[Malignant centroblastic and immunoblastic lymphoma. Results of a direct immunofluorescence study of 43 samples after inclusion in paraffin].

The immunosecreting character of tumour cells was shown using a direct immunofluorescent technique on histological sections of fixed tissue, included in paraffin, of 43 specimens from 39 patients suffering from highly malignant ML. These ML corresponded, in the Lennert-Kiel classification, to immunoblastic (9 cases, 4 in plasmoblastic transformation) or polymorphic centroblastic ML (34 cases). In seven patients, the highly malignant ML was secondary to a low grade malignancy ML (3 CLL; 1 lymphoplasmocytic ML; 1 centroblastic centrocytic follicular ML) or to previous dysimmune state (1 Gougerot-Sjögren syndrome 1 lymphomatoid granulomatosis of Liebow). Of these 43 specimens from diverse tissues, but predominantly lymphatic lymph nodes (22 cases), 14 did not contain any monoclonal intracytoplasmic immunoglobulin (12 polymorphic centroblastic ML; 2 immunoblastic ML). On the other hand, 8 immunoblastic ML and 21 polymorphic centroblastic ML did secrete a monoclonal immunoglobulin. Among the 29 cases, 23 contained a heavy chain associated with only one light chain. In one case of Richter's syndrome, a double population, one mu, the other gamma, associated to only the light chain Kappa, was demonstrated. The heavy chain mu was secreted in 25 cases out of 29, associated 19 times to a Kappa chain and 4 times to a Lambda chain. The authors stress the morphological aspects of the positive fluorescent reactions, which plead in favour of a real secretion by the cells: perinuclear border with small grains projecting into the nucleus; intranuclear vacuoles; positive cytoplasmic reactions with more or less confluent granules, predominantly perinuclear.(ABSTRACT TRUNCATED AT 250 WORDS)

[Anatomopathological aspects of non-Hodgkin's malignant rectocolic lymphoma].

The malignant lymphomas (ML) of the colon are the rarest of the non-hodgkinsian ML of the digestive tract. The caecum and rectum are the most frequent locations for these colonic ML. They can occur as primary tumours or as secondary manifestations resulting from the evolution of other primary ML in other locations. The macroscopic appearance varies according to the size and number of lesions. The histological classifications in current use are based on both morphological and functional data. The most interesting of these is the one proposed by Lennert which distinguishes between the ML of low malignancy (tumours where small cells predominate) and those of high malignant (tumours made up of large cells). The prognostic depends on both the stage of growth and the type of treatment applied.

[Gougerot-Sjögren syndrome, periarteritis nodosa, non-Hodgkin's lymphoplasmocytic lymphoma and acquired C4 deficiency].

A Sjögren syndrome was confirmed histologically in a 19 year old woman. Four years later, periarteritis nodosa (PAN) with characteristic vascular lesions on muscle biopsy occurred simultaneously with lymphatic hyperplasia comprising splenomegaly and polyadenopathy. The PAN was cured with corticosteroids and cyclophosphamide and the lymphadenopathy regressed. Several months after treatment was stopped the lymphadenopathy recurred which histologically resembled a malignant non-hodgkin lymphoplasmocytoma secreting an IgM kappa monoclonal immunonoglobulin. During the PAN and the establishment of the lymphoproliferative syndrome a severe C4 deficit was detected which disappeared after chemotherapy.

Peliosis of the spleen. Report of a case associated with chronic myelomonocytic leukemia, presenting with spontaneous splenic rupture.

A 62-year-old man had chronic myelomonocytic leukemia (untreated) for a period of 8 years. He developed a sudden, spontaneous rupture of the spleen and a splenectomy was performed. Grossly and microscopically the spleen showed peliosis. The blood-filled cavities were similar to the pseudosinuses described in the spleen of hairy-cell leukemia. The lesion common to these disorders apparently is a progressive disappearance of lining cells and of ring fibers.

[Morphologic methods in the study of lymph nodes].

A good histopathological study of lymph node biopsies is only possible if the structure and the cell morphology are well preserved. It is necessary, therefore, to ensure that the material is correctly handled from the onset of the investigation. The method of bisection, the fixation procedure, the paraffin and plastic processing, the staining techniques are exposed with practical details. The investigation of lymphoid tissue requires a multiparameter approach. Additional data can be obtained from other techniques, for example cytology, histoenzymology, immunohistochemistry, electron microscopy. These techniques are described so that they can be undertaken in routine pathology.

[Cutaneous necrosis of the breast: a complication of oral anticoagulant treatment].

Cutaneous necrosis of the breast is a rare complication of oral antivitamin K drugs. It occurs shortly after instituting therapy with well-limited, irreversible skin necrosis developing rapidly. The scar is superficial and only involves the subcutaneous tissue; early and extensive surgery does not therefore seem to be justified. Normal scarring is the eventual spontaneous outcome but this is always a long process. Several arguments are advanced in favour of a toxic effect of the drug itself on the vascular walls of the vessels in the dermis.

