Long-term bladder symptomatology following radiotherapy for cervical carcinoma.

The prevalence of long-term bladder symptoms 5-11 years after radiotherapy for cervical carcinoma was investigated by postal questionnaire. Sixty-six replies (68%) were received. Only 29 (44%) were asymptomatic and 17 (26%) had severe symptoms. Urgency and urge incontinence were the most common symptoms occurring in 30 women (45%). Significant frequency and nocturia occurred in 23 women (35%) but voiding problems were less common. These results suggest that long-term bladder dysfunction is a common problem following radiotherapy for cervical carcinoma.

Effect of high-intensity exercise on the VE-VCO2 relationship.

The intrinsic relationship between ventilation (VE) and carbon dioxide output (VCO2) is described by the modified alveolar ventilation equation VE = VCO2 k/PaCO2(1-VD/VT) where PaCO2 is the partial pressure of CO2 in the arterial blood and VD/VT is the dead space fraction of the tidal volume. Previous investigators have reported that high-intensity exercise uncouples VE from VCO2; however, they did not measure the PaCO2 and VD/VT components of the overall relationship. In an attempt to provide a more complete analysis of the effects of high-intensity exercise on the VE-VCO2 relationship, we undertook an investigation where five subjects volunteered to perform three steady-state tests (SS1, SS2, SS3) at 60 W. One week after SS1 each subject was required to perform repeated 1-min bouts of exercise corresponding to a work rate of approximately 140% of maximal oxygen uptake (VO2max). Two and 24 h later the subjects performed SS2 and SS3, respectively. This exercise intervention caused PaCO2 during SS2 and SS3 to be regulated (P less than 0.01) approximately 4 Torr below the control (SS1) value of 38.8 Torr. Additionally, significant alterations were noted for VCO2 with corresponding values of 1.15 (SS1), 1.10 (SS2), and 1.04 (SS3) l/min. No changes were noted in either VD/VT or VE. In summary, it seems reasonable to suggest that the disproportionate increase in VE with respect to VCO2 noted in earlier work does not reflect an uncoupling. Rather the slope of the VE-VCO2 relationship is increased in a predictable manner as described by the modified alveolar ventilation equation.

Oxygen uptake kinetics and lactate concentration during exercise in humans.

For constant-load exercise of moderate intensity, oxygen uptake (VO2) increases monoexponentially, reaching a constant value within 3 min, i.e., steady state. However, at work rates associated with increased blood lactate, i.e., above the lactate threshold (LT), VO2 continues to increase slowly beyond 3 min; this delays or precludes steady state. We therefore correlated the characteristics of this slow phase of the VO2 kinetics with the increase in blood lactate during 6 randomized, constant-load cycle exercise tests in 6 normal men. One test was below and the others were at various levels above our gas exchange estimate of LT. The slow kinetic phase of VO2, characterized as the increase between the third and the sixth minute of exercise [delta VO2 (6-3)] or the third minute and termination of exercise [delta VO2 (term-3)], was linearly correlated with the blood lactate increase. The VO2 at termination of a given work rate above LT was greater than predicted from the sub-LT VO2 versus work-rate relationship. Work rates less than approximately 50% of the difference between VO2max and LT resulted in lactate and VO2 curves that reached recognizable asymptotes. At the higher work rates, both the lactate and VO2 curves continued to rise to the point of fatigue. We conclude that positive values for delta VO2 (6-3) and delta VO2(term-3) during constant-load exercise only occur at work rates above the LT, and the magnitudes of which are highly correlated with the increase in blood lactate.

Cytoprotective effects of glycine and glutathione against hypoxic injury to renal tubules.

