[The liver. Anatomo-radiologic review].

A precise knowledge of the sectorial anatomy of the liver and its variations is essential in order to be able to localize lesions in the parenchyma and to guide segmental resection of the liver. The hepatic systematization proposed by Couinaud and the nomenclature of the segments, which are numbered from I to VIII, are of obvious practical value and are used routinely by surgeons and radiologists. However, they do not take into account the exact position of the liver in the abdominal cavity. For this reason, we prefer to use a topographical nomenclature, based on J. Hureau's classification, consisting of a right posterior column, corresponding to Couinaud's right lateral sector (segments VI and VII), a right lateral column, corresponding to the right paramedian sector (segments V and VIII), a right paramedian column, corresponding to segment IV, a left lobe column, corresponding to segments III and II, and a dorsal sector corresponding to the caudate lobe or segment I. This nomenclature, which takes into account the position of the liver in the abdominal cavity, constitutes a compromise between the Anglo-Saxon nomenclature and Couinaud's segmental classification. It clearly emphasizes the posterior nature of the right lateral sector, the lateral position of the right para-median sector and the anterior position of the two sectors of the left lobe.

[Participation of the external carotid artery in orbital vascularization].

The authors present the result of 70 dissections of the arterial vascular system of the orbit. The craniofacial junction is an important location because of possible anastomosis between the two carotid systems. These anastomosis can be situated at the meningeal level, palpebral and also inside the orbit cavity. The embryology of the encephalic vascular supply allows one to explain the possible dual origin of the orbital blood supply. This work shows the frequent participation of the external carotid artery to the orbital vascularisation by mean of three collateral branches: the infraorbital artery, a branch of the maxillary artery, partakes in the arterial supply of the inferior oblique muscle in 85,7% of cases. Usually, the inferior muscular artery which is a branch of the ophthalmic artery also partakes in this supply. In very exceptional cases (2 of 70) the infraorbital artery alone supplies the muscle. the lacrimal artery which can come from the external carotid system either from the middle meningeal artery or from the anterior deep temporal artery. This meningolacrimal artery can be either a solo lacrimal artery (17,14% of cases) or be associated with another classical lacrimal artery, from the ophthalmic artery (11,43%). These lacrimal arteries coming from the external carotid system have the following characteristics: they are thinner than classical arteries, they penetrate the orbit by their own orifice, the Hyrtl canal, and they can participate in the muscular vascularisation, especially of the lateral rectus. When these arteries are unique, most often they are encountered when the ophthalmic artery has undercrossed the optic nerve.(ABSTRACT TRUNCATED AT 250 WORDS)

Anatomical bases of echography of the liver. Applications in peroperative echography.

Peroperative echography is bound to play an increasingly important role in indications for hepatic exeresis, particularly as this type of surgery is anatomical and patterned. It is not easy however to have a clear idea of the stereo-spatial representation of one segment or another when the liver is in situ. The aim of this investigation is to define precisely, with the help of echographic sections, the topography of the different hepatic segments, thereby enabling the surgeon to carry out a complete and systematised exploration. It provides the surgeon with information which has not been accessible until now with classic peroperative explorations.

The arteries of the lacrimal gland.

The vascularization of 70 lacrimal glands was studied by orbital dissection subsequent to injection of the arterial bed with red-dyed latex. The origin, diameter and collateral branches of the lacrimal artery and its anatomical relations were investigated. Three types of lacrimal vascularization were seen. In the type I variety, the lacrimal artery originates from the ophthalmic artery and runs along the margin of the rectus lateralis muscle. In this case, the lacrimal artery is a major source of vascular supply to the muscle. In the type II variety, the lacrimal artery originates from the middle meningeal artery. In this case, the lacrimal artery is only a very modest source of vascular supply to the muscle. The type III variety features two lacrimal arteries vascularizing the lacrimal gland. One of the arteries originates from the ophthalmic, while the other arises from the middle meningeal. In this case, the lacrimal gland is the site of an intraorbital anastomosis between the internal and external carotid systems. The lacrimal gland is innervated by the lacrimal nerve and the lacrimal rami of the maxillary nerve. Preliminary results regarding certain morphological features of the lacrimal nerve are reported in this paper.

Anatomical basis for the use of the pectoralis major myocutaneous flap in reconstructive surgery.

