Trends in the use of antituberculosis drugs in Spain 1993-1998.

PURPOSE: To determine and analyse the trends in the use of antituberculosis agents in Spain during 1993-1998. METHODS: Pharmacoepidemiological study on the use of antituberculosis drugs using the data from prescriptions redeemed by the National Health Service. RESULTS: The use of antituberculosis drugs over the study period was 6,271,909 defined daily doses (DDDs) in 1993, which fell to 4,433,066 DDDs in 1998. A decreasing trend was thus seen, involving a 29.6% drop between the beginning and the end of the study period. The use of daily doses per 1000 inhabitants was 0.42 in 1993 and 0.30 in the end of the period in 1998. The drug most used during the period of study was Rifinah. In 1993, its percentage of use was 41.2, which then decreased to 34.5 in 1998. After this drug, Rifampicin and Etambutol were those with the highest percentages of use, with values close to 18 and 15, respectively. Regarding estimation of tuberculosis, the prevalence was 25.25 cases per 100,000 inhabitants in 1993 and 16.62 in 1998. A decrease of 23.1% occurred in the cost in pesetas between 1993 and 1998. CONCLUSIONS: Drug's combinations are the most common therapy for tuberculosis and there were important differences between regions. The highest estimation of this disease is in the northwest region of Spain.

The role of ERCP in the diagnosis and treatment of idiopathic recurrent pancreatitis in children and adolescents.

ERCP was performed in 51 patients, aged 1 to 18 years, as part of diagnostic evaluation for recurrent pancreatitis without obvious cause or as therapeutic intervention. ERCP was successful in 50 patients (98%). Thirty-four patients (68%) had anatomic findings indicating a possible cause of recurrent pancreatitis. Three patients (6%) had findings suggestive of sphincter of Oddi dysfunction. Eighteen of the 37 patients (49%) with ductal abnormalities underwent endoscopic therapy, with a favorable outcome in 15 (83%). Eleven patients were treated surgically, and 8 of these patients (73%) improved symptomatically. Eight patients received no treatment, and 6 of them (67%) had recurrent bouts of clinical pancreatitis. One mild case of pancreatitis (1.9%) occurred after ERCP, and 3 mild cases followed endoscopic therapy. In conclusion, ERCP is a relatively safe technique that produces opacification of the desired ductal system with a high degree of accuracy and provides useful information in the evaluation of children with idiopathic recurrent pancreatitis. These data suggest that endoscopic pancreatic therapy may result in symptomatic improvement, eliminating the need for surgery in selected children. Furthermore, the study demonstrates that manipulation of the pancreatic duct is comparatively safe and less hazardous than formerly believed.

Expression of TCF, TPF/YY1, and the Sp family transcription factors in rabbit endometrium throughout pregnancy.

BACKGROUND: TCF, TPF/YY1, and the Sp family are specific transcription factors that bind sequences found within the uteroglobin (UG) gene promoter region that are necessary for transcription. To date, UG gene expression and regulation in vivo are not fully understood. The purpose of this study was to assess the expression patterns of these factors in the rabbit endometrium throughout pregnancy. METHODS: Endometrial nuclear extracts were obtained from female rabbits on days 0, 3, 5, 7, 15, and 28 after mating. Transcription factor expression was assessed by DNA-protein binding assays using endometrial nuclear proteins and specific oligonucleotides. Band shifts were observed on 4% acrylamide gels and analyzed by densitometry. RESULTS: The expression patterns of the transcription factors analyzed here differed, as TPF/YY1 and Sp3/SpR-2 were expressed constitutively while TCF and Sp1 showed variable expression patterns throughout pregnancy. CONCLUSIONS: Our results suggest that UG gene expression in the intact pregnant rabbit is controlled by two constitutive and two regulated factors, and that the DNA-binding sites are located at the TATA box and the GT1 sites within the gene promoter.

[Anti-tuberculosis drug consumption as an indicator of the epidemiological situation of tuberculosis in Spain].

