Intestinal lymph flow and lymphatic transport of protein during fat absorption.

A substantial increase in intestinal lymph flow and protein content follows fat ingestion. The effect of intraduodenal feeding of fats was studied in the rat to define the mechanisms responsible. The change appears to be largely independent of the route of fat absorption, that is, whether by the portal venous route or, alternatively, by the lymphatic route. It must be presumed that it is related to events unconnected with the route taken by absorbed fat leaving the intestinal cell.

Metabolism of biliverdin. Biliary excretion of bile pigments after intravenous injection of biliverdin isomers.

14C-labelled biliverdins IX alpha, beta, gamma and delta have been prepared in vitro from haemoglobin obtained from duck erythrocytes incubated with 5-amino[4-14C]-laevulinic acid. When injected intravenously into rats with biliary fistulae, about 60% of the label was recovered in the bile in 24 h after the alpha isomer was given, while approximately 10% was recovered with injection of the beta isomer. The gamma and delta isomers gave intermediate values. In each experiment, most of the recovered isotope was found in association with conjugated bile pigment. Thus, the metabolic pathway for bile pigment excretion in the rat handles the IX alpha isomer preferentially but is not specific for it.

[The medical management of carcinoma of the stomach (author's transl)].

Only a small proportion of patients with carcinoma of the stomach are suitable for radical surgery. For the remainder symptomatic treatment and radiotherapy alone or in combination with chemotherapy can be used to relieve distressing symptoms of pain, nausea and vomiting, and severe anemia. In the case of radiotherapy or chemotherapy, a degree of tumor regression can be expected. There have been encouraging results recently in palliating this disease with new radiotherapy techniques, notably fast neutron irradiation.

Effect of pentagastrin on small bowel structure and function in the rat.

Since chronic pentagastrin stimulates pancreatic and proximal duodenal growth (15), we studied jejunal structure and function following subcutaneous pentagastrin (2 mg-100 g-1 day-1). After 15 days, macroscopic enlargement was confined to the proximal duodenum with a significant increase in wet weight (from 231 +/- SEM 6 to 308 +/- 13 mg) but there was no significant difference in wet and defatted dry intestinal weights, histological measurements of villous height and mucosal thickness or in glucose absorption per unit length of distal duodenum and proximal jejunum. Since chronic pentagastrin causes parietal cell hyperplasia and hypersecretion of gastric acid, the results suggest that the adaptive changes seen in the jejunum after ileectomy are neither mediated by gastrin nor by factors present in gastric secretions.

The tropic action of gastro-intestinal hormones.

There is evidence that under various physiological circumstances long-term adaptation of structure and function of the gut occurs. The mechanisms of these changes are not clear but gastro-intestinal hormones may be involved. In particular, gastrin which has been shown experimentally to stimulate growth and development of parts of the gut, probably has a role in maintaining the structure of the normal upper alimentary tract. Cholecystokinin may be of major importance in producing adaptive changes in the pancreas in response to dietary modifications and enteroglucagon is possibly concerned with maintaining a normal small intestinal structure. The importance of the 'trophic' action of gatrointestinal hormones is becoming more widely recognised and as new gastro-intestinal hormones become established, this aspect of their physiological importance as well as their acute effects, will deserve attention.