The aim of this study was to clearly identify the vascularization of the pectoralis major myocutaneous flap owing to the importance of its use in the surgery of cervicofacial cancer. This work was based on the study of 40 pectoralis major muscles. The anatomical specimens were dissected after non selective injection of red latex followed by selective injection via the arterial pedicle of the muscle and finally injection of the cutaneous territory supplied by this pedicle. This study confirms the existence of a main arteriovenous pedicle vascularizing the donor site. The pedicle, lying at the deep surface of the pectoralis major, originates from the thoracoacromial artery. Rotation of the myocutaneous flap is achieved around this pedicle, the latter ensuring the survival of the flap. Participation of other arteries in the vascularization of the pectoralis major myocutaneous flap was also noted in this study.

Computerized reconstruction of an anatomical structure based on digitized sections. Preliminary study.

The authors propose a technique of spatial reconstruction of serial sections using the modern methods of computer graphics. The technical problems encountered and the duration of the classical Born method render the proposed technique difficult to apply as a routine procedure. The results of this work show that computer assisted image reconstruction could be used in daily routine work only when appropriate software programs and sufficient memory capacity become available to allow rapid image restitution. With the technique described in this paper, the reconstruction and display of a single image require 7 to 8 h of computer time.

The mechanical role of the digital fibrous sheath: application to reconstructive surgery of the flexor tendons.

The movements of flexion of the digital articular chain are dependent upon a vast functional complex comprising the long flexor tendons, their antagonist long extensor tendons, and the coupled action of the interosseous and lumbricales muscles. Based on theoretical biomechanical data backed up by experimental investigation, the authors emphasize the important role of the digital fibrous sheath in the mechanism of action of the long flexor tendon. Any proximal resection (even of minor degree) of the proximal pulley of the digit induces severe mechanical disturbances by enhancing flexion of the metacarpophalangeal joint to the detriment of the proximal interphalangeal joint. Conversely, distal resection of the pulley enhances flexion of the interphalangeal joint to the detriment of the metacarpophalangeal articulation, although the resulting abnormalities are less pronounced than those due to proximal resection. The fibrous pulley plays a role in tendinous orientation in the frontal plane and in digital orientation of the index finger where the angulation between the pulley and flexor tendon is most pronounced. In this latter case ulnar deviation of the second finger occurs on flexion. Reconstructive surgery of the long flexor tendons requires that the digital fibrous sheath, especially its proximal segment, be preserved. The metacarpophalangeal joint seems to be the most vulnerable to morphological modifications of the pulley, as stressed from the anatomical standpoint by the absence of insertion of the long flexor tendons on the proximal phalanx.

[Anatomic pitfalls of recurrent laryngeal nerve dissection].

Based on findings during 600 thyroid operations and pathological examination of 100 specimens, the principal difficulties encountered during dissection of the recurrent laryngeal nerve related to its anomalies and normal variations are discussed. Studied successively are variations in relations of the nerve with the inferior thyroid artery and its branches, the extralaryngeal bifurcations of the nerve, and the non-recurrence of the inferior laryngeal nerve.

[Variations in the parathyroid glands. Number, situation and arterial vascularization. Anatomical study and surgical application].

The authors have made a study of the variations in the parathyroid glands, basing their report on 100 block dissections of the neck injected with latex. The results allow a better understanding of certain types of parathyroid insufficiency following surgery to the thyroid gland. In almost half the cases the vascular arrangement was sufficient to explain how hypoparathyroidism might come about following surgery to the thyroid gland.

[Modern peroperative radiology in biliary surgery. Results obtained in a continuous series of 341 cases (author's transl)].

The authors study the results obtained in biliary surgery using modern radiological and electronic equipment, combining rapid-sequence image intensified fluorography modular cabinet, television screens and videorecorder. The performance of this apparatus has enabled them to obtain precise measurements of pressure as required, a continuous film reproducing the televised image, films taken in series at the time of and during the desired programming with a very short time required for setting up and with multiple views. Under such conditions, it is possible to avoid troublesome superimpositions, to reduce to the order of 0% the figure for missed calculi and to adequately explore the choledoco-duodenal junction with the possibility of precisely determining the indications for sphincterotomy (Oddi). Finally, the apparatus offers the possibility of combining an abdominal vascular assessment with the biliary investigation, in those cases where such a study may appear useful. Finally, the price of the apparatus is acceptable since it may be used by several departments at the same time within the context of a common operating suite.
