[Bicomponent analysis of the contractile capacity of the heart in ischemic lesions of the myocardium (experimento-clinical study)].

A study was made of cardiac pumping capacity in animals with local myocardial ischemia (9 experiments on cats), in 25 healthy persons and 21 patients suffering from coronary heart disease with stage I-IIA circulatory insufficiency using afterload characteristics (ALC). The latter reflected correlation between left ventricular pressure and aortic blood flow in constant blood filling and in a given inotropic heart condition. For combined assessment of ALC one should know its two parameters: power (maximum pressure in the zero blood flow) and rate (maximum blood flow rate in zero antipressure). It was shown that early stages of cardiac insufficiency were characterized both under experimental and clinical conditions by change in the inclination angle of an afterload curve and a shift of a working point on it to the right.

[Pharmacodynamic study of nitroglycerin preparations by methods of tetrapolar rheoplethysmography of the finger and impedance cardiography in ischemic heart disease].

A method of tetrapolar impedance plethysmography of the finger (TIPF) is suggested for the evaluation of the peripheral hemodynamic effect of drugs containing nitroglycerin. The method is based on the changes in the beta wave amplitude in TIPF as compared to the initial level. The central hemodynamics was studied by means of impedance cardiography (by the cardiac index) simultaneously with TIPF. The results are presented of the comparative pharmacodynamics of 6.4 mg sustac and 6.5 mg nitrong. The hemodynamic effect of sustac was higher that that of nitrong but its duration was 1--1 1/2 h shorter. Extreme variants have been demonstrated of the individual sensitivity of patients to the long-acting nitroglycerin agents.

[Central hemodynamic indices studied by use of a tetrapolar variant of integral rheography of the human body].

Integral rheography is conducted by M. I. Tishchenko's method by means of bipolar rheographs which do not ensure accurate reading of the values which form the formula for determining the stroke volume. A modification of the method is suggested with the use of a tetrapolar diagram of impedance measurement and a technically perfect PPG-202 device with a widened range of measurement of the resistance of the object being studied, which ensures accurate impedance reading and stable calibration. To study the possibilities of the method, comparative investigations by means of the echocardiography and thermodilution methods and by means of the bipolar variant of integral rheography were conducted. The results of the investigations bear evidence that the method described is sufficiently accurate for studying central hemodynamics in the clinic and in the prehospital stage of management of the patients.

[Use of Visken-15 for treating hypertension].

The hypotensive effect of visken-15, a blocking agent of the beta-adrenergic receptors, was studied in 7 patients with stage IB hypertensive disease, in 18 with stage IIA, and in 8 patients with stage IIB under conditions of in- and out-patient establishments. A favourable effect was produced in 81.5% of patients who were given one tablet of the drug once or twice a day. Treatment with visken-15 led to a decrease in systolic, diastolic and mean arterial pressure, in the rate of cardiac contractions, and in the blood minute volume at rest, in the orthostatic position, and during physical load. Visken-15 is marked by low toxicity, it is tolerated well and seldom causes side effects. The drug is recommended for the treatment of patients with early stages of hypertensive disease.

[Comprehensive study of central and regional hemodynamics in hypertension using bloodless research methods].

Tetrapolar chest rheography is a convenient, noninvasive method for determining the stroke and minute blood volumes. Its application made it possible to reveal various types of hemodynamics in patients with hypertensive disease and apply differentiated treatment with due account for these peculiarities. Study of regional hemodynamics showed a significant increase in the tonus of the arteries of the brain and crura and diminished tonus of the veins. The method is most valuable in a follow-up during treatment and particularly for recording rapid shifts in hemodynamics in functional loads. Assessment of the reactions of patients with hypertensive disease during the orthostatic test revealed various degrees of disorders of the circulatory system adaptation mechanisms; with the progression of the disease these disorders become more intense.

[Cerebral circulatory disorders in patients with cardiovascular diseases accompanied by cardiac rhythm disorders].

The authors examined 180 patients where disorders of cerebral circulation developed on the background of different disturbance of the heart rhythm. The patients suffered from rheumatic heart disease, hypertensive disease or atherosclerosis in concomitance with ischemic heart disease. For comparison, 100 patients with similar disorders were studied, but without disturbances of heart rhythm. It was established that disorders of cerebral circulation were more frequently seen in permanent forms of cardiac fibrillation and less so in paroxysmal cardiac fibrillation and extrasystoles. The most severe disorders of cerebral circulation were seen in combinations of permanent forms of cardiac fibrillation with single and frequent ventricular extrasystoles in patients with rheumatic heart disease.

[Response to the orthostatic test in patients with macrofocal myocardial infarct under different tempo of activization].

In 68 patients with large-focal myocardial infarction subjected to different tempos of activization the orthostatic test was conducted while assuming the vertical position for the first time and prior to being discharged from the hospital. Orthostatic complications and changes in the terminal part of the ventricular complex on ECG were found to be much more frequent in cases of slow tempo activation. A qualitatively different response to the orthostatic test was revealed in myocardial infarction patients with or without circulatory insufficiency. In those with signs of circulatory insufficiency the acceleration of the cardiac contractions rate in the orthostatic position, the shortening of the ejection phase, the reduction of the rheographic amplitude in the lungs and the lower extremities were negligible in comparison with those free of circulatory insufficiency and having statistically significant shifts in the above parameters. The patients with circulatory insufficiency tended to a greater increase of the diastole-systolic ratio of the lower extremities rheogrammes which proves a hindered outflow of blood from this vascular zone. Faster tempos of activization, even in the presence of circulatory insufficiency, helped to prevent the above shifts.

[Determination of cardiac output by the method of tetrapolar chest rheography and evaluation of its metrological possibilities].

Tetrapolar chest rheography is an up-to-day, convenient and bloodless method of determining the cardiac stroke volume (CSV) and cardiac output (CO) in dynamic observations. The paper presents and analysis of the method and considers the possibilities and limits of its application in determining the stroke volume and cardiac output. Advantages of the Kubicek procedure as modified by the authors by comparison with other rheographic methods are shown, along with methodological procedures employed for dtermining the CSV and CO and a nomogram that significantly facilitates the calculation of the values under study is offered. Results of contrasting the cardiac ejection values in patients with congenital heart diseases and an increased pulmonary circulation as against those in patients with ischemic heart disease, obtained by means of tetrapolar chest rheography and direct Fick's method are cited. The possibility of applying tetrapolar chest rheography in assessing the effect operative treatment in patients with congenital cardiac defects and also when investigating compensatory reactions of the cardio-vascular system in patients with hypertensive disease in the course of the orthostatic and Valsalva tests is demonstrated.
