[Effect of a new anti-arrhythmia drug allapinin on hemodynamics in patients with a persistent form of atrial fibrillation before and after restoration of sinus rhythm].

Echocardiographic study was performed in 24 patients with persistent atrial fibrillation (PAF) without clinical signs of circulatory failure. When treated with allapinin , all the patients with PAF showed a significant increase in heart rate (HR) and cardiac output (CO) and a decrease in total peripheral vascular resistance (TPVR). No substantial changes in the major hemodynamic parameters were found in patients with higher left ventricular dimensions; however, a significant rise in end systolic volume (ESV) was noted. There was significantly lower HR, diminished ESV, higher stroke volume and increased CO, elevated ejection fraction and TPVR with sinus rhythm. In PAF patients without apparent signs of circulatory failure, hemodynamic effects of allapinin may be accounted for by its direct vasodilatory action on the arterial bed and by its ability to affect cardiac autonomic innervation. A moderate cardiodepressive effect of the agent may be reflected by deteriorated latent signs of myocardial incompetence which are levelled off following sinus rhythm recovery.

[Changes in systemic and intracardiac hemodynamics during bicycle ergometry in patients with fibrillation arrhythmia treated with allapinin before and after sinus rhythm recovery].

In atrial fibrillation, allapinine was shown to enhance rhythm by 7% and to increase cardiac output (p less than 0.05), as well as to slightly lower mean blood pressure and peripheral vascular resistance at rest. With exercise, both in atrial fibrillation, and sinus rhythm, there was a decrease in end-diastolic and end-systolic volumes of the left ventricle (p less than 0.05), a slight drop in ventricular ejection, that was statistically significant only with sinus rhythm (p less than 0.05). Physical exercise was not followed by an apparent additional aggravation of myocardial contractility, which makes allapinine preferable for long-term application to preserve sinus rhythm in patients without evident signs of heart failure.

[The effect of allapinine on external respiratory function in patients with diseases of the cardiovascular system].

The cardiorespiratory system was studied comprehensively in 27 patients with permanent atrial fibrillation (PAF) without the clinically marked signs of circulatory failure. After allapinine treatment 15 patients with bronchial obstruction manifestations demonstrated, in the presence of lasting PAF, a significant increase of the forced expiration capacity per s and of the maximum volumetric expiration rates. The data obtained may attest to the fact that the drug has beta-stimulating properties, confirming the reported evidence.

[Left ventricular function (based on two-dimensional echocardiographic data) during bicycle ergometry and transesophageal electrical stimulation of the atria in patients with hypertrophic cardiomyopathy].

The central hemodynamics was studied and compared in 20 patients with hypertrophic cardiomyopathy (HCMP) at rest, during bicycle ergometry (BEM) and transesophageal pacing of the atria (TEPA). According to the BEM readings, HCMP patients did not show any material differences from normals' reaction to exercise on the part of the volumetric and hemodynamic parameters as well as left ventricle contractile function. At the same time as revealed by TEPA, HCMP patients did manifest differences from normals' response in the hemodynamics and myocardial contractility. Alterations that occur in HCMP patients undergoing TEPA in response to an isolated increase of frequency of the cardiac rhythm are likely to reflect latent disorders of systolic function of the asymmetric hypertrophied left ventricle which is not capable of making adequate volumetric and functional restructure.

[Functional state of the left and right heart ventricles in chronic alcoholic damage to the heart (findings of two-dimensional echocardiography)].

Twenty eight patients aged 31 to 56 years with Stages II-III chronic alcoholism were examined. All the patients underwent a thorough clinical, X-ray, and instrumental studies which provided the possibility to exclude coronary artery and valve diseases. Two-dimensional echocardiography revealed that chronic alcoholics had profound ventricular dysfunctions. The abnormalities in the right ventricle (its dilation and reduced ejection fraction) occur earlier than in the left one and they become deteriorated with lower left ventricular contractility.

[Determination of the right ventricular volume and ejection fraction by echocardiography (a comparison with angiographic data)].

The determination of function of all cardiac chambers is important in clinical practice. The improvement of the method of two-dimensional echocardiography permitting visualization of the right ventricle (RV) in different topographic projections made possible ultrasonic assessment of RV function. The RV volume and ejection fraction were determined by two-dimensional echocardiography in 25 patients and the results obtained were compared with contrast ventriculography findings. Taking into account the complexity of geometrical shape of the right ventricle its volume based on the results of echocardiography, equaled the sum of the volumes of 3 figures: 2 frusta and one cone. Comparing the results obtained by two methods, significant correlation of all indices characterizing RV function (r-0.85-0.89) was revealed. The prospects of future development of the calculation of RV volumetric indices by the results of two-dimensional echocardiography were discussed. Limitations of the use of this method were described.

[Segmental left-ventricular contractility in patients with hypertrophic and dilated cardiomyopathies].

