Isolation of large bacterial plasmids and characterization of the P2 incompatibility group plasmids pMG1 and pMG5.

Large plasmids from Agrobacterium tumefaciens, Salmonella typhimurium, Escherichia coli, Pseudomonas putida, and Pseudomonas aeruginosa were routinely and consistently isolated using a procedure which does not require ultracentrifugation but includes steps designed to separate large-plasmid DNA from the bacterial folded chromosome. It also selectively removes fragments of broken chromosome. A variety of large plasmids was readily visualized with agarose gel electorphoresis, including five between 70 and 85 megadaltons (Mdal) in size, six between 90 and 143 Mdal, one that was larger than 200 Mdal, and one that was larger than 300 Mdal. This isolation procedure allowed initial estimation of the molecular sizes of the two IncP2 plasmids, pMG1 and pMG5, which were 312 and 280 Mdal, respectively. A standard curve for size determination by gel electrophoresis including plasmids between 23 and 143 Mdal in size did not extrapolate linearly for plasmids of the 300-Mdal size range. Unique response of different plasmids to the isolation procedure included sensitivity of IncP1 plasmids to high pH and the co-isolation of a 20-Mdal "cryptic" plasmid in conjunction.

Late voice function after surgical injury to the recurrent nerve.

In the period 1960 to 1970, a total of 213 patients underwent subtotal thyroidectomy for benign cervical toxic goitre. Postoperatively, immobile vocal cord indicating paralysis of the recurrent nerve was found in 17 patients. In 8 patients, immobility of the vocal cord was permanent. Seven of the 17 patients received voice training which was initiated within 3 weeks after operation. After a period of 5-10 years, on an average 8 years, the 17 patients had a clinical and a comprehensive objective examination of the voice function comprising stroboscopy, electroglottography, phono-oscillometry, voice range, phonation time, peak-flow and pitch. Only a few complaints were ciliated whereas the objective examination of the voice function revealed abnormal findings in all but one patient. The findings were less abnormal in patients who had received early voice training. It is concluded that despite abnormal objective findings, all 17 patients found their voice function satisfactory. Moreover, early voice training seemed to offer a fair chance of minimizing late voice problems, whether the paralysis was permanent or transitory.

Muscle tone under fentanyl-nitrous oxide anaesthesia measured with a transducer apparatus in cholecystecomy incisions.

Fentanyl is a strong, synthetic analgesic which may cause muscular rigidity when administered intravenously. To obtain a quantitative measure of the possible increase in muscle tone after intravenous fentanyl, the muscular tension of the right rectus abdominis was measured in 20 patients before and after administration of this drug. A traction transducer apparatus was fastened between the anterior and posterior rectus sheath in a right oblique laparotomy incision. Premedication was with pentobarbitone, and the anaesthesia and muscle relaxation were effected by thiopentone or enibomal and nitrous oxide-oxygen with 75% nitrous oxide, and suxamethonium infusion (0.2%) until the measurement of tension was started. Immediately after the action of suxamethonium had ceased, fentanyl, about 0.004 mg/kg body weight, was administered. An increase in tone was found in 15 cases (75%). The mean increase was 9.2 N. The influence of the anaesthesia upon the result is discussed, and it is concluded that fentanyl must be responsible for the increase in muscle tone.

Incidence of sterility in women operated on in childhood for perforated appendicitis.

Of 80 women who had appendicectomy in childhood 78 were reviewed with reference to sterility. Thirty-nine of them had been operated on for acute perforated appendicitis (APA) and 39 for acute non-perforated appendicitis (ANPA). There was no statistically significant difference in the incidence of sterility between the two groups. In the APA group the incidence of sterility differs from that in sterility models with the same age composition. It may therefore be advisable for young girls to have appendicectomy at at the least indication.

Long-term observation of thyroid function after surgical treatment of thyrotoxicosis.