OBJECTIVES: To describe anti-tuberculosis drug consumption in Spain for the period, 1985-1995, compare the associated time trend and geographical pattern against case reports of tuberculosis (TB), and estimate the number of persons undergoing anti-tuberculosis therapy in 1995. METHODS: The official Drug Database was used to ascertain consumption of anti-tuberculosis drugs (isoniazid, rifampicin, pyrazinamide and ethambutol) in Spain during the period, 1985-1995. The technical units of measurement used for comparison purposes were daily defined dose (DDD) and DDD rate per day per 100,000 population. Annual trends and geographical patterns of consumption were plotted. The respective numbers of persons treated in 1995 with each of the four drugs were first estimated and then compared against TB case reports. RESULTS: There was an overall decline in the consumption of isoniazid, rifampicin and ethambutol over the period, 1985-1995, though the former two registered rises in 1991 and 1992. Pyrazinamide consumption showed growth throughout the study period. The highest 1995 consumption rates were registered by Galicia, Cantabria, Asturias, the Basque Country, Ceuta and Melilla, and the lowest by the Canary Islands and Navarre. Comparisons run against TB case reports revealed a greater degree of underreporting in certain provinces. In 1995, approximately 18,858 persons (48 per 100,000 population) must be assumed to have undergone pyrazinamide therapy in Spain, indicating that the reported TB rate of 22 per 100,000 population could well represent underreporting in excess of 100%. CONCLUSIONS: The trend in anti-tuberculosis drug consumption reflects shifts in treatment guidelines and is compatible with a rise in TB incidence in recent years. Major underreporting of TB marked by wide inter-regional and -provincial differences was in evidence. Pyrazinamide consumption is probably the best indicator for estimating minimum TB incidence.

[On Lutzomyia davisi (Root, 1934) and some related species (diptera, Phlebotomidae) (author's transl)].

Lutzomyia davisi (Root, 1934) and two related South American species are distinguished. Both sexes of L. robini n. sp. and the female of L. claustrei Abonnenc, Léger et Fauran, 1979 are described and illustrated for the first time. A redescription of both sexes of L. davisi is provided. The known distribution in French Guiana and Brazil for each species is given.

Chronic portal hypertension in the rat by triple-portal stenosing ligation.

A surgical technique based on the development of a triple stenosing ligation is used to worsen the complications inherent to the prehepatic chronic portal hypertension. The results have been compared with those obtained in rats with a single-portal stenosing ligation. An increase (p <.05) in the body, liver, spleen, and kidney weights as well as a decrease (p <.001) in the testes weight to body weight ratio were produced in both groups of animals. In addition, the variability in the obtained weights, particularly in the liver weight, stands out. The incidence of portosystemic and portohepatic collateral circulation and of the mesenteric venous vasculopathy increases in the animals with triple-portal stenosing ligation. The new proposed technique is a valid alternative to the classic one that used single portal stenosing ligation.

Microsurgical extrahepatic cholestasis in the rat: a long-term study.

An experimental model of microsurgical cholestasis is studied as an alternative to the most frequently used surgical techniques, based on the section of the common bile duct. This microsurgical technique consists of the resection of the extrahepatic biliary tract, that is, of the common bile duct in continuity with the bile ducts that drain the four lobes of the rat liver. At 30 days of evolution, rats with microsurgical cholestasis do not develop biliary pseudocysts or intraperitoneal hilar hepatopulmonary abscesses and show an increase (p < 0.001) in total bilirubin (9.50 +/- 1.50 mg/dL vs. 1.60 +/- 0.35 mg/dL), bile acids (225 +/- 87 micromol/L vs. 12.5 +/- 14.50 micromol/L), gamma-glutamyltranspeptidase (375 +/- 143 U/L vs. 8 +/- 11 U/L), and alkaline phosphatase (73 +/- 25 U/L vs. 23 +/- 4 U/L) levels. The histological study shows fibrosis with biliary proliferation. The microsurgical cholestasis technique is a valid alternative to other techniques and can be an adequate experimental model for the study of etiopathogenic mechanisms of obstructive jaundice and especially to study extrahepatic biliary atresia.

[Exposition and sensitization to Tyrophagus putrescentiae in a allergic population to Dermatophagoides pteronyssinus in Huelva, Spain].