Segmental left-ventricular contractility was assessed on the basis of computerized analysis of two-dimensional echocardiograms in 35 patients with hypertrophic cardiomyopathy (HCMP), 24 patients with dilatation cardiomyopathy (DCMP), 20 patients with coronary heart disease (CHD) and 30 normal subjects. The HCMP patients showed hyperkinetic segments at different sites of the left ventricle in the presence of a general rise of myocardial contractility. Segmental contractility disorders of DCMP patients were similar to dyssynergia of CHD patients. No hyperkinetic segments were found in DCMP and CHD patients showing signs of heart failure. The detected segmental contractility disorders cannot be used as a criterion in the differential diagnosis of DCMP versus CHD.

[Effect of peripheral vasodilators on pulmonary respiratory function and the indices of bicycle spiroergometry in heart failure patients].

The effect of corinfar and isosorbide on respiratory function of the lungs and indices of spirobicycle ergometry was studied in 40 patients with heart failure resulting from various heart diseases. The results have shown that despite difference in the mechanism of action, the use of these drugs at the given doses combined with digoxin is accompanied by a distinct and comparable positive influence on external respiratory function and indices of spirobicycle ergometry in patients with heart failure.

[Dynamics of respiratory function and and bicycle ergometry in the treatment of patients with chronic circulatory failure].

Data are presented on external respiration at rest and during exercise (bicycle ergometry) in 20 patients with chronic circulatory insufficiency due to a variety of heart diseases, as well as variation in the above in the course of treatment with digoxin and calcium antagonist corinfar. External respiration parameters of the examined patients were compared with those of 40 apparently normal controls. Some mechanisms of action and the effects of digoxin, corinfar and combined use of the two on external respiration parameters are discussed. Evidence is obtained that digoxin and corinfar improve these parameters, particularly when used in combination, therefore combined digoxin/corinfar therapy is certainly advisable as part of combined treatment for chronic circulatory insufficiency.

[Function of the right heart based on 2-dimensional contrast echocardiography data in primary pulmonary hypertension and dilated cardiomyopathy].

The authors provide the results obtained during studies of the function of the right atrium and right ventricle in 15 patients with primary pulmonary hypertension (PPH) and 28 patients with dilated (congestive) cardiomyopathy (DCMP). The studies were based on comparison of the sectional areas during systole and diastole measured by means of two-dimensional contrast echocardiography. Ultrasonic location was performed from the apical position in the four-chamber cross section projection. A study was also made of the effect of the development of relative insufficiency of the tricuspid valve and atrial fibrillation on the function of the right heart. In both the diseases, there was a decrease in the parameters which mirror the function of both right atrium and right ventricle. However, in patients with DCMP, that decrease was coupled with a relatively greater reduction in the function of the right atrium than in patients with PPH. It is possible that such a functional interrelationship between the right atrium and right ventricle presents one of the mechanisms of compensation in the development of circulatory failure in patients with PPH and DCMP.

[Experience with introduction of an instrumental-diagnostic complex in outpatient clinics].

The paper is concerned with introduction under outpatient conditions of an instrumental diagnostic complex including, apart from routine electrocardiographic and x-ray examinations, more specialized techniques, namely phonocardiography and echocardiography. Based on examination of 273 patients with different heart diseases the authors worded indications for echocardiography with emphasis on the necessity of rejecting or confirming the diagnosis, of specifying the diagnosis upon the deciding on the surgical treatment, and so forth. The authors conclude that it is necessary to use the instrumental diagnostic complex under outpatient conditions. They stress, however, that its successful use completely depends on the number of outpatients. The present paper is of interest for physicians specialized in functional diagnosis made in- and outpatiently.

[Right heart function based on 2-dimensional echocardiographic data in heart failure patients].

The right atrium and right ventricle were examined by contrast echocardiography. The function of the right chambers was evaluated by determining the changes in the heart cavity section areas while using echocardiography during different phases of the cardiac cycle. The patients with primary pulmonary hypertension and mitral valve disease and healthy subjects were examined. It was typical of the patients with mitral valve disease to have a marked decrease in the functional parameters of the right atrial fibrillation and diminished right ventricular function indices. The indices of the functional state of the right ventricle in the patients with primary pulmonary hypertension changed to a less extent despite the fact that they had a more pronounced dilatation of the right ventricle than those with mitral valve disease.

[Comparative study of the action of a series of new beta-adrenergic receptor blockaders in patients with effort angina].

In 32 patients with angina of effort, the antianginal effects and action duration of propranolol, atenolol, labetalol and nadolol were compared during the exercise test after a single dose of the drug. Atenolol and nadolol have been ascertained to be effective long acting antianginal drugs. Except the antianginal effect duration, the above mentioned beta-blockers have not been found to be superior to propranolol, including to atenolol, to its effect on the total peripheral vascular resistance. Labetalol provided a sufficient antianginal effect in a number of patients, including those in whom propranolol was not effective enough. It have been found that high incidence of side effects makes it inexpedient to use high doses of labetalol (more than 200 mg) in patients with angina pectoris who have normal blood pressure.
