[The prognosis of extracapsular extraction methods in cataract].

For the purpose of prognosticating the method of extracapsular cataract extraction ultrasound, quantitative echography (a-method) was conducted in 207 patients in the preoperative period. The value of ultrasound attenuation in decibels in lenses of different density was determined by a tentative way. It is established that the most optimum condition for aspiration is "acoustic lens density" to 9 decibels, for phacoemulsification--from 10 to 15 decibels, for usual extracapsular cataract extraction--above 15 decibels and higher. In all patients during the operation the results of quantitative echography were compared with the real density of the lens. In all cases full coincidence of "echographic" and real density was recorded. The proposed method is objective and the obtained figure values allow to recommend the method of extracapsular cataract extraction.

[The efficacy of the antioxidant preparation Histochrome in the treatment of hemophthalmos in hypertension and diabetes mellitus].

Hemophthalmos in patients with essential hypertension and diabetes mellitus was treated by antioxidant agent Histochrome manufactured by the Pacific Institute of Bioorganic Chemistry, Far Eastern Division of Academy of Sciences, Vladivostok. The results were analyzed with reference to localization, volume, and period elapsed since hemorrhage into the vitreous body and by hemophthalmos index determined by biomicroscopy and echography of the vitreous body. Histochrome was the most effective in fresh hemophthalmos with 1-2 point index. Visual functions improved in patients with nonproliferative diabetic retinopathy without hemorrhagic syndrome.

[Topography and angioarchitectonics of the orbital portion of the optic nerve according to ultrasonic findings and data of three-dimensional optic analysis].

Super-sharp gray scale image, highly sensitive digital wide-band Doppler analysis, three-dimensional visualization of vessels in pulsed energy Doppler mode, and three-dimensional optic tomography of the optic disk were used in examination of the posterior segment of the eye in 20 healthy volunteers (40 eyes) aged 20-50 years. Vascular network of the optic nerve was examined under clinical conditions; blood filling of elements of the Zinn-Haller arterial circle, reticular plate, and posterior short ciliary arteries were studied. Vascular architectonics of the retrobulbar space was studied by three-dimensional reconstruction in the energy Doppler mode and biometrical parameters of intraocular and retrobulbar part of the optic disk were determined.

[Acoustic studies in proliferative diabetic retinopathy].

Based on analysis of published reports and his own experience of acoustic studies (B-method) in more than 500 patients with proliferative diabetic retinopathy, the author concludes that preretinal fibrovascular membranes (posterior detachment of the hyaloid membrane with neovascularization) are often mistaken for retinal detachment. Acoustically detected solitary or multiple adhesions between fibrovascular and eye membranes are an important differential diagnostic sign indicating the absence of retinal detachment. These adhesions result from gradual staged "creeping" posterior vitreal detachment and indicate the sites of previous contact of the hyaloid membrane with the retina. In subtotal detachment of the retina there are no adhesions with ocular membranes and the retina acquires the configuration of a rigid fibrovascular membrane.

[Ultrasound in the diagnosis of macular pathology of the eye].

MARK-500 gray-scale digital processing B scanner (ATL) with a probe frequency of 7.5 MHz was employed in examinations of 112 patients with various clinically verified macular conditions. B scanorgams were normal in nearly all dry maculodystrophy cases. In manifest fibrous exudative maculodystrophies the scanograms showed local thickening (up to 2.6 mm on an average) of the inner membranes (edema), dense sites (fibrosis, lipid and protein substance deposits). Only in several Irvin-Hass' syndrome cases with manifest cystous macular edema the scanograms showed local thickening of the membranes. Local detachment of the retina in the macular area was well seen, as was premacular fibrosis. Sub- and proretinal hemorrhages, neovascular membranes, poorly manifest fibrosis within the membranes were as a rule undetectable by echography. In cases with impaired transparency of the optic media or when the pupil cannot be dilated, present-day B echo scanning may be employed to detect grave abnormalities in the macular area.

[Hemophthalmos and posterior detachment of the vitreous body (an ultrasonic study)].

A total of 133 patients with hemophthalmos and its aftereffects have been examined with the use of a digital processing gray scale ultrasonic B-scanner. Posterior detachments of the vitreous body (PDVB) have been revealed in 45 (33.8%) patients. In half of these patients the hyaloid membrane has been found bound to the optic disk or to the retina; this bond indicates the site of the hemorrhage from the injured vessel. V-shaped PDVB, connected with the optic disk, may be difficult to differentiate from a detachment of the retina. Three major factors should be borne in mind in such a differential diagnosis: the amplitude of the echo signal, the detachment topography and mobility. In 5 patients PDVB were combined with detachments of the retina. In 9 case PDVB have been a consequence of a retrovitreal hemophthalmos.