During the 10-year period from 1960 to 1970 a total of 213 patients with diffuse or nodular toxic goitre underwent subtotal thyroidectomy. A short-term preoperative treatment with antithyroid drugs and iodine was given systematically. The observation period ranged from 5 to 15 years, average 9 years. A total of 203 patients (95%) were traced and followed up. The concentration of se-T4, se-T3, and se-TSH was measured and in selected cases a TRH test and BMR was carried out in addition. All thyroid specimens were re-examined by the same pathologist for histological grading. It was found that 85% of the patients had been rendered euthyroid for 5 years or more. The recorded incidence of thyroid hypofunction was 6% and the incidence of recurrent thyrotoxicosis was 9% of the series. Se-TSH was found to be elevated in 17%. The correlation between elevated se-TSH and se-T4 values was calculated and found to be fairly reasonable (r = -0.66). It is concluded that subtotal thyroidectomy preceded by short-term antithyroid drug therapy supplemented ultimately with iodine offers significant advantages in the management of thyrotoxicosis by rendering 85% of patients euthyroid on long-term surveillance.

Ruptured arteriosclerotic aneurysm of the splenic artery.

A case of arteriosclerotic aneurysm of the splenic artery with rupture into the transverse colon resulting in secondary perforation of the cecum is presented. Symptomatology, diagnosis and treatment of aneurysms of the splenic artery are briefly illustrated. Aneurysm of the splenic artery must be suspected by the presence of uncharacteristic upper abdominal pain and the annular calcification in the left upper quadrant on roentgenogram. The diagnosis is verified by arteriography. Operation of all symptomatic splenic artery aneurysms is recommended. Asymptomatic aneurysms measuring more than 3 cm in diameter and aneurysms with radiographic evidence of enlargement, should be operated too.

Radical emergency operation for prolapsed and strangulated haemorrhoids.

Conservative treatement of prolapsed and strangulated haemorrhoids is still widely used. According to several recent publications, however, emergency operation is a reasonable, safe, and effective treatment. Twenty-five patients with prolapsed and strangulated haemorrhoids underwent operation by the Milligan method. The results are compared with those in a similiar group having elective operation of chronic haemorrhoids. Since there were no differences in postoperative complications, length of stay in hospital, period off work, or late results, and since conservative treatment entails lengthy, painful treatment in bed and a long period off work, emergency operation is recommended for all strangulated haemorrhoids.

Anastomotic leakage following resention for cancer of the colon and rectum.

A total of 270 concecutive patients were examined for anastomotic leakage following resection for cancer of the colon and rectum. A 2-layer anastomosis was established 215 cases and a 3-layer one in 43. Twelve patients underwent the resection during manifest ileus. The total frequency of anastomotic leakage after elective resections was 14% significantly higher in the left than in the right half of the colon, irrespective of the anastomotic technique used. Age and sex per se did not influence the frequency of leakage. On the other hand, the presence of various complications at admission (cardiopulmonary, weight loss, metabolic disorders and anaemia, hypoproteinaemia, or electrolyte disturbances) was attended by an increased frequency of anastomotic leaks after left-sided resections. Pallative resection as well as resection of left-side tumour during manifest ileus was accompanied by an increased frequency of anastomotic leakage. There was also an increased tendency for leakage to occur when left-sided tumours were treated by a temporary defunctioning caecostomy rather than by transverse colostomy.

Fine needle and open biopsy in thyroid disorders.

The diagnostic value of cytological, serological and scintigraphical examination was evaluated in 20 histologically verified cases of thyroid disorder. The predictive values of specificity as to malignancy, thyroiditis and thyroid autoantibodies were respectively 0 percent (0-71), 80 percent (28-99) and 75 percent (19-99), and the predictive values of sensitivity were 88 percent (64-99), 100 percent (78-100) and 94 percent (70-100). Cold nodules were malignant in 40 percent. Fine needle biopsy can be used as a supplementary test without any risks and is a valuable aid in the diagnosis of Hashimoto's thyroiditis, but not in the diagnosis of malignant lesions. If any clinical or scintigraphical (cold nodule) suspicion of malignancy exists, a surgical biopsy must be done.
