Two models of antishock trousers were studied for the effect of their inflation on the pulmonary function of 20 healthy men. Vital capacity was measured for each model at pressures of 0, 30, 60, and 100 mm Hg. The antishock suit with the abdominal compartment extending over the entire abdomen decreased the vital capacity 13.8% with inflation to a pressure of 100 mm Hg. The antishock suit with a smaller abdominal compartment, considered the standard model, decreased vital capacity only 5% at the same pressure.

A personal reprint file is an important educational tool for every physician. We have developed a microcomputer-based system for the filing of journal articles and reprints relevant to the practice of emergency medicine. The system uses a Northstar microcomputer, but could be adapted for use on other similar microcomputers. The filing system is easy to use and allows filing and retrieval of articles coded according to the subject outline of the Emergency Medicine Core Content.
Three hundred seventy-nine consecutive patients with adenocarcinoma of the corpus uteri, in whom irradiation was part of the treatment, were reviewed. The minimum follow-up was 3 years and none were lost. An increased incidence of corpus cancer in Central New York was demonstrated but could not be attributed to estrogen usage. There were 310 patients in FIGO stage I and 40 in stage II. Adenocarcinoma was reported in 319 and adenoacanthoma in 45. Overall survival was 74% and 62% at 5 and 10 years, respectively, and in stage I, 82% and 68%, respectively. Results for adenoacanthoma were similar to those for adenocarcinoma. Ten year survival was significantly better in those patients with a histologically negative hysterectomy specimen after preoperative intracavitary irradiation than in those with residual cancer. Treatment was tolerated well. This study demonstrates the results which can be obtained in a regional radiation therapy center serving patients and gynecologists from a wide geographic region operated upon in many different hospitals.
Previous reports showed that cyclophosphamide used late in the treatment of patients with pemphigus vulgaris reduced the complications due to steroids. In this study, cyclophosphamide was used in the initial treatment of five patients with pemphigus vulgaris in combination with prednisone to avoid side-effects of prednisone. Two patients had no prior treatment with steroids for pemphigus. Results indicate that initial combined program may be a highly effective treatment method for pemphigus vulgaris. Followup from one to four years after treatment shows all five patients in remission; four require no medication.
With the escalation of health care costs during the past decade, it has become increasingly important for the physician to be aware of the cost of various components of health care delivery. The following study was undertaken to ascertain the "cost awareness" of four different groups of health care providers. This was accomplished by having these groups estimate the cost of patient visits to an emergency department. Significant errors were observed in these cost estimations, and error trends were seen to occur that were independent of education and experience.
Clinical evaluation of ventilation performance during resuscitation is largely subjective. A mechanical device, the resuscitation bag controller (RC), which encircles the bag and allows controlled compression may improve the precision and accuracy of ventilation with manual resuscitation bags (MRB). We hypothesize that more precise, controlled pressure ventilation can be delivered with the RC, compared to the MRB. Prehospital (N = 13) and hospital personnel (N = 12) who routinely perform manual ventilation were randomized to either method of ventilation. Operators were instructed to ventilate an intubated adult mannequin. The percent of breaths delivered within a specified range, 800 to 1200 mL, was compared using simple regression analysis. The precision of tidal volume (TV) and peak airway pressure (PAP) was compared between methods and groups using the coefficient of variation. Comparison for significant differences between methods and groups in the number of breaths with a TV less than 800 mL and those with pressures greater than 30 cm H20 was performed using chi square or Fisher's exact test. There were no significant differences in the percent of acceptable breaths or mean TV delivered between methods or groups. The precision (i.e., reproducibility of the same value) of TV for both methods was best for hospital personnel. PAPS were less than or equal to 30 cm H2O for 93% of all breaths. Hospital personnel delivered a significantly greater percent of inadequate (less than 0.8 L) breaths, 19 versus 7.4%, and excessive pressure breaths, 9.2 versus 4.2%, when compared to prehospital personnel. We conclude that the resuscitation bag controller offers little advantage over standard bag resuscitation for adult resuscitation.(ABSTRACT TRUNCATED AT 250 WORDS)
In this third article in a continuing series on objectives for emergency medicine training, orthopedic objectives are presented. Orthopedic complaints are common in emergency medicine. Direction in mastering evaluative, cognitive, and procedural skills are provided utilizing behaviorally based objectives and references. These objectives provide a structure for systematically learning the content of orthopedics through enlightened patient care, questioning of attending physicians and supervisors, and directed reading.
In 1983, the Henry W. Grady Memorial Hospital in Atlanta began a family planning-based outreach program for eighth graders in a local school system. The program is led by older teenagers and focuses on helping students resist peer and social pressures to initiate sexual activity. Evaluation of the program, based on telephone interviews with 536 students from the hospital's low-income population, revealed that among students who had not had sexual intercourse, those who participated in the program were significantly more likely to continue to postpone sexual activity through the end of the ninth grade than were similar students who did not participate in the program. Because of their lower rate of sexual activity, program students also experienced comparatively fewer pregnancies than no-program students.
