Surgical complications in 500 kidney transplantations.

The surgical complications in a series of 500 transplantations done by a single transplantation unit with over ten years' experience in the field are reported. The complications are divided into vascular, urological and gastrointestinal. In addition, renal rupture, primary wound infections and operative deaths are reported. Most of the losses of transplants caused by surgical complications are due to renal rupture and vascular complications, accounting for 3.6 and 2 percent of losses respectively out of the 500 kidney transplants. The urological complications caused a loss of the remaining 1.2% of a total of 6.8%. tthe overall mortality was highest in gastrointestinal complications with 6% of the total of 10% deaths following surgical complications. The mortality rate in the group of patients with gastrointestinal bleeding and perforations and with acute pancreatitis was extremely high. More accurate screening for potential candidates for gastroduodenal ulceration and better prophylactic treatment is needed. More active treatment of acute pancreatitis is suggested.

Renin/angiotensin system in hypertension after traumatic renal-artery thrombosis.

Hypertension was found in four patients after unilateral renal-artery thrombosis following blunt abdominal trauma. In one patient, who was followed up from the time of injury, renin hypersecretion and secondary aldosteronism developed within a few days, and hypertension was present 12 weeks later. Increasing haemoglobin and raised blood-erythropoietin concentrations were also found. In the three other patients, hypertension was found casually within 3 years of trauma. In all patients, unilateral renin production by the affected kidney was significantly increased. Nephrectomy of the diseased kidney corrected hypertension and endocrine abnormalities in all patients. The delayed onset of hypertension despite early activation of the renin/angiotensin/aldosterone axis accords with the course of events observed in experimentally induced hypertension in rats, and suggests that several weeks or even months are required for hypertension to develop after sudden renal-artery occlusion in man. Slowly acting mechanisms, probably initiated by hypersecretion of renin, may be responsible for the hypertension.

Oxygen consumption by granulation tissue in bipedicle tube flaps.

The oxygen consumption rate (QO2) of granulation tissue from cylinders implanted in bipedicle tube flaps of rabbits was measured weeks 2 to 7 in vitro in a Biological Oxygen Monitor. Granulation tissue from cylinders implanted in skin folds was used as control. The QO2 of granulation tissue decreased slowly as the oxygen tension in the medium fell. All measurements were made at an oxygen tension of 75 mmHg. At week 2 the QO2 of granulation tissue in flaps was high and a decrease occurred between week 2 and week 4. A further decrease took place between weeks 6 and 7 and a very low QO2 was found at week 7. Control granulation tissue showed a higher QO2 than skin flap granulation tissue at weeks 3, 4, 5 and 7.

Protein synthesis by granulation tissue in bipedicle tube flaps.

The protein synthesis and the collagen synthesis by 2- to 7-week-old granulation tissue from implanted cylinders in bipedicle tube flaps on rabbits were measured in vitro by the incorporation of 14C-proline in proteins and in collagen hydroxyproline. Granulation tissue from cylinders implanted in skin folds were used as controls. In the skin flap group the protein synthesis was high at week 2, after which it decreased to week 5. In the controls the protein synthesis was the same as in the skin flap group at week 2. A decrease occurred between weeks 2 and 4 and a further decrease between weeks 6 and 7. At weeks 5 and 6 the protein synthesis was higher in the controls than in the skin flap group, but at week 7 it was the reverse. The collagen synthesis was high and equal in both groups at weeks 2 and 3 and then decreased to weeks 4 and 5. Between weeks 6 and 7 a further decrease was found in the controls, but not in the skin flap group.

Granulation tissue formation in viscose cellulose sponges of different design.

Granulation tissue formation was studied in subcutaneously implanted viscose cellulose sponges of different design using histological and chemical methods. Two different models of sponges were used, one of cubical shape and another rectangular and thin. The sponges had the same weight. After one week of implantation, histological examination of the cubical sponges showed granulation tissue only in the periphery and a central cavity filled with inflammatory cells. The histological texture of the thin sponges showed that they were almost completely filled with granulation tissue with a minimal central cavity and very few inflammatory cells. DNA values of the cubical sponges were about 20% higher than those of the thin sponges. After aspiration of the exudate in the cubical sponges, the DNA values were about the same as those in the thin sponges. The amount of hydroxyproline and RNA were about the same regardless of whether the sponges were cubic or thin or the exudate of the cubic sponges had been aspirated or not. At two weeks there was still a central cavity in the cubic sponges but very few inflammatory cells. DNA values at this time of observation were also the same in both models of sponges. Thus the thin-shaped model of sponge seems to be preferable to the cubic one when the time of observation is shorter than two weeks.

Granulation tissue formation in oophorectomized rats treated with female sex hormones. I. A histological study.

Granulation tissue formation was studied in experimental granulomas up to three weeks in oophorectomized rats treated with female sex hormones. The influence of estrogen and progesterone during the early phase of healing could sucessfully be followed histologically. The amount of DNA and hemoglobin in the granulomas correlated well with the amount of cells and the capillary bed found in the histological sections.

Granulation tissue formation in oophorectomized rats treated with female sex hormones. II. Studies on the amount of collagen and on tensile strength.

The content of hydroxyproline and tensile strength of granulation tissue was studied in oophorectomized rats treated with female sex hormones. Estrogen alone caused a slight decrease of hydroxyproline, but had no effect on the tensile strength of the granulation tissue at one week. Progesterone alone or in combination with estrogen decreased the content of hydroxyproline and the tensile strength of the granulation tissue up to one week in the case of progesterone-treated animals and up to two weeks in the case of animals treated with the combination of the hormones. At one week no direct correlation was found between the amount of hydroxyproline and the tensile strength of granulation tissue in oophorectomized rats treated with estrogen and progesterone in combination.
