Development of specific social skills training programmes for schizophrenia patients: results of a multicentre study.

OBJECTIVE: The efficacy of three newly developed cognitive social skills training programmes for residential, vocational and recreational functioning (experimental groups) were compared with a traditional social skills training programme (control group) referring to cognitive and social abilities, psychopathology and generalisation effects. METHOD: One hundred and five patients with a diagnosis of schizophrenia or schizoaffective disorder according to ICD-10 criteria were selected and assigned to the different treatment groups, using a matching procedure. The treatment phase lasted 6 months. A follow-up assessment was carried out after 1 year. RESULTS: Higher global therapy effects were obtained on almost all dependent variables in the experimental groups. Analyses of variance and covariance indicated higher symptom reduction for the experimental groups, but significantly greater improvements in some cognitive variables for the control group. Correlation analysis suggested associations between improvement of social behaviour with symptom reduction and improvements of cognitive skills. CONCLUSION: In view of these favourable effects, the developed cognitive social skills training programmes might facilitate the abilities of schizophrenia patients for their integration in the community.

[Spiritual healing at a public psychiatric institution].

INTRODUCTION: Complementary medicine has an increasing importance also in Switzerland, above all for somatic diseases. The spreading of these methods in psychiatry and psychotherapy is a far slower process. The psychiatric outpatient service of the Swiss region Baselland offered, besides conventional psychotherapy and several different complementary methods, the possibility of spiritual healing and evaluated it by scientific means. QUESTIONS: Is a cooperation between a spiritual healer and a public institution feasible? What are the results of the treatment? How ist the perception of the treatment by the patients? Is further research promising? METHOD: Patients with diverse and mostly long-term diseases willing to receive spiritual healing had, in addition to a conventional psychotherapy, been treated by a healer working in our facilities. Before and after the healing sequence as well as at the follow-up after 6 months the patients have been questioned by means of semistructured interviews to give their impression of the treatment and its results. The respective psychotherapists and the healer herself had to give their assessment of the treatment and its results as well. RESULTS: The analysis of the data of the first 20 patients resulted in 9 persons stating a better well-being, one a deterioration and 8 broke off the treatment prematurely in unchanged health condition. The majority of patients very much appreciated the treatment in terms of ease and relaxation, only one patient reacted with anxiety and interrupted the first treatment. At the follow-up after 6 months there has been no change to the initial results. Two persons could not be reached for the post-evaluation. DISCUSSION: Cooperation of a healer and a public psychiatric institution proved to be feasible and fruitful. For almost half of the patients, the treatment has been a remedy highly appreciated and only one patient has reacted negatively. Therefore it seems justifiable to further offer this type of treatment and to evaluate it. To back up this estimation further, also controlled research studies should be carried out with e.g. more homogeneous patient groups or with different healers.

[Filaria: a tropical disease as the etiology of acute scrotum].

In the diagnosis and treatment of acute scrotal pain and swelling it is important to differentiate between testicular torsion and epididymo-orchitis. A special case of a thirty-eight-year-old male asylant from Nepal was presented in the urological service with acute scrotal pain. To exclude testicular torsion, a diagnostic exploration of the scrotum was done with the findings of an acute funiculitis with abscess formation. Under surgical drainage of abscess together with medical treatment the acute inflammation disappeared. The diagnosis of filarial funiculitis was made after identifying the microfilariae in a blood vessel and in the peripheral blood smear. A filaricide medication with diethylcarbamacine citran was administered with good symptomatic relief.

[Accommodations for psychiatric patients. An empirical study of the Basel area canton].

The psychiatric services of the Swiss region of Basel-Landschaft observed an increasing deterioration and shortage of accommodation offers for mentally ill persons. At the same time, it grew temporarily more and more difficult to let single rooms in flat-sharing communities. Therefore a psychiatric services research project aimed at evaluating the wishes and needs of specialists in the housing sphere, and of about 600 in- and outpatients, as well as for their caregivers. The results showed, similar to other studies, on the one hand patients' predominant wish to live independently, whereas on the other hand there was a great difference between the views of patients and their caregivers. The latter view this aspiration for independence rather skeptically. The striking disagreement between patients and caregivers may be fruitful for the therapeutic process; however, this applies only if both partners are aware of this fact.

