Age-specific norms for the Mini-Mental State Exam.

We administered the Mini-Mental State Exam (MMSE) to 194 healthy men and women, ages 40 to 89 years. Total score was significantly associated with age (p less than 0.0001), but not vocabulary, education, Beck's Depression Inventory Score, or sex. The lowest quartile cutoff scores for the MMSE by decade were 40s - 29; 50s - 28; 60s - 28; 70s - 28; and 80s - 26. When screening for progressive decline in cognitive performance, the use of age-specific norms may provide greater sensitivity than the present recommended cutoff score of less than 24.

New techniques for the diagnosis of carpal tunnel syndrome.

An overview is presented of the assessment of carpal tunnel syndrome (CTS) in manual workers, with emphasis on the distinction from other compression syndromes and from diffuse polyneuropathy. Clinical examinations, electrodiagnostic studies, the determination of vibration threshold, thermography, and, recently, computerized tomography have been used. This last-mentioned method has also been employed in a study indicating that a subgroup of the general population may possess a risk factor, small carpal canal, associated with the development of CTS.

Hazards associated with anal erotic activity.

The recent liberalization of attitudes towards sexuality has brought with it the desire by some individuals to seek alternate methods of sexual stimulation and gratification, among them an exploration of anal eroticism. Various practices associated with anal sexuality carry with them the potential for medical complications. In this paper some of the hazards associated with anal eroticism are outlined and potential complications are discussed. Topics discussed include anal masturbation, enemas, sexually related anorectal disease, and the hazards of foreign objects introduced into the rectum. The intent of this paper is to provide concerned professionals dealing with various aspects of human sexuality with information relevant to anal eroticism.

Man's purgative passion.

Overconcern with bowel function has been prevalent throughout history. Overuse of laxatives is persisting for a certain segment of the population even today. This paper attempts to put this practice into perspective by presenting a review of mankind's treatment and mistreatment of the GI system from the ancient Egyptians to modern times.

Some anatomical and physiological aspects of anal sexual practices.

Anal manipulation and penetration produce stimulation enjoyed as sexual by some people. Although this type of sexual activity is not new, the current social climate of sexual freedom and experimentation has brought it out into the open. This paper reviews some of the anatomical, physiological, and behavioral aspects of this variation of human sexual gratification, and provides the practicing professional, who has to deal with questions on anal sexuality, with information on the subject and suitable background material and literature references for further study.

Klismaphilia--a physiological perspective.

Dr. Joanne Denko coined the work klismaphilia to describe the practices of some of her patients who enjoyed the use of enemas as a sexual stimulant. Since then questions occasionally appear in the professional literature asking about the relationship between enemas and sexual pleasure. This paper considers some of the physiological aspects of the human sexual apparatus that relate to anal sensitivity and explores why klismaphilia can be sexually grafifying. The paper starts with a discussion of the physiological basis for anal sensitivity and anal masturbation in both the human male and the human female. The paper then goes on to relate all this to the sexual sensations received from an enema, and discusses the similarities and differences between all these types of stimulation. Some of the psychological aspects of klismaphilia are also considered in relationship to the physiology involved. The paper concludes with a brief discussion of masked anal masturbation among the population at large. A comprehensive list of references from the literature is given to support these findings.

A new design for an implantable hearing aid.

There have been attempts to design a totally implantable hearing aid but, so far, the devices described in the literature have had various disadvantages. This paper describes a new type of hearing aid that is being implanted and tested in chinchillas. This device parallels the natural hearing mechanism and operates in conjunction with residual hearing, while providing amplified drive to the ossicular chain. Implantation procedures and results are described and discussed.

Neuropsychological function and psychosocial status of alcohol rehabilitation program residents.

This investigation examined the relationship between changes in psychosocial status and changes in neuropsychological function of clients undergoing treatment for substance use. A sample of 74 adults enrolled at a rehabilitation program for substance use were randomly assigned to two groups (one serving as a comparison group for practice effects). Participants were tested three times over 60 days on self-esteem, motivation, depression, and neuropsychological function. Consistent with other studies, participants demonstrated impaired baseline neuropsychological function that improved with alcoholic abstinence; however, changes in motivation and depression were associated with positive changes on few tests of neuropsychological function. The more common predictor of improved motor function test scores was low baseline function. Participants who remained in treatment were more likely to be Black, homeless, report less alcohol consumption, and score lower on a motivational scale than those who left treatment.
