Otolaryngic allergy. The last half-century.

The American Academy of Otolaryngic Allergy (AAOA) celebrated 1991 as its 50th anniversary. This article reviews the last 50 years in otolaryngology and examines the lives of such prominent individuals as F. Hansel, H. Rinkel, G. Shambaugh, R. Williams, and others. The development of the Academy from a small group of individuals to its present status is presented.

Perilymph fistula.

Perilymph fistula occurs when endolymph and perilymph mix, or when perilymph leaks into the middle ear space. Sensorineural hearing loss and/or vertigo may result. This paper reviews the pertinent anatomy and physiology of the inner ear, clinical presentations, diagnosis, treatment, and prognosis as a guide for the clinician.

Malignant external otitis.

Malignant external otitis (MEO) is a potentially life-threatening Pseudomonas infection of the external auditory canal in which rapid diagnosis is the key to proper treatment. We present an anatomic description of the ear and skull base, with a review of the literature on MEO, in the hope that it will enable primary care physicians to work more closely with otolaryngologists in dealing with this illness.

Effect of structure on tumor specificity of alicyclic alpha-amino acids.

The selective affinity of [carboxyl-11C]-1-aminocyclopentanecarboxylic acid (ACPC) for tumor tissue has led us to study the tumor-localizing characteristics of a series of alicyclic alpha-amino acid analogs of ACPC. The tissue distributions of [14C]-1-aminocyclopropanecarboxylic acid, 1-aminocyclobutanecarboxylic acid (ACBC), 1-aminocyclohexanecarboxylic acid, 1-amino-2-methylcyclopentanecarboxylic acid were compared with that of ACPC in Buffalo rats bearing Morris 5123C hepatomas. ACPC and ACBC were found to have significantly higher tumor-to-nontumor concentration ratios than the other four amino acids. ACBC generally had higher tumor-to-nontumor ratios than did ACPC, significantly so for muscle, kidney, and testis and marginally so for blood. These results suggest that [carboxyl-11C]ACBC may be a better agent than [carboxyl-11C]ACPC for tumor imaging by positron tomography.

Epiglottoplasty for the treatment of laryngomalacia.

Laryngomalacia is the most common congenital laryngeal anomaly. Patients present with different degrees of stridor and feeding problems that usually resolve by 18 months of age. A small number present with severe feeding problems, failure to thrive, stridor with cyanosis, and apnea, which may result in cardiopulmonary disease. These infants require surgical intervention, usually a hyomandibulopexy or tracheotomy. We present a new procedure, epiglottoplasty, that is performed endoscopically and involves excision of redundant mucosa over the lateral edges of the epiglottis, aryepiglottic folds, arytenoids, and corniculate cartilages. Ten patients have undergone this procedure with good results. Epiglottoplasty represents an alternative to tracheotomy in severe laryngomalacia. Indications, techniques, postoperative management, and complications are presented.
