[Effectiveness of pathogenetic treatment of arterial obliterative diseases of the extremities].

The authors studied the activity of blood serum amine oxidase in patients with atherosclerosis obliterans and endarteritis obliterans. It was found that amine oxidase activity exceeded the normal value. 2.6 times in patients with stage 1-2 atherosclerosis obliterans and was reduced by more than 12 times in patients with stage 3-4 of the disease. The activity of the enzyme was absent in patients with endarteritis obliterans, whatever the stage of limb ischemia. On the basis of the obtained facts, the authors voice a new opinion on the nature of hypercatecholaminemia in obliterative diseases and suggest a new trend in the treatment of chronic ischemia of the limbs-activation of the processes of catabolism of vasoconstrictive biogenic amines.

[The effect of HBO therapy in the early postoperative period on the late outcomes of "compelled" suturing in perforated gastroduodenal ulcers].

Under study were long-term results of forced suturing of perforated ulcers of the stomach and duodenum in 96 patients. It was shown that in patients who were subjected to hyperbaric oxygenation in addition to medicamentous treatment at the early postoperative period excellent and good results were obtained two times more often and there was 1.5 time less amount of bad outcomes as compared with patients who were not given HBO therapy. A conclusion is made that HBO therapy has a positive influence at the early postoperative period on restoration of the disturbed balance in the system "aggression-protection" in ulcer disease which results in greater number of patients who recovered after suturing perforated gastroduodenal ulcers.

[The structure of endogenous intoxication in acute intestinal obstruction and the methods for its correction].

The structure of endogenic intoxication by products of the intermediate metabolism was studied in 32 patients with intestinal obstruction. The leading components of the endogenic intoxication are: acetone, propionic aldehyde and ethanol. In cases of a complicated course of the intestinal obstruction the operative treatment by traditional therapy in the postoperative period proved to be insufficient. Sessions of the intravascular laser irradiation of blood reducing the concentration of acetone and propionic aldehyde were performed in 19 patients without any changes in the concentration of ethanol.

[The prognosis of the course of endogenous intoxication in emergency surgery].

On the basis of an analysis of 256 patients with diffuse peritonitis, destructive pancreatitis, mechanical jaundice and purulent cholangitis and complex investigations it was established that the degree of clinical manifestations of endointoxication corresponded to a definite degree of disorders of the central nervous system. The most adequate indices of the course and prognosing the toxic process are thought to be the level of the leukocytic index of intoxication, the prothrombin index, the degree of monocytopenia and proteinuria. Their dynamics reflects changes of the concentration of toxic intermediary metabolites and main characteristics of electron paramagnetic resonance of blood plasma.

[Endogenous toxemia in pancreatic necrosis of alcoholic etiology].

Operation for pancreonecrosis was carried out in 197 patients, in 52 (26.4%) of them the disease was of alcoholic etiology. The disease was marked by severe clinical manifestations with the development of respiratory, hemodynamic, and peritoneal syndromes and their complications, and by psychic disorders. The endo-intoxication structure was studied in 13 patients by mass-spectrometry proceeding from alcohol metabolism in the organism. Eleven intermediate products were identified: ethanol, acetaldehyde, normal butyraldehyde and isovaleric aldehyde, diethylamine, acetone, isopropanol, methyl isocyanide, trimethylamine, ethannitrile. It was noted that the clinical manifestations of alcoholic pancreonecrosis are included in the spectrum of ethanol pharmacological action; the content of ethanol in the blood was tenfold of the normal, that of the product of its metabolism acetaldehyde was 1.6 times the normal level. Inclusion of exogenic ethanol in metabolism leads to its distortion and accumulation of substances of different origin in the organism, among which an important role is played by acetaldehyde, normal butyraldehyde, and isovaleric aldehyde. The results of the study create proconditions for the search for a rational means for their desactivation.

[Indicators of the sympathetico-adrenal system in patients with peritonitis after hyperbaric oxygenation].

Hyperbaric oxygenation was used in 32 patients with acute peritonitis and terminal phases of the disease. Changes in the sympatho-adrenal system indices in the blood and excretion of catecholamines with urine during HBO course were investigated. The general pool of adrenaline and noradrenaline in the blood was shown to decrease, their excretion with urine to increase. Normalization of biosynthesis of precursors under the influence of HBO sessions was found.

[The structure of endotoxemia in peritonitis and means of eliminating it].

An examination of 43 patients has revealed 11 toxic compounds of intermediary metabolism (aldehydes, methylizocyanide, ethanol, aceton etc.) whose concentration in blood became several tens times higher. Their accumulation was most considerable at the decompensation phase. It was shown that hemosorption and hyperbaric oxygenation made the concentration of toxic substances lower.

[The kallikrein-kinin system in acute peritonitis and methods of correcting its disorders].

In experimental fecal peritonitis in 140 albino rats the kallikrein-kinin system was shown to undergo the following changes: its inhibition during the first day was followed by an activation on the second day and inanition on the third day. Under conditions of hyperbaric oxygenation the normalizing effect on the kinin system was observed during two days. The application of salicylates against the background of HBO was shown to enhance the normalizing effect which lasted for three days.

[Importance of the indices of immunity for predicting the course and assessing the effectiveness of the treatment in acute suppurative lactation mastitis].

The examination of 47 patients with acute purulent lactation mastitis and 14 patients with the resulting sepsis has shown a decreased level of nonspecific resistance factors. Generalization of the purulent process is characterized by disturbed humoral and cellular links of immunity. The dynamic research of immunity factors can give an estimation of the effectiveness of the chosen treatment and prognosis of possible complications. The general bactericidal activity of blood may be considered one of the most informative indices showing the direction of the process.

[Endogenous toxicity in acute hepatic failure and ways of eliminating it].

The structure of causes of the endogenous intoxication in 39 patients with acute hepatic insufficiency is studied. Eleven toxic compounds of an organic nature were found responsible for genesis fo the disorders and outcomes of acute hepatic insufficiency. Possible use of hemosorption and hyperbaric oxygenation for the elimination of toxic products is analysed. A combination of them is proved to be expedient. The selectivity of sorption for each toxin separately in various types of hepatic insufficiency is established.

[Effect of hyperbaric oxygenation on delayed hypersensitivity and antibody formation in patients with peritonitis].

The inhibition of the formation of hypersensitivity of delayed type to staphylococcus, Proteus, blue pus bacillus and Escherichia coli was found in patients with peritonitis given complex treatment including hyperbaric oxygenation, against the background of stimulation of the antibody formation to these bacteria. The level of staphylococcal alfa-antitoxin in the blood serum remained substantially lower than that of healthy people whether the treatment included HBO or not. The data obtained show the expediency of early use of HBO in patients with peritonitis.
