[The status of cardiovascular system in patients with acute cholecystitis and changes of hemodynamics depending on technology use during operative intervention].

Intraoperative clinico-instrumental monitoring of hemodynamic reactions was conducted in 56 patients with an acute cholecystitis during performance of laparoscopic cholecystectomy, cholecystectomy with application of upper median laparotomy and mini-invasive subcostal access. Cardiodepressive reactions were observed after creation of carboxiperitoneum. The differential choice of technology of cholecystectomy performance in patients with attendant diseases of cardio-vascular system is necessary. Advantages of "open" interventions were established.

[Topographic-anatomic basis for the access choice during cholecystectomy for acute cholecystitis].

Comparative topographoanatomic investigation of parameters of accesses during performance of open and laparoscopic interventions was conducted. There was established, that for an acute cholecystitis, complicated by peritonitis, the upper middle laparotomy constitutes an adequate access. Application of laparoscopic technologies for cholecystectomy performance is expedient if the inflammatory changes are restricted by gallbladder wall.

[The assessment of the external respiration function in patient with acute cholecystitis depending on technical aspects of cholecystectomy].

Comparative analysis of changes of the external respiration function after cholecystectomy in 92 patients with an acute cholecystitis was done. In 33 patients cholecystectomy was performed, using upper midline access, and in 27--applying miniinvasive subcostal access, in 32--laparoscopic cholecystectomy. Disorders of the external respiration function were the most prominent after performance of open cholecystectomy, and they were minimal--after application of miniinvasive subcostal access.

[Diagnostic and therapeutic punctures of pancreatic cavities under control of ultrasonic investigations].

Transcutaneous puncture of pancreatic cavities under ultrasonographic control was conducted in 68 patients. The cavity contents investigation using cytological, bacteriological and biochemical methods was conducted. The method permits to diagnose the cavity, its connection with ductal system of pancreas, to select the adequate antibacterial therapy, to conduct the cavity decompression before the operation. It may be applied for symptomatic treatment conduction. There were no complications observed as a consequence of the procedure.