BACKGROUND: In this work we analyzed the allergological importance of Tyrophagus putrescentiae (Tp) in Huelva (SE Spain). We studied the level of exposition and the grade of sensitization to Tp in a group of patients sensitized to Dermatophagoides pteronyssinus (Dpt). The allergenic cross-reactivity between Dpt and Tp was determined by RAST inhibition. METHODS AND RESULTS: We analyzed house dust samples from the dwellings of allergic patients with documented Dpt sensitization. Skin test (ST), conjunctival provocation (CP) and/or specific IgE (RAST) to Tp were performed when Tp was identificated in the house dust sample of the patient. Among the 136 dust samples studied, Dpt was the most frequently identified mite species (94.8%) and Tp was found in third position (41.1%) after Glycyphagus domesticus (54.4%). Among the 45 patients studied, 23 (51.1%) presented, at least, two positive tests, 18 (40%) were not sensitized to Tp and 4 (8.8%) showed contradictory results. 26 patients (57.7%) inhabited in urban areas and 19 (42.2%) in rural regions. We determined specific IgE (RAST) to Tp in 25 patients, and the results were positive in 12, with only 7 with values greater than 2 kU/L. No significant correlation were found between IgE-antibody levels to Dpt and Tp. The RAST inhibition studies confirmed the low cross-reactivity between these mites and only in one patient Dpt partially inhibited the IgE-binding to Dpt extract. CONCLUSIONS: Tp was the second more frequent storage mite in the house dust samples from patients allergic to Dpt in Huelva. However, only half of the exposed patients were sensitized to Tp and the majority inhabited in urban areas. No significant correlation were found between IgE-antibody levels to Dpt and Tp. The RAST inhibition studies confirmed the low cross-reactivity between these mites.

Rabies virus infection of cultured adult mouse dorsal root ganglion neurons.

An in vitro model of adult dorsal root ganglion neurons infection by rabies virus is described, Viral marked neurotropism is observed, and the percentage and the degree of infection of the neurons is higher than in non neuronal cells, even if neurons are the minority of the cells in the culture. The neuritic tree is also heavily infected by the virus.

A 70% hepatectomy in the rat using a microsurgical technique.

An experimental model of 70% hepatectomy in the rat using a microsurgical technique is described. An operative microscope (Carl Zeiss 10x2) dissection made it possible to identify the hilar anatomical variations and then to avoid unexpected damage to the arterio-portal and biliary branches belonging to the remaining hepatic parenchyma. The use of this microsurgical technique allows for a more homogenous experimental model because it avoids the functional impairment of the remaining liver.

Progressive hepatocytic fatty infiltration in rats with prehepatic portal hypertension.

BACKGROUND/AIMS: Homogenous evolution, with a narrow range of portal hypertension, degree of portosystemic shunt and hepatic atrophy has been described in the experimental model of prehepatic portal hypertension in the rat. However the great differences observed in the rats' liver weight could be attributed to a pathological alteration of the liver. Based on this, we performed an evolutive histological study of the liver. This study shows the existence of a progressive hepatocytic fatty infiltration. METHODOLOGY: Male Wistar rats with portal hypertension induced by triple stenosing ligation of the portal vein at 1 month (group II, n=4) and at 1 year (group IV, n=10) of postoperative evolution were used. The portal pressure, body, liver and splenic weights, types of collateral circulation and degree of mesenteric venous congestion were studied. The intracytoplasmatic lipid microvacuoles were quantified in hepatocytes with an image analyzer (software MIP/CID, Spain). The results were compared with those obtained in control rats with the same evolutive periods (Groups I and III). RESULTS: The hepatic fatty infiltration in Group II (TPVS 1 month) (30.12+/-53.92 micron2) is similar to that presented by Group III (Control 1 year) (16.52+/-45.20 micron2), while there is an increase (p<0.001) in Group IV (triple portal vein stenosis 1 year) (182.03+/-371.42 micron2) in relation to the other groups studied. The progressive hepatic fatty infiltration in triple portal vein stenosis rats is associated with a decrease of portal pressure and of the incidence of liver hepatic atrophy, portosystemic collateral circulation and mesenteric venous congestion. CONCLUSIONS: TPVS produces progressive hepatocytic fatty infiltration in the rat so that this prehepatic portal hypertension experimental model could also be considered as a hepatic steatosis model.

Arterial microanastomosis technique by using only one stitch.

The starting point of this work is the description of a microsurgical technique designed to carry out termino-terminal arterial anastomosis in rats. The technique is based on the intussusception of the afferent vessel of the anastomosis in the efferent vessel, both of which are fixed by a horizontal U-shaped stitch. Of the twelve cases which make up this series, haemorrhage appeared in two, and a postanastomotic aneurysm developed in one. The lack of complications both before and after operating, even in the long term, in the balance of the cases (75%), together with the simple and quick procedure of this kind of anastomosis, makes this technique a possible alternative to the classical techniques of vascular microanastomosis in experimental surgery.
