieulafoy-like erosion" of the jejunum.

[Primary hyperparathyroidism. Our experience with 30 cases].

The authors analysed thirty cases of hyperparathyroidism seen between 1969 and 1975 at Centre Hospitalier Universitaire at Poitiers. They noted the similarity of their observations with those which have generally been made in this condition. The only peculiarities of this study were a low incidence of calculi and a particularly high incidence of chance discovery by routine blood calcium measurement. The diagnostic value of blood calcium levels and blood citrate levels is confirmed in the same way as the advantage of a quantitative bone biopsy even although a few false positives limit the specificty of the test.

[Digestive lesions after abdominopelvic irradiation, in the treatment of cancers of the genital tract].

The authors report 15 cases collected over a few years on a gastroenterology unit, and emphasize the digestive complications occurring after pelvic or abdomino-pelvic irradiation for genital carcinoma. The rectum and pelvic colon are generally affected. The delays of onset after radiotherapy may reach or exceed several months. The vascular lesions, caused by ionising radiation, explain the tissue ischemia and, consequently the failure of local medical treatment. When severe hemorrhage or visceral perforation occur, surgery is necessary. However, sutures often break down on these insufficiently vascularised tissues and re-operation is always associated with a poor prognosis.

[Non-parasitic cysts of the spleen. 2 cases].

Although splenic cysts are benign, a gradual increase in volume of the liquid mass often leads to diagnosis owing to the large volume. The two cases reported are an example of this. A cure was obtained by splenectomy. It is usually easy to carry out and the prognosis is benign.

[Caustic burn and adenocarcinoma of the esophagus].

Adenocarcinoma of the esophagus is found almost exclusively at the level of the cardia. It is exceptional to find it in the middle part of the esophagus in the absence of involvement of the cardia. In the long term, caustic burns of the esophagus may favour malignant degeneration. The latter is always squamous cell carcinoma. In our patient, a 28 year old man, a cylindrical carcinoma of the thoracic esophagus was detected four years after the ingestion of concentrated sodium hypochlorite. The caustic burn followed by carcinoma were of special significance in this case owing to the development of an adenocarcinoma.

[Diabetes mellitus in pheochromocytoma].

The frequency of latent disorders of glucose regulation during pheochromocytoma, is evaluated at 75% of cases. Detailed analysis of 83 cases with a diabetic state, gave the following results: insulin dependent diabetes, 37 cases. Non-insulin dependent, 14 cases. Latent diabetes, 32 cases. The characteristics of the insulin-dependent diabetes were not always suggestive. Insulin dependency was, however, unusual above a certain age. We noted loss of weight in spite of good control of the diabetes, the absence of acidosis and ketosis contrasting with rapid loss of weight. In fact, it is above all the hypertension which should lead to diagnosis. Surgical operation, cures or improves considerably the diabetic state, thus proving the symptomatic nature of this diabetes.
