[Role of general somatic hospitals in replenishment of contingents of antituberculosis dispensaries].

The patients whom tuberculosis was detected at general somatic hospitals form 28% in the make-up of dispensary registry group IA. In these hospitals BC are found in those with suspected tuberculosis and even in its destructive forms (4-5.7%). Activization of explanatory work with physicians and nurses at a hospital has caused an increase in positive sputum tests for BC and favoured a 7-fold increase in their detection by bacterioscopy after Ziehl-Neelsen.

[Formation and sociomedical characteristics of antituberculous dispensary contingents registry group IA patients].

There are 77.1% of new cases in antituberculous dispensary registry group IA. The proportion of patients diagnosed at hospitals of different disciplines has been higher that of patients identified at preventive fluorography. In 18.3% of cases, group IA has been replenished by patients with exacerbation and recrudescence; 4.6% has come from imprisonments, 17.8% of antituberculous dispensary registry group IA patients are socially disadaptive. The most severe destructive forms of tuberculosis with bacterial isolates detected at bacterioscopy are common mainly in patients whose disease has been diagnosed due to pulmonary complains in polyclinics and hospitals. Various concomitant diseases are present in 82.3%, 10.3% of patients are followed up in group IA for more than 3 years.

[Characterization of epidemic foci of tuberculosis and improvement of the follow-up of children and adolescents from the environment of patients with tuberculosis].

Despite unfavorable socioepidemiological conditions, there are potentials of reducing tuberculosis morbidity in the epidemic foci of children and adolescents (by more than 3.5 times). For this, all the children and adolescents from the foci of infection should be thoroughly examined and followed up and educational and preventive measures including intensified antituberculosis propaganda should be made in these foci.

[Experience in using serological tests in detecting tuberculosis in patients with severe mental pathology].

113 patients with severe mental diseases were examined for tuberculosis using enzyme immunoassay which provided positive or doubtful results in 15% of the examinees. At clinicoroentgenological examination the patients were found to have active respiratory tuberculosis, foci of indefinite activity and posttuberculous changes in the lungs and pleura in 4.4, 3.5 and 7.1%, respectively.

[Factors facilitating the development of tuberculosis in pregnant women and puerperae and the role in their examination of contact film thermal indication].

The rate of tuberculosis detection in pregnant women and puerperae in Moscow 1.5 to 2.5 times surpassed the average incidence of tuberculosis in women in 1988-1992. Patients falling ill during or soon after pregnancy more frequently developed acute and complicated forms of the process similar by their origin to primary disease. Contact film thermoindication became an effective method for the detection of respiratory diseases in pregnant women (3/4 clinical thermoscopic coincidences). Among pregnant women, 6.5% are at risk of tuberculosis. These are women previously registered at tuberculosis control centers or with a history of contacts with tuberculosis patients, alcohol abusers, or women with stable thermoasymmetry.

[Pregnancy and delivery as risk factors for tuberculosis].

A total of 1,474 females who fell ill with tuberculosis in the reproductive age in 1988-1992 (12% of them fell ill during pregnancy and after labour) and 690 pregnants were examined. The tuberculosis detection rates in pregnants and parturients in Moscow were 1.5-2 times higher than the average rates in the above period. The women who fell ill during pregnancy and just after labour frequently developed acute, frequently complicated forms of the process, which were close to the primary forms in their genesis. Contact film thermoindication is an effective method of detecting respiratory diseases in pregnant females (3/4 of clinical and thermoscopic coincidences). A risk group for tuberculosis was identified among the pregnants, which was 6.5% of the examinees: those who had been registered at the tuberculosis dispensary or had contacts with patients; those who suffered from chronic non-specific respiratory diseases or had steady-state thermal asymmetry.

[The role of groups with high risk of tuberculosis in developing patients' contingents in the anti-tuberculosis clinics].

The role of the highest tuberculosis risk groups in the enlargement of dispensary contingents increases under conditions of endemic tuberculosis decline while it is less manifest in the epidemiologically unfavourable regions. Risk contingents of polyclinics should be supplemented by a socially unfavourable group of subjects who together with patients having several risk factors and those with severe diabetes mellitus are the most vulnerable with respect to tuberculosis.

[Prevention of tuberculosis in patients in psychiatric hospitals].

Mental hospital patients are at high risk of tuberculosis reactivation, especially those with posttuberculous changes in the lungs and progressive severe schizophrenia. Outbreaks of tuberculosis are 2.5 times more frequent in them than in alcohol abusers and other mental patients. In subjects with posttuberculous changes on the lungs preventive chemotherapy can cause a 6-fold decrease in the likelihood of process reactivation.
