An anomalous calyx in the mid-kidney: an anatomical variant.

Three cases in which there was an anomalous short calyx without a papilla in the mid kidney are described. This is believed to be a developmental abnormality which is associated with a pelvicalyceal pattern of two major calyces, and with renal "pseudotumour". It is important to recognize this calyceal appearance in order to distinguish it from calyceal disease.

Hospital topics. Pulmonary function after lymphography.

The effects of lymphography on ventilatory function and gas transfer factor were studied in nine patients. Serial measurements made up to one month showed no change in the forced expiratory volume in one second or vital capacity. A small but reversible fall in transfer factor was found. The greatest reduction was at 24 or 48 hours. It was concluded that patients with normal lungs are unlikely to encounter difficulties but that patients with severe lung disease require careful assessment before lymphography.

Use of peroxidase-conjugated antiglobulin as an alternative to immunofluorescence for the detection of antinuclear factor in serum.

A method of detecting serum antinuclear factor by the use of antiglobulin conjugated with horseradish peroxidase has been compared with a standard procedure using a fluorescent label. The horseradish peroxidase conjugate was evaluated by block titration against positive serum and by its performance as a test reagent for screening and titrating sera. It is concluded that the horse-radish peroxidase method, in which an ordinary light microscope is employed, provides a satisfactory alternative to immunofluorescence for routine tests for antinuclear factor.

Translocation of Sugars in Cucurbita melopepo IV. Effects of Temperature Change.

A study has been made of the temperature control of translocation localized to regions of the stem, petiole and hypocotyl of Cucurbita melopepo. The basipetal and acropetal movement of translocated (14)C-labeled compounds in the phloem tissue, measured over a 45-minute period, was almost completely inhibited at 0 degrees . At 10 degrees a partial inhibition occurred while an extremely variable degree of inhibition occurred at 15 degrees . Above 15 degrees to 35 degrees temperature ceased to be a limiting factor in the movement of (14)C-labeled compounds. At 45 degrees partial inhibition was observed while at 55 degrees there was an almost complete cessation. The localized temperature treatment of the plant parts did not disturb the rate of (14)CO(2) assimilation or the export of (14)C compounds by the leaf blade. Translocated compounds unable to pass a temperature inhibited zone were diverted toward other importing regions of the plant. The similarity of the translocation response to temperature change in the various organs of the plant indicated a uniform mechanism throughout the plant controlling movement of the major proportion of the translocated compounds. The temperature characteristics of the mechanism were found to closely parallel those of protoplasmic streaming in chill-sensitive plants.

Low field strength magnetic resonance imaging of bone marrow in acute leukaemia.

The accuracy of low field strength (0.08 Tesla) magnetic resonance imaging (MRI) of bone marrow for the detection of acute leukaemia in adults has been assessed by comparison with bone marrow biopsy results. Spin lattice relaxation time (T1) measurements from patients were compared with those from 90 volunteers. Eighteen patients were studied at the time of diagnosis of leukaemia. Bone marrow T1 was prolonged in all cases. One of two patients with refractory anaemia with excess of blasts in transformation (RAEBt) had prolonged bone marrow T1, the other had normal T1. T1 at the time of diagnosis for patients with acute leukaemia or RAEBt correlated with the cellularity and blast cell count in the marrow. None of the 17 patients who were studied when in long-term remission of leukaemia had prolonged marrow T1. Serial studies were undertaken in five of the newly diagnosed patients. An increase in bone marrow T1 was observed in each of four patients studied seven days after the start of treatment, at a time when they showed a decrease in leukaemic cells on peripheral blood examination. T1 measurements made 3 weeks after the commencement of chemotherapy were similar to pretreatment values and did not reflect the reduction in leukaemic infiltration observed on bone marrow needle aspirate. The implications and possible explanations for these findings are discussed.

Renal involvement in patients with non-Hodgkin's lymphoma: clinical and pathological features in 23 cases.

