[Radiography used during operations for biopsies of non-palpable lesions of the breast which have been suspected because of mammography (author's transl)].

In order to prove that non-palpable lesions of the breast that have been removed after suspicion from mammography have been removed completely it is appropriate to X-ray during the operation of biopsy in order to make a diagnosis. Tests carried out with the X Faxitron X-ray apparatus on 369 patients show that even X-ray departments that carry out mammography and do not have specialised equipment can with quite modest means use this reliable technique for diagnosis.

Clinical results in the management of patients with uterine sarcoma.

From 24 Patients which were treated for uterine sarcomas during 1970-1977, 10 patients had a leiomyosarcoma, 6 an endometrial stromal sarcoma, and 8 a mixed mesodermal sarcoma. Surgical treatment was performed in 15 cases, and in 13 cases was followed by postoperative radiation. Eight patients were treated primarily by combined radium and cobalt 60 radiation therapy. Seventeen of 24 patients under control died, 15 of these within on year after onset of therapy. Regarding the histological patterns of sarcomas, both mitotic activity and vascular invasion was without significance in subsequent outcome.

[Complications in 1,092 radical abdominal hysterectomies with pelvic lymphadenectomies (author's transl)].

1,092 patients were operated for carcinoma of the uterine cervix according to a standardized radical abdominal hysterectomy with obligatory pelvic lymphadenectomy. The operative specimen showed metastases of the lymph nodes in 45,6% of the cases. In 15% of the cases embolization of tumor cells was found. In 8.7% of the specimes micro-metastases were seen and in 21.9% gross metastases were found. Postoperatively 1.5% of the cases developed bleeding. Ileus occured in 0.7% of the cases. A vesicovaginal fistula occurred in 0.3% of the patients. 1.4% of the patients developed a uretero-vaginal fistula. The operative mortality up to the 30th post-operative day was 1%.

[Pregnancy following renal transplants (author's transl)].

Women with uremia are usually infertile. Following successful renal transplants ovarian function resumes and conception becomes possible. Two cases of pregnancy following renal transplants are reported. The first patient had a Caesarean section at 33 weeks gestation for maternal indications, the second patient had a Caesarean section at 36 weeks gestation because of premature spontaneous rupture of the membranes. Both mothers and infants are in satisfactory condition. The problems of pregnancies following renal transplants are discussed and the literature since 1963 is reviewed. Pregnancy following renal transplantation is a high risk pregnancy for both the mother and the fetus. These high risks are acceptable if close supervision of the pregnancy by the nephrologist, gynaecologist and paediatrician are possible and if co-operation of the mother is satisfactory.

Tumor size and lymph node metastases in squamous cell carcinoma of the uterine cervix.

Tumor size was accurately measured in 684 patients who had surgical treatment of cervical carcinoma after a small biopsy and received no preoperative radiotherapy. Frequency of lymph node metastases increased as five different tumor size parameters went up, depth of tumor invasion being of particular importance. When micrometastases, macrometastases, and tumor cell emboli were considered, a correlation was statistically verifiable for micrometastases and macrometastases. Such correlations could not be determined for tumor cell emboli. They are to be regarded as a random event.

[The operated adenocarcinoma of the cervix uteri (author's transl)].

Out of a total of 1092 women operated because of cervical carcinoma, 4.8% had an adenocarcinoma of the cervix. Compared with patients with squamous cell carcinoma, there was no difference in signs and clinical pattern with those patients who had adenocarcinoma of the cervix. Treatment results, too, corresponded with those of patients with squamous cell carcinoma of the cervix.

[Late cervical pregnancy (author's transl)].

A cervical pregnancy which develops into the last trimester of gestation with a viable infant is a very dangerous and rare condition. A personal observation of such a case at 31 weeks gestation is added to the 8 cases known in the literature. Extreme dilatation of the cervical wall with extensive vascularization is typical for this condition associated with placenta increta or accreta. The uterine body tops the cervical pregnancy like a cap. Cesarean section followed by hysterectomy is the management of choice.

[Histological diagnosis and individual treatment of cancer in gynecology (author's transl)].

Individualization of cancer therapy in gynecology is based on a perfectly precise histological diagnosis. This implies close cooperation between the clinician and the histopathologist. Making a reliable and therapeutically decisive histopathological diagnosis is possible by means of certain diagnostic interventions which provide a maximum of clinical and histological information on the uterus and the breast, and by means of a systematic fine structure processing of the surgical specimens in carcinomas of the uterus, vulva and breast. Only in this way can the clinician take the extraordinary responsibility of an individual treatment of cancer.

[Hemoblastosis and genital manifestations (author's transl)].

The course of the disease in a woman with a malignant lymphoma is described. The primary disease was recognized through an early involvement of the uterus which led to hemorrhages. While observations of this kind are relatively seldom communicated, examination of the literature showed that involvement of the genitals is to be reckoned with relatively frequently with hemoblastoses in the advanced stages. Regular gynecological investigations are therefore essential for women with hemoblastoses.

[Balanced synthetic diet in gynaecological operations].

A balanced synthetic diet, the so called astronaut-diet was given to nine gynaecological patients with recto-vaginal fistulas or operation for aplasia of the vagina. By the reduction of stool quanitity and the change to an almost sterile stool, wound healing was very much enhanced. This diet is recommended for some operations involving the vagina and rectum.

[On the question of recurrent carcinoma in-situ of the uterine cervix].

Among 280 patients with carcinoma in-situ of the uterine cervix treated from 1961 to 1970, a review of the histology revealed 50 patients in which the carcinoma in-situ had been removed with certainty by conization. None of these patients was found to have had a recurrence. In the literature, 13 observations of a recurrence of carcinoma in-situ after complete removal by conization were located among 3, 129 cases. It is concluded that true recurrence after complete removal of carcinoma in-situ by conization is rare and that conization is adequate treatment for carcinoma in-situ for patients who attend regular follow-up examinations.