Angiofollicular and plasmacytic polyadenopathy: a pseudotumourous syndrome with dysimmunity.

Two cases are reported of an apparently distinct type of immune disorder. Beginning with mild anaemia and widespread massive lymphadenopathy, the disease progressed to a fatal autoimmune type haemolytic anaemia. Serum investigation showed polyclonal hypergamma-globulinaemia and some autoantibodies. Repeat lymph node biopsies in each case showed hyperplasia within B lymphocyte territory (follicular hyperplasia and polyclonal plasmacytosis with IgG predominance) and atrophy of T dependent areas. Dilatation of lymph sinuses, vascular proliferation, and sclerosis were striking features. This appears to be a new entity, and reasons are given for separating this disease from other pseudotumourous lymph node disorders associated with dysimmunity.

[Visceral Kaposi sarcoma with angioimmunoblastic lymphadenopathy and latent sicca syndrome (author's transl)].

A case clinically and morphologically typical angio-immunoblastic lymphadenopathy in a 64-year old man is reported. A sicca syndrome was discovered with histological localization in the labial accessory salivary glands. Death occurred after a course of a few months. The autopsy showed an association of angio-immunoblastic lymphadenopathy lesions of the bone marrow and the lymph node and of typical multivisceral localizations of Kaposi's sarcoma (lymph node, liver, thyroïdd gland) without skin involvement. This association of two diseases characterized by severe dysimmunity with B lymphocytes hyperplasia is discussed, and compared to the others diseases which could be associated with Kaposi's sarcoma.

[Apropos of a case of myospherulosis].

The case was one of a 25-year-old woman with a polycystic subcutaneous nodule in the posterior iliac region, developing three weeks after a series of injections. Histological examination revealed the presence of cysts with a saccular formation filled with endospherules characteristic of the disorder. Myospherulosis is reviewed in the light of this first european case.

Primary pulmonary malignant lymphoma, clinical and pathological findings, immunocytochemical and ultrastructural studies in 15 cases.

Fifteen cases of primary pulmonary malignant lymphoma (M.L.) were studied using strict criteria to distinguish them from secondary pulmonary manifestations. All were M.L.'s of the lymphoid B type. Diagnosis on the basis of non specific clinical, X-ray and endoscopic findings if often difficult but can be facilitated by the systematic use of immunocytochemical and ultrastructural techniques. Their evolution is in good agreement with the prognostic data given by the Kiel-Lennert histopathological classification. 14 of the patients had M.L. of low grade malignancy; the one case of high grade malignancy was quickly fatal. A few cases appeared as a complication of a preexisting dysimmune disease (Pigeon breeder's disease, Gougerot-Sjogren disease, Liebow's lymphomatoid granulomatosis).

Primary intestinal lymphoma of Ki-1 large cell anaplastic type with mesenteric lymph node and spleen involvement in a renal transplant recipient.

A laparotomy performed for an occlusive syndrome, disclosed the presence of an ileal lymphoma in a renal graft recipient, 17 years after a first renal transplant. Mesenteric lymph node and spleen involvement were associated. The morphology of the large cells, the type of infiltration of the lymph node simulating metastatic carcinoma or malignant histiocytosis were highly suggestive of a new type of high grade malignancy lymphoma recently described which is called 'large cell anaplastic ML'. The immunolabelling study confirmed this diagnosis, demonstrating the following phenotype CD4, CD25 (TAC), CD30 (Ki-1), OKT 9 (transferrin receptor), EMA and HLA DR positivity. The tumour cells were negative for the other T, B and histiomonocytic markers. This case, of a large cell anaplastic Ki-1 positive lymphoma, expressing CD4, in a renal graft recipient shows that high grade ML in such patients are not all of the B cell type.

Histopathological study of bone marrow biopsies in 30 cases of T-cell lymphoma with clinical, biological and survival correlations.

Forty-five bone marrow (BM) biopsies have been studied in 30 T-cell malignant lymphoma (ML). According to the updated Kiel classification, these ML comprised 12 low grade ML and 18 high grade ML. BM involvement was not significantly more frequent in low grade ML (41.6 per cent) than in high grade (33.3 per cent). This involvement was discovered in 85 per cent of the cases at the time of diagnosis. A correlation was found between BM and other visceral localizations for histological type in all cases. Infiltrates principally showed a nodular pattern in low grade and a diffuse pattern in high grade ML. Hematopoietic hyperplastic reaction was frequent (66.6 per cent) not correlated to involvement. Clinical staging showed extensive spreading. Our patients had an overall median survival of 40 months, worse in high grade ML (median: 19 months) than in low grade ML (41 months) but not statistically different (p = 0.25). Extranodal localizations are a significant criteria for poor prognosis (p = 0.018). Among them, BM involvement appears to be the most significant criteria (Cox model, p = 0.006). Patients with BM localization had a median survival of 9 months contrasting with 40 months in patients without BM localization (p = 0.007). Thus, BM biopsy is useful for the diagnosis of patients with T-ML and essential to establish the prognosis.