Twenty-three cases of renal or perirenal involvement by non-Hodgkin's lymphoma were identified from a retrospective review of computed tomography (CT) scans performed on patients at St Bartholomew's Hospital over an 8-year period. The histology, clinical features and survival of these patients were examined. Eighteen of the 23 patients had high grade histological subtypes according to the Kiel classification. The majority of cases were identified from scans performed at the time of presentation. In nine cases renal lymphoma was observed in the absence of detectable retroperitoneal lymphadenopathy. Staging was rarely altered by the finding of renal lymphoma as 21 of the 23 patients had involvement of other extranodal sites in addition to the kidney. Survival did not appear to be adversely affected by the presence of renal lymphoma at the time of initial diagnosis. During the same period no cases of renal involvement were identified in patients with Hodgkin's disease, although one patient had perirenal involvement.

Relationship among social and intrapersonal risk, alcohol expectancies, and alcohol usage among early adolescents.

Numerous studies have shown that adolescents' expectancies about the effects of alcohol influence usage. Expectancies are described as mediators between social influences (such as peer and parental influences) and alcohol use. The present study examined the relationship between social risk factors (peer influence and parental approval), intrapersonal risk factors (tolerance of deviance and sensation seeking), alcohol expectancies, and alcohol usage in a cohort of seventh graders. It was hypothesized that social risk factors affected alcohol usage indirectly through expectancies, while intrapersonal risk factors influenced usage both directly and indirectly. Structural modeling was used to examine the hypothesized relationships between these four constructs. Results indicated that the hypothesis that expectancies mediate social influences was not supported, and that social influences exerted a direct influence on usage independent of expectancies. Results suggested that the view that expectancies mediate social risk factors may need modification. Suggestions for future research in this area include the need to examine the relationships among these constructs longitudinally and with adolescents over a broader age range.

Influence of family disharmony and parental alcohol use on adolescent social skills, self-efficacy, and alcohol use.

Deficits in social skills have been found to be related to adolescent substance use. Little effort has been devoted to understanding how family factors influence the acquisition of these skills. This study examined the manner in which family disharmony and parental alcohol use affect adolescent alcohol use through their influence on the acquisition of social skills and self-efficacy regarding one's ability to utilize these skills. It was hypothesized that family disharmony and parental alcohol use directly influenced usage and affected usage indirectly through their influence on acquisition of social skills. Social skills were also hypothesized to affect usage directly and to affect usage indirectly by influencing beliefs regarding one's ability to implement these skills. Results indicated that family disharmony was directly related to adolescent alcohol use and social skills. Parental alcohol use was related only to adolescent usages, not to social skills. Social skills were related to self-efficacy, which was related to alcohol use but was not directly related to usage. Results are discussed in terms of the importance of family influences on adolescent alcohol use, the importance of including families in prevention efforts, and the importance of the acquisition of self-efficacy relative to social skills.

Detection of a systemic effect of malignancy in vivo by magnetic resonance imaging.

In animals bearing tumors prolongation of spin lattice relaxation time (T1) has been detected in vitro in organs not directly affected by the malignancy. This has been termed the "Systemic Effect." In this study the possible existence of such an effect in the liver, muscle and fat of humans with lymphoma has been investigated. In vivo T1 measurements were made using a low field strength (0.08 Tesla) magnetic resonance imager. The mean liver T1 for 19 lymphoma patients with normal liver histology was 206 ms, compared with a mean of 191 ms for 61 volunteers (p less than 0.0001). Among these patients prolongation of liver T1 was related to the extent of disease elsewhere in the body. For 23 patients with Hodgkin's disease (HD) examined at the time of diagnosis, liver T1 was significantly correlated with other known markers of disease extent or activity (alkaline phosphatase level, erythrocyte sedimentation rate and the presence of systemic symptoms). No such correlations were observed among 25 patients with non-Hodgkin's lymphoma (NHL). Muscle and fat T1 was measured in 26 patients with lymphoma, 14 patients with acute leukemia and 88 volunteers. Seven of the patients with lymphoma and 2 of those with leukemia had muscle T1 values above the range observed for volunteers. Similarly, 3 patients with lymphoma and 1 with leukemia had prolonged fat T1. These findings indicate that a systemic effect of malignancy on T1 is detectable in a proportion of humans with lymphoma or leukemia.