Roles for both the tripeptide, GSH, and individual amino acids in modifying the cellular response to oxygen deprivation-induced injury have been suggested by prior work in kidney and other tissues, but the precise interrelationships have not been clearly defined. We have studied the effects of GSH, its component amino acids, and related compounds on the behavior of isolated renal proximal tubules in a well characterized model of hypoxic injury in vitro. GSH, the combination of cysteine, glutamate, and glycine and glycine alone, when present in the medium during 30 min hypoxia, a duration sufficient to produce extensive irreversible injury in untreated tubules, were protective. Significant effects were detected at 0.25 mM concentrations of the reagents, and protection was nearly complete at concentrations of 1 mM and above. Glutamate and cysteine alone were not protective. The exogenous GSH added to the tubule suspensions was rapidly degraded to its component amino acids. Treatment of tubules with GSH or cysteine, but not glycine, increased intracellular GSH levels. Oxidized GSH was protective. Serine, N-(2-mercaptopropionyl)-glycine, and a panel of agents known to modify injury produced by reactive oxygen metabolites were without benefit. These observations identify a novel and potent action of glycine to modify the course of hypoxic renal tubular cell injury. This effect is independent of changes in cellular GSH metabolism and appears to be unrelated to alterations of cell thiols or reactive oxygen metabolites. Further elucidation of its mechanism may provide insight into both the basic pathophysiology of oxygen deprivation-induced cell injury and a practical way to ameliorate it.

Response of epithelial cells of the choroid plexus in the ferret model for Reye's syndrome.

The structural integrity of epithelial cells of the lateral choroid plexus tissues from the ferret model for Reye's syndrome was examined by transmission electron microscopy after the animals were inoculated intranasally with influenza B, were administered aspirin, and/or were fed an arginine deficient diet singly and in combination. No cellular alteration in the choroidal epithelium was caused by influenza B nor by aspirin, however, the synthetic diet produced random intercellular edema. The structural integrity of choroidal epithelial cells appeared intact after ferrets were infected with influenza B and were given aspirin in combination. Cellular changes were seen, however, after each of these agents was combined with the presentation of the arginine-deficient diet. Intracytoplasmic vacuoles were observed in tissues of the choroid plexus from animals inoculated with influenza B and fed the arginine-deficient diet. Both intercellular vacuolization and intercellular edema were seen in the choroidal epithelia after ferrets were administered aspirin and were fed the synthetic diet. Such cellular changes observed in the choroidal epithelia in response to the paired presentation of these agents were not as severe as those seen in the choroid plexuses of animals in which all three agents were given in combination. These results support the hypothesis that metabolic events that occur after the administration of these agents collectively impact on epithelial cells of the choroid plexus.

DNA sequences of human papillomavirus types 11, 16, and 18 in lesions of the uterine cervix in the west of Scotland.

Punch biopsy specimens of the cervix were examined both histologically and for the presence of human papillomavirus (HPV) DNA sequences. The presence of HPV DNA sequences was sought with the Southern blot technique using radioactively labelled HPV-6, 11, 16, and 18 DNA probes, both together and separately. Twenty six biopsy specimens were examined. Histological examination showed cervical intraepithelial neoplasia grade 2 or 3 in 16 specimens, viral changes (koilocytosis) in four, and inflammation or a normal appearance in three. Eleven specimens were negative for HPV DNA sequences, 10 contained HPV-16 DNA, four contained HPV-18 DNA, and one contained both HPV-18 and HPV-11 DNA. Episomal HPV-16 DNA was detected in one case of cervical intraepithelial neoplasia grade 3 and in five cases of cervical intraepithelial neoplasia grade 2/3 with koilocytosis; and episomal HPV-18 DNA was found in two specimens classed as cervical intraepithelial neoplasia grade 2/3, one of which also contained HPV-11 DNA, and in one specimen that showed viral changes alone. Integrated HPV DNA was found in six specimens (four with HPV-16 DNA and two with HPV-18 DNA), including two cases of chronically inflamed cervix with no histological evidence of viral infection or cervical intraepithelial neoplasia. Detection of viral DNA in early lesions may identify patients at risk of malignant progression. This is the first report of HPV-18 DNA in cervical intraepithelial neoplasia in Scotland.

