[Clinical double-blind study with two different dosages of maprotiline (150 and 225 mg per day) (author's transl)].

Effects and side-effects of 150 mg and 225 mg maprotiline per day were compared by means of a double-blind trial in 20 depressed inpatients. The first 2 days patients received a high initial dosage of 300 mg per day. Patients were examined on days 0, 2, 5, 10, 15, 20, and 30. Symptoms were evaluated by the AMP system and the Hamilton scale for depressions. Laboratory examinations were carried out on days 0, 10, 20, and 30. Exanthemas developed in five patients, three of whom were on the higher dosage. Moreover, the lower dosage caused less fine hand tremor. Coinciding with the beginning of treatment a linear decrease of depressive symptoms was noted. This demonstrates the rapid onset of the antidepressant effect. Moreover, contrary to what has been stated for antidepressants generally, the onset of action was frequently noted well before 10-20 days of treatment. Some patients improved within a few days while others needed more time. The time lag until antidepressants influence depressive symptoms shows pronounced individual differences. No significant difference between the two dosages was found. The profiles show a better antidepressant effect for the higher dosage; however, because of a higher incidence of side-effects on the higher dosage of maprotiline, it cannot be recommended as routine. On the other hand, depressive inpatients should receive a daily dosage of at least 150 mg. Our findings suggest a dose-effect relationship for maprotiline.

[Development of drug, alcohol and tobacco consumption. Comparison between 2 studies from 1971 and 1974 in the canton Zurich].

Consumption of drugs, alcohol and tobacco has been compared in two populations of 19-year-old men, of 1971 (2785) and 1974 (1617), drawn from three different geographical regions of the Canton of Zurich. The hypothesis that the percentage of drug users has risen over the last three years was not confirmed: as previously, 25% have had experience of drugs. As expected, however, there was more regular and heavy drug users in 1974 than in 1971. Also, there is a more marked trend towards extension of drug consumption to rural areas than in 1971. The percentage of cigarette smokers has remained stable, but per capita cigarette consumption has increased. Contrary to expectation, alcohol consumption showed the sharpest rise in all three regions. The study suggests that drug consumption is not to be considered in isolation but in conjunction with the use of alcohol and tobacco.

[Activity profile of carpipramine. Results of an open trial and a double-blind trial versus doxepin].

During an uncontrolled trial 46 depressed patients (39 endogenous depressions, 5 schizoaffective psychoses and 2 paranoic schizophrenics with depressive syndromes) were treated for 43 days on the average with 3 X 100 mg 1-[3-(10,11-dihydro-5H-dibenz[b,f]-azepin-5-yl)-propyl]-4-piperidino-piperidine-4-carboxamide-dihydrochloride-monohydrate (carpipramine) daily. The clinical impression of the improvement and the results of the Hamilton-Scale for depressions (19 patients, 24 items) showed a clear antidepressive effect of carpipramine. During a double-blind trial 14 patients were treated with carpipramine and 16 with doxepine for 30 days. Most patients suffered from endogenous depressions with paranoic symptoms or from schizophrenia with depressive syndromes. Statistical analysis of the Hamilton-Scale for depressions and the AMP-System showed the antidepressive and antipsychotic effect of carpipramine. Analysis of covariance showed no significant difference between carpipramine and doxepine. Altogether we treated 60 depressive patients with carpipramine. 26 patients improved very well and 11 moderately, that means 37 patients out of 60 reacted positively to therapy with carpipramine. One endogenous depression and a schizoaffective psychosis changed into a manic phase. A provocation of schizophrenic symptoms was not noticed. Carpipramine was very well tolerated and can be classified as a non-sedative antidepressant with an antipsychotic effect.

[Studies with the AMP system: comparison of symptoms from Berlin and Zürich].

Symptoms of 1830 psychiatric patients from Berlin and of 783 patients from Zurich were assessed using the AMP-system. The symptoms were analysed according to their frequency and their potency to differentiate diagnostic groups. The following 4 categories of symptoms proved to be useful: frequently occurring and specific or unspecific, rarely occurring and specific or unspecific. Factor analyses of the frequently occurring unspecific symptoms yielded three general dimensions, i.e., "depression", "disturbance of memory and thinking" and "hostility".

Comparison of a new antidepressive, lofepramine, with imipramine in a double-blind multicentre trial.

Lofepramine, an imipramine derivative, shows lower acute toxicity in animals when compared with desipramine and imipramine. Its anticholinergic effect is less pronounced than that of desipramine. In an open clinical trial lofepramine showed a marked antidepressive action. A double-blind multicenter trial of lofepramine v. imipramine, evaluated by means of the AMP system, showed a remarkable degree of concurrence with regards to the effects of those two products.

[The logic of decision in symptom rating (author's transl)].

The rating systems used in psychiatry consider sometimes different ways of classifying parameters, such as the valuability and the certainty of decision. Those two parameters have been analysed by means of the AMP-ratings of two samples (Berlin: 1879 patients, Zürich: 786 patients). We have found that the reply "not valuable" appears much less frequently than the reply "questionable valuable". For delusional symptoms and disorders of perception the certainty of decision is lowest. The various analyses carried out lead us to the conclusion that without loosing informatory value of any consequence we can renounce any separate documentation on valuability and certainty of decision.

[Clinical study on five depot neuroleptics. Comparison of effective profiles of fluphenazinedecanoate, fluspirilene, penfluridol, perphenazine-enanthate and pipothiazinepalmitate].

Five depot neuroleptics (fluphenazinedeconoate, fluspirilene, pipothiazinepalmitate, penfluridol and perphenazine-enanthate) were compared based on clinical trials in subacute and chronic schizophrenic patients. The psychopathological symptoms were documented by means of the AMP-system. Statistical analyses showed several differences between the effects of the five substances. The AMP-system proved a useful instrument to differentiate similar drugs.

[Clinical trial of lofepramine versus imipramine].

Lofepramin is a new derivative of imipramine. Its effects and side effects were compared to those of imipramine in a double-blind trial. 49 hospitalized patients, suffering from endogenous depression, were treated for 20 days with 210 mg lofepramin daily and 52 other patients with 150 mg imipramine daily. The results of the repeated psychiatric examinations were documented using the AMP system. Both substances proved to have high antidepressant effects and showed no significant difference in the quality of their effects.

[Onset and course of schizophrenia (the results of a statistical study)].

The report contains the results of an examination of 709 patients with catatonia, paranoid, schizoaffective forms of schizophrenia. The study involved an investigation of the form, sex, general amount of psychotic episodes depending upon the time of observation, duration of productive episodes and the intervals between them. The achieved results indicate to a close correlation between the productive manifestations in schixophrenia and affective psychosis.
