Crystallization of purified recombinant human interleukin-1 beta.

The gene for human interleukin-1 beta was cloned from SK-hep-1 hepatoma cellular RNA and expressed at high levels in Escherichia coli both as the naturally processed form (rIL-1 beta) and as a variant with an additional sequence of three amino acids on the N-terminus (rIL-1 beta +). Expressed protein was purified to homogeneity by a sequence of steps, which included low pH incubation, adsorption and desorption from Procion Red Sepharose, sizing on a Superose 12 fast-performance liquid chromatography (FPLC) column, and anion exchange chromatography on QAE Sepharose. The final step provided a biologically active protein that migrates on two-dimensional (2-D) gels as a single spot with a pI of 6.7 +/- 0.2 and a molecular mass of 17,500 daltons. Concentrated solutions of rIL-1 beta have produced crystals by ammonium sulfate precipitation. The crystals are tetragonal, show the symmetry of space group P4(1) or its enantiomer, have lattice constants of a = 58.46 (1) and c = 77.02 (3) A, and scatter to at least 2 A resolution. A structure determination based on these crystals is under way.

Optimization of ligation reaction conditions in gene synthesis.

Several phosphorylation, annealing and ligation reaction conditions have been investigated for the enzymatic assembly of genes of interest. The use of longer oligodeoxyribonucleotides (40-60 bases long) also improved the enzymatic reaction. Furthermore, the use of longer oligonucleotides and the elimination of long stretches of G's or C's lowered the mutation rate.

Buccal cellulitis in an infant due to ampicillin-resistant Hemophilus influenzae.

Hemophilus influenzae type B is known for its predilection to produce infections in the cheeks of infants. Early recognition and aggressive intervention are essential to avoid potentially life-threatening sequelae. A case is presented in which a resistant strain obscured the diagnosis and prolonged the illness. Clinical manifestations and differential diagnosis of the disease are discussed.

A method for increasing the resolution of scanned projection radiography and other digital X-ray systems.

A method for increasing the sampling frequency of digital X-ray systems is described. The method employs discrete, stepwise magnetic deflection of the focal spot and the recombination of the resulting displaced images. The technique was applied to a GE CT/T 8800 scanner operating in "Scout View" mode. The hardware and software modifications were minor. The transverse resolution was shown to improve from 0.63 to 0.9 line pairs per mm, using a test phantom. The effect of the improvement in resolution is also illustrated in the human. The possibility of further improving the resolution of the system is discussed.

Transconjunctival resection of an orbital dermoid tumour.

An unusual case of an orbital dermoid tumour containing a well differentiated tooth-like structure is documented. A previously unreported surgical approach to a maxillofacial dermoid tumour is introduced. Using a conjunctival incision, orbital floor exploration was undertaken with excellent visibility and access. A large dermoid tumour was removed without the need for an external skin incision. The surgical approach and pathology of this unusual lesion are reviewed.

Surgical approach to the high subcranial internal carotid artery.

Two cases are presented to describe surgical approaches to the high cervical internal carotid artery permitting surgical treatment of conditions such as aneurysms of the high carotid artery, fibromuscular dysplasia, and arteriosclerosis by endarterectomy or resection of a segment with the interposition of a graft.

Problems in ANA test interpretation: a comparison of two substrates.

Comparison of serum antinuclear antibody (ANA) test results on commercial HEp-2 cell culture preparations and fixed mouse kidney sections demonstrated significant differences in end-point titers and pattern production. When manufacturer's suggested screening titers are used, there is also a significant difference in qualitative results and correlation with clinical status. With individual intralaboratory establishment of screening titer levels, some of these differences become less significant, although this study suggests that mouse kidney substrate slides are more sensitive in detecting nonspecific ANA, and that HEp-2 substrate slides are more specific in detecting ANA from cases of systemic lupus erythematosus. Antinuclear antibody substrate selection must be based on classic sensitivity-specificity considerations and the clinical correlation performance desired. Comparisons of interlaboratory or follow-up ANA results are invalid without consideration of substrate variations. Regardless of substrate utilized, each laboratory should establish its own individual screening titers in relation to suitable age groupings.

Morphologic effect of hyperosmolarity on rabbit corneal epithelium.

The morphologic effect of hyperosmolarity, equivalent to that seen in the tear film of patients with keratoconjunctivitis sicca (KCS), on rabbit corneal epithelium in vitro and in vivo was studied. In the in vitro studies, corneal epithelium was grown in explant cultures. Control tissue was cultured in a 307 mOsm/L medium. Epithelium cultured in the 333, 361 and 363 mOsm/L media showed decreased intercellular connections, blunting and loss of microplicae, disruptions in cell membranes and cellular swelling with decreased cytoplasmic density. In in vivo studies, corneas bathed in balanced salt solutions (BSS) concentrated to 330, 360, or 407 mOsm/L showed increased cell desquamation, and the cell changes observed at similar osmolarities in the in vitro studies. The tear film osmolarities observed in KCS are sufficient to cause the corneal epithelial changes seen in patients with this disease.