Fluid in the female pelvis: cyclic patterns.

A total of 254 pelvic sonograms were performed on 40 volunteers in order to determine a cyclic pattern, if any, of free fluid in the pelvis in asymptomatic premenopausal women. Free fluid was observed during all phases of the menstrual cycle. The highest percentage of positive-for-fluid sonograms was observed within 5 days preceding onset of menses in both oral contraceptive users and non-oral contraceptive users. The second highest percentage was observed between days 13 and 21 in non-oral contraceptive users.

Alpha-adrenoreceptor reagents. 3. Synthesis of some 2-substituted 1,4-benzodioxans as selective presynaptic alpha 2-adrenoreceptor antagonists.

The synthesis and pharmacological activity of a series of 2-substituted derivatives of the selective alpha 2-adrenoreceptor antagonist idazoxan (RX 781094) is described. Substitution in this position by alkyl, alkenyl, cycloalkenyl, and alkoxy groups in many cases gives compounds whose potencies and selectivities are significantly greater than those of the parent compound.

Involvement of zinc in the regulation of pHi, motility, and acrosome reactions in sea urchin sperm.

When sperm of Strongylocentrotus purpuratus or Lytechinus pictus are diluted into seawater, motility is initiated; and when exposed to egg jelly, an acrosome reaction is induced. In the presence of a variety of structurally different metal chelators (0.1-1 mM EDTA, EGTA, phenanthroline, dipyridyl, cysteine, or dithiothreitol), motility initiation is delayed and the acrosome reaction is inhibited. Of the metals detected in the sperm of these two species, very low levels of Zn+2 (0.1 microM free Zn+2) uniquely prevent this chelator inhibition. L. pictus sperm concentrate 65Zn+2 from seawater, and EDTA removes 50% of the accumulated 65Zn+2 by 5 min. Since both sperm motility and acrosome reactions are in part regulated by intracellular pH (pHi), the effect of chelators on the sperm pHi was examined by using the fluorescent pH sensitive probe, 9-aminoacridine, EDTA depresses sperm pHi in both species, and 0.1 microM free Zn+2 reverses this pHi depression. When sperm are diluted into media that contain chelators, both NH4Cl and monensin (a Na+/H+ ionophore) increase the sperm pHi and reverse the chelator inhibition of sperm motility and acrosome reactions. The results of this study are consistent with the involvement of a trace metal (probably zinc) in the pHi regulation of sea urchin sperm and indicate a likely mechanism for the previously observed effects of chelators on sperm motility and acrosome reactions.

Lactate, pyruvate, and lactate-to-pyruvate ratio during exercise and recovery.

The pattern of lactate increase and its relation to pyruvate and lactate-to-pyruvate (L/P) ratio were studied during exercise and early recovery in 10 normal subjects for incremental exercise on a cycle ergometer. Gas exchange was measured breath by breath. Lactate and pyruvate were measured by enzymatic techniques. Lactate and log lactate changed only slightly at low levels of O2 uptake (VO2) but both began to abruptly increase at approximately 40-55% of the maximal VO2. However, the point of abrupt increase in pyruvate occurred at higher work rates and the rate of increase was not as great as that for lactate. Thus L/P ratio increased at the same VO2 as the log lactate increase. Following the exercise, pyruvate continued to increase steeply for at least the first 5 recovery min, whereas at 2 min lactate increased only slightly or decreased. Thus arterial L/P ratio reversed its direction of change and decreased toward the resting value by 2 min of recovery. Lactate, as well as L/P ratios, decreased in all subjects by 5 min. This study demonstrates that lactate and pyruvate concentrations increase slightly at low levels of exercise without a change in L/P ratio until a threshold work rate at which lactate abruptly increases without pyruvate. The resulting increase in L/P ratio is progressive as work rate is incremented and abruptly reverses when exercise stops.