[Positive role of family in coping with schizophrenic illness].

Research on the influence of family variables on the course of schizophrenia has mainly centered on the concept of expressed emotions. The emphasis of this construct is centering predominantly on potentially destructive aspects of family behaviour. The authors criticize approach as too biased towards the negative. From their clinical practice and a review of the literature, the authors argue for a more positive view of relatives' role in rehabilitation, relapse prevention and coping with schizophrenia.

[Social background and the importance of organized, voluntary neighborhood support].

For some years we have been trying to promote projects providing for social support and a social network to improve the health and the social situation of persons who lack a supporting network of their own. The following article explains the social background and the social importance of the work done by an office for mediating voluntary neighbourhood support in a city district. The voluntary helpers and the persons who come for aid are compared with matched, representative random samples taken from the same district. The empirical study shows that persons seeking aid are socially underprivileged since they have only few resources, a low income, a social network that fails to support them, and a low sense of self-esteem. The mediating office for neighbourhood help has been successful in so far as its target group of persons in need of aid was actually reached. Neighbourhood help exercises a preventive action by the possible avoidance or alleviation of stress-conditioned diseases.

[Possibilities and limitations of public relations activities in the development of organized neighborhood assistance].

The article deals with the question how well known a new social service becomes which is completely based on voluntary and honorary work. The new service, organized neighbourhood help in Zürich-Altstetten, has been in existence since three years. To find out to what degree this new social service was known, a representative survey was carried out by telephone in which 208 persons living in the district referred to were interviewed one and a half years after the organized neighbourhood help had started working. --50% of the people living in the district Zürich-Altstetten were aware of the organized neighbourhood help, twice as many women than men knowing about it. The same relationship was observed between people who were fully occupied and people occupied in part-time jobs or not working: twice as many people of the latter group knew about this new social service. --About half of the people interviewed were interested in receiving more information about the service and 36% could even imagine participating in it. Although more information about the neighbourhood help was sent to these people, one year later still no one had applied. --The findings hint at a division of labour between the different channels of information: newspapers provide general information whereas pamphlets and recommendations from other social services are more likely to lead to registration.

[Burden and possibility of respite for family caregivers].

70 family caregivers and non related supporters at home of impaired elderly in urban conditions were interviewed regarding the burdens imposed on them and the kind of relief that would help them. Strain connected with the physical and mental deterioration of the supported and with their insecure future is more often mentioned than physical or financial burdens. The wishes most frequently expressed concern more and better available information, personal counseling, and more substitution in the evenings, night and weekends.

[Can layman volunteers for the elderly be mobilized?].

In order to improve the informal support system to the aged population it is necessary to improve the knowledge of the disabilities that burden everyday life and to gather information on specific needs. A pilot study in an urban area hints at the need for voluntary support by one third of the aged population. 88% declare their willingness to give voluntary support to others. The process of actual mobilization is then described.

[Principles of growth in the acetabulum. Consequences for the shelf operations (author's transl)].

Not unlike a Mercedesstar the arms of the Y-growthzone divide the cup of the acetabulum into three equal segments. The three branches of the Y-zone are spheroid. So the socket can expand evenly and conform to the head by feed back. Otherwise the socket would fill up. For the cartilage of the concave acetabulum proliferates by apposition to its uppermost layers, exactly as the hyaline cartilage does in the convex condyles. The trend of the socket during growth goes outwards, forwards and downwards. It moves away from a metal mark, set into the ileum flush with the shelf. It is precisely this increasing distance, that happens to a surgical shelf in a child. Failure will be inevitable. 614 shelf operations checked.
