Some legal and managerial strategies for managing healthcare fraud.

Healthcare fraud is an increasingly serious problem in the industry. The problem is complicated by disagreement over its basic causes and how it can be managed. Attempts to manage healthcare fraud introduce privacy issues as well as the proper role of the government. Even law enforcement officials appear to be undecided concerning jurisdiction and prosecution. Healthcare fraud is everyone's loss and will require the cooperation of all if it is to be controlled. The medical and legal as well as insurance organizations and the public must get involved.

Preparing for and handling the results of regulatory inspections of biomedical facilities.

Although there have been numerous presentations on how to conduct and survive a government inspection, it appears that these sessions do not always adequately include suggestions from those who have been the subjects of regulatory inspections. This paper will include comments from the experience of one of the regulated. Remarks apply to experience with U.S. regulatory authorities, principally the U.S. Food and Drug Administration (FDA) and the U.S. Environmental Protection Agency (EPA). This paper attempts to give guidance on how to prepare for an inspection and what to do at the debriefing and after the inspector(s) leave your facility.

Triphasic waves in coma from brainstem infarction.

There have been reports on patients in stupor or coma from a variety of disorders that affect the brain diffusely, notably hepatic failure, whose EEGs showed triphasic waves. We describe a patient in coma with neurologic signs localizing to the brainstem who had triphasic waves on EEG. This case further confirms the nonspecific nature of triphasic waves.

Acute sensory neuropathy-neuronopathy from pyridoxine overdose.

We report two patients who developed an acute, profound, and permanent sensory deficit after treatment with massive doses of parenteral pyridoxine. Aside from rapid onset, their clinical picture resembles that described in chronic pyridoxine neurotoxicity. It also is consonant with experimental models of acute pyridoxine intoxication and is probably secondary to a sensory ganglion neuronopathy. These patients also had transient autonomic dysfunction, mild weakness, nystagmus, lethargy, and respiratory depression. These previously undocumented features may be attributable to either the preservative used in the parenteral pyridoxine preparation or to the exceptionally high doses of pyridoxine these patients received.

Choroid plexus blood flow: evidence for dopaminergic influence.

Choroid plexus blood flow was measured in adult female sheep using the radioactive microsphere technique. The response of choroid plexus, renal and cortical blood flow to the infusion of dopamine (11 sheep), haloperidol (7 sheep) and propranolol (6 sheep) were compared. Choroid plexus and renal blood flow significantly increased after dopamine infusion (55% and 49% respectively). Choroid plexus and renal blood flow decreased significantly following haloperidol infusion (-24% and 29% respectively). Cortical blood flow did not significantly change. Propranolol infusion did not significantly change blood flow in these regions. These observations suggest that dopaminergic mechanisms play a role in the regulation of choroid plexus as well as renal blood flow.

Tandem duplication of the proviral DNA in an avian sarcoma virus-transformed quail clone.

Previous study has shown that an avian sarcoma virus-transformed quail clone, Q-B77-11, apparently contains full-size proviral DNA and releases virus particles which contain polymerase activity, but are unable to form foci. Recently, we have found that after extended tissue culture of Q-B77-11, the predominant cell type changed from one in which there was a single copy of the integrated viral DNA to one with two copies in tandem. There was approximately one copy of the large terminal redundancy between these two tandem proviruses. Polyadenylic acid-containing RNA species of twice the size of the viral genome were detected in these cells and are interpreted as transcripts of the tandem provirus.
